Use of a minicondylar plate for metacarpal and phalangeal periarticular injuries.

The minicondylar plate fixation system is a new method for stable internal metaphyseal bone fixation about the metacarpophalangeal (MP) and proximal interphalangeal (PIP) joints. Indications for its use are rigid stabilization of fracture, osteotomy, and the bony components of composite tissue transfer. The device should be used in deference to other techniques when early active range of motion is pursued. Its best use is in the face of combined injury or osteotomy with tenolyses/capsulotomy. This implant permits stabilization of small epiphyseal fragments with minimal interference of joint fu

[One-stage reconstruction of intraorbital fractures by means of plate osteosynthesis (author's transl)].

Considering the multiplicity of sequelae of central midface fractures (rhinoliquorrhea, anosmia, impediment to the drainage of the superior sinuses and to the lacrimal fluid, traumatic telecanthus, dish-face etc.) the importance of rhinosurgical treatment as the foremost procedure is shown. The small-fragment sets according to Champy as stabilizing elements in operations of the interorbital space (Converse) and its osseous supports are mentioned. Both the technical and the functional advantage of establishing a stable osseous trajectory in the rhinobasis after fractures is emphasized. Stable o

[Treatment of mandibular fractures in children. Our therapeutic approach (author's transl)].

This is a study of 15 fractures of the mandible in children. We were able to make the following conclusions: --the usual causes of these fractures are road traffic accidents or accidents at play, --the fracture is usually single (7 cases) multiple (8 cases) and in 13 children out of 15 there was a fracture of the condyle. --the presence of the first dentition and dental germs posed special problems. The treatment is mainly orthopedic, and the fracture is maintained in place by metallic arches which may or may not be completed with a resin dropper. Condyle fractures are treated classically by f

[Evaluating the success of bringing the ends of fractured lower jaw bones into close apposition by means of plates and bolts (author's transl)].

The present paper analyzes the results of 100 osteosyntheses performed on 79 patients, with the ends of fractured lower jaw bones having been brought into close appostion through the use of plates and draw-in bolts. The major emphasis of this study centered on localizing the fracture and determining the etiology, the measures to be taken before performing an operation, the time of operation, and the duration of hospitalization.--Inflammatory disturbances of fracture healing were observed in 4 percent of the cases, and malocclusion was determined for another 4 percent of the cases included in t

Fracture-dislocation of the mandibular condyle: report of case.

A rare type of ramus fracture-dislocation has been presented. The inadequacy of standard mandibular fracture radiographic views for proper diagnosis of variable dislocations has been stressed. It was noted that proper positioning of the mandible in panoramic radiography would facilitate a satisfactory radiographic survey of the glenoid fossa and the condyle. In this case, interpositional silastic gap arthroplasty was performed to correct an extraarticular ankylosis and to restore mandibular function.

Update on leg fractures in paediatric patients.

Leg fractures are common and further increasing in prevalence in paediatric patients. The diagnosis is readily made in most cases. Choosing the best treatment is the main issue. Non-operative treatment is the reference standard for non-displaced or reducible and stable fractures but requires considerable expertise and close monitoring, as well as an immobilisation period that far exceeds 3 months in many cases. Some surgical teams therefore offer elastic stable intra-medullary nailing (ESIN) as an alternative to children who do not want to be immobilised for several months. Internal fixation i

Two case reports of a technique of medial external fixation in calcaneal fractures: indirect control of the sustentacular fragment.

In attempted open reduction and internal fixation of displaced calcaneal fractures, comminution of the sustentacular fragment can pose major problems. Two cases of a form of external fixation are presented. The method takes advantage of the strong superomedial and plantar calcaneonavicular ligaments, which link the sustentaculum to the navicular. Traction along these structures (by pins placed in the first metatarsal) indirectly controls the sustentacular fragment, while countertraction from medially placed pins in the tuberosity allows derotation of the fragment and correct repositioning in r

An Innovative Technique for Intraoperative Radiological Assessment of Pelvic and Spine Fractures in Austere Environment.

INTRODUCTION: Managing pelvic and spine fractures in the austere environment can be challenging even for skilled orthopedic surgeons, largely due to the difficulty of radiological visualization of bone structures. We present a simple alternative to the metallic operating table by placing the patient on a spinal board that will allow for a better radiological assessment of these fractures. MATERIAL AND METHODS: A cross-sectional, descriptive, and retrospective study was carried out in the period between 2015 and 2020. The study population was all patients with pelvic o spine fractures, managed

[POSTERIOR MINIMALLY INVASIVE APPROACH FOR RECONSTRUCTION OF SCAPULA OF FRACTURES].

OBJECTIVE: To investigate the effectiveness of posterior minimally invasive approach for internal fixation of displaced scapula fractures. METHODS: Between January 2006 and December 2011, 16 patients with scapular fractures underwent surgical fixation by a minimally invasive approach, including 11 cases of displaced glenoid fractures and 5 cases of unstable scapular necklbody fractures. There were 12 males and 4 females, aged 35-69 years (mean, 53 years). The causes of injury were traffic accident in 10 cases, falling from height in 4 cases, and tumble in 2 cases. In 11 cases of displaced gle

[Research progress of femoral neck system in treatment of femoral neck fracture in young and middle-aged patients].

OBJECTIVE: To summarize the research progress of femoral neck system (FNS) in the treatment of femoral neck fracture in young and middle-aged patients. METHODS: The literature on FNS at home and abroad in recent years was extensively reviewed, and the results of mechanical and clinical studies on FNS were summarized based on clinical experience. RESULTS: FNS has good mechanical stability, which can reduce complications such as femoral neck shortening, internal fixation failure, and varus caused by mechanical instability. At present, FNS is mainly selected for comparison with cannulated compr

Non -Union of Lateral Hoffa Fracture - A Case Report.

INTRODUCTION: External fixation, various subcutaneous screw fixations, and plate fixation can be considered as fixation methods for unstable pelvic ring fractures. We describe a first case of treated unstable pelvic ring injury using a dual internal anterior subcutaneous fixator we called "dual INFIX," comprising four screws, two subcutaneous rods, and two cross-link connectors, without posterior fixation. CASE REPORT: An 81-year-old man sustained an unstable pelvic injury (AO type B2) with fracture of the left ilium and pubis. Dual INFIX was used to stabilize the pelvic ring injury. Polyaxia

Self-designed posterior atlas polyaxial lateral mass screw-plate fixation for unstable atlas fracture.

BACKGROUND CONTEXT: Most atlas fractures can be effectively treated nonoperatively with external immobilization unless there is an injury to the transverse atlantal ligament. Surgical stabilization is most commonly achieved using a posterior approach with fixation of C1-C2 or C0-C2, but these treatments usually result in loss of the normal motion of the C1-C2 and C0-C1 joints. PURPOSE: To clinically validate feasibility, safety, and value of open reduction and fixation using an atlas polyaxial lateral mass screw-plate construct in unstable atlas fractures. STUDY DESIGN: Retrospective review
