Immunodeficiencies.

Primary immunodeficiencies (PIDs) are uncommon, chronic and severe disorders of the immune system in which patients cannot mount a sufficiently protective immune response, leading to an increased susceptibility to infections. The treatment of choice for PID patients with predominant antibody deficiency is intravenous immunoglobulin (Ig) replacement therapy. Despite major advances over the last 20 years in the molecular characterization of PIDs, many patients remain undiagnosed or are diagnosed too late, with severe consequences. Various strategies to ensure timely diagnosis of PIDs are in plac

Pilot study of vaccination with recombinant CEA-MUC-1-TRICOM poxviral-based vaccines in patients with metastatic carcinoma.

PURPOSE: Poxviral vectors have a proven safety record and can be used to incorporate multiple transgenes. Prior clinical trials with poxviral vaccines have shown that immunologic tolerance to self-antigens can be broken. Carcinoembryonic antigen (CEA) and MUC-1 are overexpressed in a substantial proportion of common solid carcinomas. The primary end point of this study was vaccine safety, with immunologic and clinical responses as secondary end points. EXPERIMENTAL DESIGN: We report here a pilot study of 25 patients treated with a poxviral vaccine regimen consisting of the genes for CEA and M

Suppression of murine allergic contact dermatitis by CTLA4Ig. Tolerance induction of Th2 responses requires additional blockade of CD40-ligand.

Blockade of costimulation through the B7-CD28 pathway by CTLA4Ig can lead to Ag-specific T cell tolerance. Most models studied to date involve a Th1-dependent response. To investigate whether the tolerizing effects of CTLA4Ig might vary depending upon the cytokine nature of the immune response, we studied its effects on contact hypersensitivity (CHS) in response to two allergens. In BALB/c mice, both 2,4-dinitrofluorobenzene (DNFB) and FITC induce CHS. However, the DNFB response is Th1-predominant, while the FITC response is Th2 predominant. CTLA4Ig treatment during primary sensitization induc

Immunology of chronic hepatitis.

Of the two major types of chronic active hepatitis, that associated with autoimmunity, CAH-Type A, has been the more extensively studied and is better recognised. On the other hand, the type associated with the HBV carrier state, CAH-Type B, is the more prevalent, particularly so in South East Asia, and the time is ripe for the immunology of this type to be clarified. Whilst most encounters between host and HBV will result in an acute hepatitis with lasting immunity, a non-sterilizing outcome with a persisting carrier state may ensue. The reasons for persistent infection with HBV are still not

Persistent inflammation, immunosuppression, and catabolic syndrome: A new look at a growing problem.

Nutrition support among critically ill patients in the intensive care unit remains a cornerstone to standard-of-care strategies. Clinicians rely on a robust body of literature, societal guidelines and recommendations, evidence-based protocols, and more to aid in the appropriate delivery of nutrients to this patient population. Among critically ill patients, there is a growing population of patients with smoldering multiorgan dysfunction (MOD) known as persistent inflammation, immunosuppression, and catabolic syndrome (PICS-1). The growth of this patient population is likely related not only to

Bushen Xiaozheng Decoction Improves Immunosuppression in a Rat Model of Endometriosis by Reducing IL-10 and TGF-β Levels.

PROBLEM: How does the traditional Chinese compound Bushen Xiaozheng Decoction (BSXZD) affect the immunosuppressive microenvironment in rat endometriosis models? METHODS: Sprague Dawley rats were randomly divided into normal, sham operation, model, Yasmin, and low-, medium-, high-dose BSXZD groups with 10 rats in each group. The endometriosis model was established by autologous endometrial transplantation, and corresponding interventions were given for 28 days. The volume of ectopic lesions was measured, and histological changes of endometrium were observed by hematoxylin-eosin staining. The p

A wave of Thetis cells imparts tolerance to food antigens early in life.

In the intestine, peripherally induced regulatory T (pTreg) cells play an essential role in suppressing inflammatory responses to food proteins. However, the identity of the antigen-presenting cells that instruct food-specific pTreg cells is poorly understood. We found that a subset of Thetis cells, TC IV, is required for food-specific pTreg cell differentiation. TC IV were almost exclusively present within mesenteric lymph nodes, suggesting that their presence underlies the phenomenon of oral tolerance. A wave of TC IV differentiation in the periweaning period was associated with a window of

Helicobacter pylori outer membrane vesicles mediate central tolerance in C57BL/6J mice offspring T cells via maternal-fetal transmission.

Outer membrane vesicles (OMVs) released by Helicobacter pylori (H.pylori) can enter the blood circulation of the host and cause extra-gastric lesions such as atherosclerosis and hyperemesis gravidarum. This study aimed to investigate the effect of OMVs released by H.pylori on the development of thymic T cells in offspring mice and its underlying mechanisms. Through experimental observations, we found that H.pylori OMVs were able to cross the placental barrier, leading to a decrease in the number of CD3+ and CD4+ T cells in the peripheral blood of the offspring mice and a decrease in the respon

Identification of lncRNAs in peripheral blood mononuclear cells associated with sepsis immunosuppression based on weighted gene co-expression network analysis.

BACKGROUND: Sepsis-induced immunosuppression involves complex molecular mechanisms, including dysregulated long noncoding RNAs (lncRNAs), which remain poorly understood. OBJECTIVE: We aimed to identify immunosuppression-related lncRNAs and their functional pathways in sepsis. Methods: Using weighted gene coexpression network analysis (WGCNA), we analyzed lncRNA profiles from peripheral blood mononuclear cells (PBMCs) of three sepsis patients and three healthy controls. Key modules linked to immunosuppression were validated via RT-PCR and external datasets. Pathway enrichment and protein inter

Casein sIgE as the most accurate predictor for heated milk tolerance in Finnish children.

BACKGROUND: Up to 83% of children with cow's milk allergy (CMA) tolerate baked milk, but research on heated milk (HM) tolerance and allergen-specific IgE (sIgE) cutoff values predicting oral food challenge (OFC) outcomes are lacking. We hypothesized that casein sIgE is the best predictor for HM tolerance. METHODS: Finnish children suspected of having IgE-mediated CMA were recruited. sIgE levels to milk, casein, alpha-lactalbumin, beta-lactoglobulin, and bovine serum albumin were measured alongside skin prick tests (SPT) to fresh milk (FM) and HM. The diagnosis was confirmed with an FM OFC. Ig

Autologous Tolerogenic Dendritic Cells for Rheumatoid Arthritis-2 (AuToDeCRA-2) study: protocol for a single-centre, experimental medicine study investigating the route of delivery and potential efficacy of autologous tolerogenic dendritic cell (TolDC) therapy for rheumatoid arthritis.

BACKGROUND: Dendritic cells are professional antigen presenting cells with the ability, in their immature state, to induce tolerance in T cells. A protocol to develop phenotypically stable tolerogenic dendritic cells (TolDC) was developed in Newcastle and cells administered to participants in the phase I AuToDeCRA study, demonstrating that TolDC were safe and well tolerated. More knowledge of the TolDC product is now needed, such as optimal dose, route of administration and antigen loading. Establishing this and developing a biomarker profile to demonstrate favourable immunomodulation is the f

Generation of tolerogenic antigen-presenting cells in vivo via the delivery of mRNA encoding PDL1 within lipid nanoparticles.

Tolerogenic antigen-presenting cells (APCs) are promising as therapeutics for suppressing T cell activation in autoimmune diseases. However, the isolation and ex vivo manipulation of autologous APCs is costly, and the process is customized for each patient. Here we show that tolerogenic APCs can be generated in vivo by delivering, via lipid nanoparticles, messenger RNA coding for the inhibitory protein programmed death ligand 1. We optimized a lipid-nanoparticle formulation to minimize its immunogenicity by reducing the molar ratio of nitrogen atoms on the ionizable lipid and the phosphate gro

Eliminating myeloid-derived suppressor cells alleviates immunosuppression and reduces susceptibility to secondary infections in a two-hit sepsis model.

Myeloid-derived suppressor cells (MDSCs) are known for their immunosuppressive effects on both innate and adaptive immunity, particularly targeting T cells, and they undergo continuous expansion during sepsis. However, the pathophysiological significance of MDSCs in sepsis-induced immunosuppression remains to be fully elucidated. In this study, we investigated the dynamic changes in MDSCs during sepsis and their contribution to sepsis-induced immunosuppression using a clinically relevant "two-hit" sepsis model. Our findings revealed that mice surviving cecal ligation and puncture (CLP) exhibit

Human tolerogenic dendritic cell subtypes exert divergent effects on induction of cytotoxic CD4+ T cells.

Tolerogenic dendritic cells (tolDC) are currently in clinical trials for the treatment of autoimmune diseases such as rheumatoid arthritis and multiple sclerosis. The methods for producing therapeutic tolDC vary widely, with little being known about the commonalities and differences of distinct cell products in terms of their regulatory actions on CD4+ T cells. We compared human monocyte-derived tolDC generated with vitamin D3 alone or in combination with dexamethasone. We found marked differences in the surface expression of HLA-DR and immune regulatory molecules, but also found commonalities

Single-cell RNA-sequencing of human spleens reveals an IDO-1+ tolerogenic dendritic cell subset in pancreatic cancer patients that is absent in normal individuals.

Local and systemic immunosuppression are prominent features of pancreatic cancer, rendering anti-tumor effector cells inactive and immunotherapeutic approaches ineffective. The spleen, an understudied point of antigen-presentation and T cell priming in humans, holds particular importance in pancreatic cancer due to its proximity to the developing tumor. As main effectors of antigen presentation, dendritic cells display antigens to lymphocytes, thereby bridging the innate and adaptive immune response. While tumor-infiltrating anti-inflammatory dendritic cells have been described, splenic dendri
