Trovafloxacin is active against Toxoplasma gondii.

Drugs currently used for treatment of toxoplasmosis in pregnant women, congenital infections, immunocompromised patients, and patients with the ocular disease are not always effective or may be dangerous to use; therefore, there is a need for more-effective and less-toxic drugs. Recently, we examined a group of fluoroquinolones for in vitro and in vivo activities against Toxoplasma gondii. Among those examined in vitro (ciprofloxacin, fleroxacin, ofloxacin, temafloxacin, and trovafloxacin), only trovafloxacin significantly inhibited intracellular replication of T. gondii without significant to

Cytomegaloviral acalculous cholecystitis in acquired immunodeficiency syndrome patients.

Abdominal pain and fever in patients with the acquired immunodeficiency syndrome (AIDS) may indicate cytomegaloviral (CMV) acalculous cholecystitis. We reviewed clinical, laboratory, and outcome data from 12 patients with CMV cholecystitis. Ten of 12 patients were homosexual males. Six patients had markedly low CD4: CD8 lymphocyte count ratios. Total leukocyte counts were normal or decreased, serum liver function tests normal or cholestatic, and only one patient had hyperbilirubinemia. Sonographic transmural gallbladder edema is typically more severe than expected for the presenting illness. F

RSV vaccination in immunocompromised adults.

BACKGROUND: Respiratory syncytial virus (RSV) is a major cause of lower respiratory tract disease in immunocompromised adults, including solid organ and hematopoietic stem cell transplant recipients. Following the 2023 licensure of three RSV vaccines for older adults, increasing numbers of immunocompromised individuals have become eligible for vaccination, although these populations were largely excluded from pivotal phase 3 trials. METHODS: We synthesize emerging data from prospective immunogenicity studies and large real-world effectiveness analyses evaluating RSV vaccines in immunocompromi

Surgical Outcomes and Exploratory Preoperative Risk Stratification for Invasive Pulmonary Fungal Infections in Paediatric Patients: A Single-Center Retrospective Study.

BACKGROUND: Invasive pulmonary fungal infections (IPFIs) in immunocompromised children carry high morbidity and may interrupt life-saving chemotherapy or haematopoietic stem cell transplantation (HSCT). Evidence on perioperative risk stratification for surgical management is limited. PATIENTS AND METHODS: We conducted a single-center retrospective cohort study of patients < 16 years with proven/probable IPFI who underwent lung resection from 2017 to 2025, with fungal infection confirmed by pathology. Preoperative clinical variables, laboratory tests and CT features were collected. Outcomes in

Factors associated with severe COVID-19 in immunocompromised subgroups in England from 2020 to 2024: an OpenSAFELY cohort study.

BACKGROUND: Individuals with compromised immune systems are particularly vulnerable to COVID-19. Although several studies have assessed factors associated with COVID-19-related hospitalisation and mortality in the general population, few studies have focused specifically on immunocompromised populations. METHODS: With the approval of NHS England, we conducted a cohort study using the OpenSAFELY platform covering successive waves of COVID-19 (wave 1, alpha, delta, omicron BA.1/BA.2, JN.1). In each wave, we included adults in five hierarchically assigned immunocompromised subgroups: solid organ

Cutaneous Invasive Opportunistic Fungal Infections in Dogs Receiving Ciclosporin for Immune-Mediated Diseases: A Case-Control Study.

BACKGROUND: Cutaneous opportunistic invasive fungal infections (OIFIs) are mainly reported in immunosuppressed dogs, especially those receiving ciclosporin (CsA). Studies investigating factors other than CsA that favour cutaneous OIFIs in dogs are lacking. HYPOTHESIS/OBJECTIVES: To identify host factors, clinicopathological data, and therapeutic regimens associated with the development of cutaneous OIFIs in dogs receiving CsA for immune-mediated diseases. ANIMALS: Client-owned dogs treated with CsA, with or without systemic glucocorticoids (GCs) for various immune-mediated diseases. Dogs wer

Aspergillus fumigatus brain abscess in an immunocompromised patient receiving a second-generation Bruton Tyrosine Kinase inhibitor (BTKi).

Chronic lymphocytic leukemia (CLL) is a frequent type of adult leukemia. Bruton Tyrosine Kinase inhibitors (BTKi) are first-line treatments for CLL. Despite being efficient, BTKi have also been associated with significant side effects, including arrhythmias, hemorrhagic complications, and opportunistic infections. Zanubrutinib is a second-generation BTKi that recently received FDA approval for CLL and is associated with improved outcomes and a better safety profile compared to first-generation BTKi. In the present study, we report a case of a patient with CLL, with an in-range absolute neutrop

Epidemiology, ventilation, and outcomes of acute respiratory failure in immunocompromised patients from 103 intensive care units in 26 countries: a retrospective observational study.

BACKGROUND: Acute hypoxaemic respiratory failure (ARF) is the leading cause of intensive care unit (ICU) admission among immunocompromised patients. However, contemporary data regarding the epidemiology, management, and outcomes of ARF in this population remain scarce. We aimed to identify predictors of mortality and intubation in immunocompromised patients admitted to the ICU with ARF. METHODS: This retrospective observational study was conducted in 103 ICUs in 26 countries. Adults (≥18 years) with ARF and immunodeficiency were eligible for inclusion. Patient data, including information on t

Pitfalls in the differential diagnosis of diarrhea after kidney transplantation: challenges in identifying Yersinia enterocolitica infection.

We present the case of a 14-year-old boy with a history of kidney transplantation due to focal segmental glomerulosclerosis who developed severe diarrhea and abdominal pain following an episode of antibody-mediated rejection. Despite stable kidney function, the patient required increased immunosuppressive therapy, raising concerns regarding possible drug-induced enteritis or infections. Initial investigations, including stool tests for common pathogens and imaging, failed to identify the causative agent. Colonoscopy revealed thickening of the terminal ileum and aphthae in the colon; however, c

Pathogenic signatures and therapeutic evaluation of emergent MPXV Clade Ib in low-susceptibility and immunocompromised mouse models.

BACKGROUND: Monkeypox (Mpox) is a zoonotic disease that threatens global public health. Different clades of monkeypox virus (MPXV) vary in transmissibility and pathogenicity. In 2023, a Clade Ib MPXV variant emerged in the Democratic Republic of the Congo, continued to spread in parts of Africa, and subsequently spilled over to other regions, posing new challenges for outbreak prevention and control. RESULTS: We established stable mouse models of MPXV Clade Ib infection by intranasally infecting C57BL/6/STAT1-/-, AGB6 (C57BL/6-Ifngr1-/-Ifnar1-/-), C57BL/6, and BALB/c mice. Susceptible strains

Disseminated human parvovirus B19 encephalitis and pure red cell aplasia unmasking immune dysregulation in angioimmunoblastic T-cell lymphoma.

BACKGROUND: Acute encephalitis caused by human parvovirus B19 (PVB19) is an uncommon clinical manifestation observed in both immunocompetent and immunocompromised individuals. Angioimmunoblastic T-cell lymphoma (AITL) is a distinct subtype of lymphoma characterized by aberrant immune dysregulation, predisposing patients to opportunistic infections even in the absence of chemotherapy. CASE PRESENTATION: A 66-year-old woman with newly diagnosed, treatment-naïve AITL presented with acute confusion, dysarthria, and severe anemia. Cerebrospinal fluid (CSF) analysis and neuroimaging findings were n

Clinical and Molecular Perspectives on Epidermodysplasia Verruciformis.

Epidermodysplasia verruciformis (EV) is a rare dermatologic disorder marked by an increased susceptibility to β-human papillomavirus infections and a heightened risk of cutaneous squamous cell carcinoma. While classically associated with autosomal recessive pathogenic variants in TMC6, TMC8, and CIB1, recent reports have expanded the disease spectrum to include acquired forms occurring in immunocompromised individuals, including those with human immunodeficiency virus/acquired immunodeficiency syndrome (HIV/AIDS), organ transplants, and autoimmune disease. Both inherited and acquired EV share

Chest X-ray prior to biological treatment in inflammatory bowel diseases: it is not only about tuberculosis - a case report and literature review.

BACKGROUND: Biological treatments have substantially improved management of inflammatory bowel disease, but their profound immunosuppressive effects necessitate infection screening before initiation. Current guidelines recommend chest X-ray (CXR) alongside Mycobacterium tuberculosis testing. While this approach is widely accepted, the necessity of routine CXR in patients without tuberculosis (TB) risk factors has recently been questioned in low-incidence regions. METHODS: We report a rare case of a patient with ulcerative colitis undergoing routine preinfliximab CXR revealing an apical right-

Predicting persistent SARS-CoV-2 shedding in immunocompromised patients: a probability-based approach.

BACKGROUND: Although recommended isolation periods for Coronavirus disease 2019 (COVID-19) have been shortened as the pandemic has subsided, prolonged Severe Acute Respiratory Syndrome-Coronavirus-2 (SARS-CoV-2) shedding remains common in immunocompromised patients. This study estimated the probability of viral clearance in these patients based on elapsed days and specific risk factors. METHODS: We prospectively enrolled immunocompromised patients with a confirmed COVID-19 diagnosis from January 2022 to May 2023 during the Omicron variant era. We collected weekly respiratory specimens for vir

Screening and targeted prophylaxis for Clostridioides difficile infection.

OBJECTIVE: Patients receiving immunosuppression for oncology treatment and solid organ transplantation are at high risk for developing hospital-onset Clostridioides difficile infection (HO-CDI). We studied the impact of a prophylactic enteral vancomycin intervention on the reduction of HO-CDI during high-risk inpatient admissions among immunocompromised patients. METHODS: The screening and targeted prophylaxis (STOP) intervention was implemented over a 2-year period. Patients admitted for solid organ transplant, autologous stem cell transplant, chimeric antigen receptor T-cell, or treatment f

A Pilot Metabolomic Study for Diagnosing Aspergillus Infection in Immunocompromised Pediatric Cancer Patients.

Fungal infection caused by invasive Aspergillus is a life-threatening complication in immunocompromised pediatric cancer patients. However, the early diagnosis of invasive infection remains a clinical challenge due to the lack of specific, non-invasive biomarkers. The current study investigates plasma metabolomic profiling integrated with an AI-derived fungal secondary metabolite database to identify potential biomarkers for rapid, non-invasive detection of Aspergillus infection. Plasma samples from thirteen pediatric oncology patients were analyzed using untargeted metabolomics based on UHPLC
