Persistent and dormant tubercle bacilli and latent tuberculosis.

Tubercle bacillus has remarkable ability to persist in the human host and has caused latent infection in one third of the world population. The current tuberculosis (TB) chemotherapy while effective in killing growing bacilli is largely ineffective in killing persistent or dormant bacilli, leading to prolonged therapy. There is considerable recent interest to study mechanisms of persistence and dormancy in mycobacteria. Meanwhile, there is also confusion about the use of terminology of dormant and persistent bacilli. Different models of mycobacterial persistence have been established. Various

The effect of immunosuppression with cyclosporin A on the development of sheep scab.

The present study confirms that following infection with the ectoparasitic sheep scab mite, Psoroptes ovis, there is a rapid (within 24 h) inflammatory influx of eosinophils and apoptosis of the keratinocytes at the site of infection. In order to investigate whether these inflammatory reactions are important in the maintenance of mite infection, a group of animals were treated daily after the establishment of infection with the potent anti-inflammatory drug, Cyclosporin A. The course of infection was monitored by determining the lesion area and mite numbers, systemic antibody and blood eosinop

Gonococcal infection of the newborn in Florida, 1984-1989.

An increase in neonatal gonococcal infections was recorded in Florida between 1984 and 1988. By reviewing Florida sexually transmitted disease surveillance case records between 1984 and 1989, 68 cases of neonatal gonococcal infections were identified state-wide. Those 68 cases included 55 (81%) cases of gonococcal ophthalmia neonatorum, 4 genital infections, 1 nasal infection, 1 ear infection, 1 skin infection, and 1 scalp infection. At birth, positive culture results were demonstrated in 3 gastric and 2 respiratory aspirate cultures. A case-control study using birth certificates as the source

Effect of topical treatments on irritant hand dermatitis in health care workers.

BACKGROUND: Irritant contact dermatitis (ICD) from repetitive hand hygiene is the primary reason for compliance failure among health care workers (HCWs). Chronic ICD has implications for infection control because higher bacterial counts are associated with increased skin compromise. Guidelines recommend lotions/creams to lessen irritation. We evaluated the effects of 5 to 10 daily applications of a test cream (A, glove and chlorhexidine gluconate compatible) and current lotions/creams (B) compared with a control of normal skin care. METHODS: Outcomes were visual skin erythema and dryness, exc…

How macrophage heterogeneity affects tuberculosis disease and therapy.

Macrophages are the primary host cell type for infection by Mycobacterium tuberculosis in vivo. Macrophages are also key immune effector cells that mediate the control of bacterial growth. However, the specific macrophage phenotypes that are required for optimal immune control of M. tuberculosis infection in vivo remain poorly defined. There are two distinct macrophage lineages in the lung, comprising embryonically derived, tissue-resident alveolar macrophages and recruited, blood monocyte-derived interstitial macrophages. Recent studies have shown that these lineages respond divergently to si

The Role of Endovascular Procedure for Peripheral Arterial Disease in Diabetic Patients With Chronic Limb-Threatening Ischemia.

Type 2 diabetes mellitus is a major risk factor for all forms of cardiovascular diseases, including peripheral arterial disease (PAD). Chronic limb-threatening ischemia (CLTI) is determined by the presence of ischemic rest pain, and may or may not be accompanied by tissue loss (such as ulcers and gangrene) or infection. Treatments for CLTI consist of wound treatment, infection control, and ischemia control by arterial revascularization, which can be performed by either open surgical procedure (bypass) or an endovascular approach. We present two cases of chronic limb-threatening ischemia, one w…

β-Lactam Resistance Mechanisms: Gram-Positive Bacteria and Mycobacterium tuberculosis.

The value of the β-lactam antibiotics for the control of bacterial infection has eroded with time. Three Gram-positive human pathogens that were once routinely susceptible to β-lactam chemotherapy-Streptococcus pneumoniae, Enterococcus faecium, and Staphylococcus aureus-now are not. Although a fourth bacterium, the acid-fast (but not Gram-positive-staining) Mycobacterium tuberculosis, has intrinsic resistance to earlier β-lactams, the emergence of strains of this bacterium resistant to virtually all other antibiotics has compelled the evaluation of newer β-lactam combinations as possible contr

Occupational transmission of Neisseria meningitidis --- California, 2009.

Neisseria meningitidis is a leading cause of bacterial meningitis and sepsis . The case-fatality rate for meningococcal disease is 10%--14%; … d recommends measures to control and prevent secondary transmission of N. meningitidis. Breaches in infection control, notification delays, and lack of worker exposure assessment and postexposure chemoprophylaxis (PEP) likely contributed to secondary cases. Employers should provide adequate infection-control training to staff members, PEP to exposed workers, and re …

Hidden Mould in the Air: A Practical Narrative Mini-Review of Invasive Mould Infection Prevention for High-Risk Patients and Clinicians.

Airborne mould spores, including those of the Aspergillus species, are ubiquitous both indoors and outdoors. While they are generally harmless, inhaled spores can cause severe infections, particularly invasive pulmonary aspergillosis, in patients with impaired immune defences. Allergic manifestations such as asthma arise by different mechanisms and are not covered. This narrative mini-review summarises key evidence and guideline recommendations for healthcare teams and provides practical, patient-friendly advice by highlighting clinical and infection-prevention considerations. Core Interventions for the Prevention of Peritoneal Dialysis-Related Infections.

For some patients, peritoneal dialysis (PD) has several advantages over center-based hemodialysis. PD-related infections such as peritonitis and PD catheter exit site and tunnel infections are a significant source of morbidity. Peritonitis leads to increased mortality, and it is the leading cause of transfer off PD. Infection prevention practices may vary widely across dialysis centers, resulting in significant differences in infection rates. To address these issues and improve patient outcomes, the American Society of Nephrology facilitated the development of core interventions aimed at reduc

Vascular Access Team Central Line Dressing Changes to Reduce Infection Risk: A Focused Two-Person Approach in High-Risk Patients.

Due to low compliance by bedside nursing with a central line-associated bloodstream infection (CLABSI) prevention bundle and increased CLABSI rates, a mandatory re-education initiative at a 1200-bed university-affiliated hospital was undertaken. Despite this, 2 units, housing high-risk immunocompromised patients, continued to experience increased CLABSI rates. A quality improvement before-after project design in these units replaced bedside nursing staff with 2 nurses from the vascular access team (VAT) to perform central vascular access device (CVAD) dressing changes routinely every 7 days or

Antibiotic Stewardship in the Neonatal Intensive Care Unit.

Antibiotic stewardship is a multidisciplinary, evidence-based approach to optimize antibiotic use and mitigate development of antibiotic resistance. Neonates have high rates of antibiotic exposure, particularly those born preterm and admitted to the NICU, and mounting evidence describes the adverse consequences of such exposures in the absence of infection. Here, we review the general principles of antibiotic stewardship and how they can be applied in NICUs. The unique characteristics of NICUs and patients cared for in this setting, which warrant unique implementation strategies and special co

Does longer peripheral intravenous catheter length optimise antimicrobial delivery? Protocol for the LEADER study.

BACKGROUND: Hospitalised patients receiving intravenous antimicrobial therapy require a reliable device through which this is delivered. Short peripheral intravenous catheters (PIVCs) are the default device for antimicrobial therapy but up to half fail before therapy completion, leading to suboptimal drug dosing, patient distress from repeated insertions, and increased healthcare costs. This study will investigate the use of long PIVCs to determine if they are more reliable at delivering antimicrobial therapy. METHODS: A two-arm, parallel randomised controlled trial of hospitalised adults req
