Apatinib monotherapy for pretreated advanced squamous cell carcinoma of the penis: a phase 2 trial.

BACKGROUND: Treatment options for patients with advanced penile squamous cell carcinoma who have disease progression on systemic chemotherapy are scarce. Antiangiogenic drugs have shown antitumor effects in case series. This phase 2 trial evaluated the activity and safety of apatinib (a Vascular Endothelial Growth Factor (VEGF) receptor inhibitor) in such patients. METHODS: This investigator-initiated, single-arm, phase 2 trial was registered at the University Hospital Medical Information Network Clinical Trials Registry (UMIN000021849) and performed at a tertiary cancer center in Shanghai, C

Novel subtypes of metabolic associated steatotic liver disease linked to clinical outcomes: implications for precision medicine.

BACKGROUND & AIMS: Although metabolic dysfunction-associated steatotic liver disease (MASLD) is associated with high multimorbidity and mortality, existing classification systems and risk prediction models largely ignore the heterogeneity of MASLD. Improved subtype definition could improve prediction of outcomes and inform new precision treatment strategies. METHODS: We analyzed individuals with MASLD from population-based electronic health record resource from UK Biobank (n = 125,197) and Health examinee dataset of Nanfang Hospital (n = 995). We identified subtypes with K-means clustering me

Short-term prognostic score for very elderly patients, aged 85 or older, initiating hemodialysis.

BACKGROUND: As the population ages, factors affecting prognosis after dialysis initiation in very elderly patients remain unclear. We aimed to develop a prognostic score to predict short-term survival in this group. METHODS: This retrospective cohort study included 116 patients aged 85 or older who initiated hemodialysis at our hospital between 2010 and 2023, out of a total of 2279 patients. Using a Cox regression model, we analyzed factors affecting 3-year all-cause mortality. Based on these analyses, a prognostic scoring system was developed to predict patient outcomes. RESULTS: Baseline f

A qualitative exploration of STI partner notification services delivery models among key stakeholders in rural southwestern Uganda.

BACKGROUND: Comprehensive STI care requires not only patient treatment but STI partner notification (PN) and evaluation to prevent ongoing STI transmission and reinfection. In rural, southwestern Uganda, we explored healthcare practitioners' views on three STI PN models that focused on task-shifting to non-physician practitioners to inform the development of a novel STI PN services delivery system. METHODS: From September to November 2023, we conducted individual in-depth interviews with 32 participants from 4 categories (8 participants from each category): patients with a self-reported histo

Unraveling the COVID-19 Severity Hubs and Interplays in Inflammatory-Related RNA-Protein Networks.

The rapid worldwide transmission of coronavirus disease 2019 (COVID-19), caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), has led to severe cases of hypoxia, acute respiratory distress syndrome, multi-organ failure, and ultimately death. Small-scale molecular interactions have been analyzed by focusing on several genes/single genes, providing important insights; however, genome-wide multi-omics comprehensive molecular interactions have not yet been well investigated with the exception of GWAS and eQTLm, both of which show genetic risks. From April of 2020 until now, we h

Measuring economic burden in families of individuals with Angelman Syndrome in Poland: a caregivers' survey.

BACKGROUND: Angelman Syndrome (AS) is a rare neurogenetic disorder characterized by severe intellectual disability, seizures, and motor and speech impairments, requiring lifelong, intensive care. Although caregivers of individuals with AS often face reduced quality of life and substantial emotional and financial strain, little is known about the cost-of-illness in Poland. To address this gap, we conducted a cross-sectional study in collaboration with the Foundation for Angelman Syndrome Therapeutics Poland (FAST Poland). An anonymous web-based survey was completed by 85 parents of individuals

Recanalization of the Portal Vein and Transjugular Intrahepatic Portosystemic Shunt (PVR-TIPS) as an Aid for Live Donor Liver Transplantation in Cirrhotic Patients with Portal Vein Thrombosis.

Purpose: Portal vein thrombosis (PVT), is seen in about 25% of patients with cirrhosis. Chronic portal vein thrombosis can significantly alter anatomy, often leading to the diversion of splanchnic blood into expansive and compliant vascular channels. This process generates extensive collateral networks and large varices that function as portosystemic shunts. Portal cavernous transformation represents a critical vascular condition marked by the formation of a network of collateral veins that develops to bypass an obstructed portal vein [PV]. Given these physiological changes, performing liver t

People and sites as community resources for preventing and managing chronic health conditions: A conceptual analysis.

The prevention and management of chronic non-communicable diseases (NCDs) require community-based efforts-especially as their burden grows, and recognition of the need for engaging diverse stakeholders in care grows. The aim of this conceptual analysis was to categorise existing community resources (people and sites) used to support such efforts, the mechanisms by which they work, and the conditions that constrain their effectiveness. We developed an initial framework for categorising community resources. We then used a sample of the literature- 24 studies from 12 countries-to refine and enric

Combined Systemic Immune-inflammatory Index (SII) and Geriatric Nutritional Risk Index (GNRI) predict survival in elderly patients with hip fractures: a retrospective study.

PURPOSE: The Systemic Immune-inflammatory Index (SII) and Geriatric Nutritional Risk Index (GNRI) have undergone comprehensive examination and validation in forecasting the outcomes of diverse medical conditions. Nevertheless, the correlation between the combined use of GNRI and SII metrics and hip fractures has yet to be elucidated. This study aimed to determine whether the amalgamation of SII and GNRI scores constitutes an independent prognostic factor for elderly patients with hip fractures. METHODS: We conducted a retrospective analysis of elderly patients admitted to our facility with hi

A review of climate change and cardiovascular diseases in the Indian policy context.

There is growing evidence that climate change adversely affects human health. Multiple diseases are sensitive to climate change, including cardiovascular diseases (CVDs), which are also the leading cause of death globally. Countries such as India face a compounded challenge, with a growing burden of CVDs and a high vulnerability to climate change, requiring a co-ordinated, multi-sectoral response. In this framework synthesis, we analysed whether and how CVDs are addressed with respect to climate change in the Indian policy space. We identified 10 relevant national-level policies, which were an

Computerized analysis of hypomimia and hypokinetic dysarthria for improved diagnosis of Parkinson's disease.

BACKGROUND AND OBJECTIVE: An aging society requires easy-to-use approaches for diagnosis and monitoring of neurodegenerative disorders, such as Parkinson's disease (PD), so that clinicians can effectively adjust a treatment policy and improve patients' quality of life. Current methods of PD diagnosis and monitoring usually require the patients to come to a hospital, where they undergo several neurological and neuropsychological examinations. These examinations are usually time-consuming, expensive, and performed just a few times per year. Hence, this study explores the possibility of fusing co

Patient-Reported Outcomes and Computed Tomography Review After Minimally Invasive Fusion of the Sacroiliac Joint With Aggressive Joint Decortication and Joint Compression.

The sacroiliac joint (SIJ) is a common, underrecognized source of low back pain. We evaluated outcomes in patients undergoing sacroiliac joint fusion (SIJF) using a novel, minimally invasive SIJF system emphasizing compressive forces across an aggressively debrided SIJ. We retrospectively reviewed data from a continuous set of patients presenting to a large, tertiary care hospital from September 2017 to August 2019. All patients received the novel SIJF device. Outcomes were assessed at 8 weeks, 6 months, and 12 months using the Oswestry Disability Index (ODI) score, Numerical Rating Scale (NRS

Pattern and distribution of prenatally diagnosed congenital anomalies among high risk pregnant women in Ibadan, South Western Nigeria.

INTRODUCTION: Congenital anomalies (CA) are structural or functional disorders present at birth. Routine prenatal ultrasound screening has become an indispensable tool for early detection of CA in developed countries which will facilitate appropriate preemptive actions for safe guarding the health of both mother and the unborn fetus. The prevalence of CA in the general population has been researched widely but very few studies exist on the prevalence of CA among high risk pregnancies. Aims and objective: the aim of this study was to determine the prevalence and pattern of congenital anomalies
