Thymic carcinoid responds to neoadjuvant therapy with sunitinib and octreotide: a case report.

Carcinoids are malignant neuroendocrine tumors consisting of a spectrum of neoplasms from low-grade typical carcinoid to high-grade small cell carcinoma. We report a case of atypical thymic carcinoid that responded to neoadjuvant therapy with octreotide and sunitinib, an oral multikinase inhibitor. After 3 weeks of treatment, tumor size significantly decreased to allow for a safe surgical resection with clear margins. We believe that further study of sunitinib and octreotide with the neoadjuvant intent of preparing tumors for resection is warranted as a strategy to improve curative management

Dihydroartemisinin-piperaquine rescue treatment of multidrug-resistant Plasmodium falciparum malaria in pregnancy: a preliminary report.

Dihydroartemisinin-piperaquine (DHA-PPQ) is a promising new artemisinin combination treatment. There are no published data on the intentional use of the drug in pregnancy. Between June 2006 and January 2007, 50 Karen pregnant women with recurrent P. falciparum infections, despite 7-day treatments with quinine or artesunate (+/-clindamycin) or both, were treated with DHA-PPQ. This rescue treatment was effective and well tolerated and there was no evidence of toxicity for the mothers or the fetus. The PCR adjusted cure rate by Kaplan Meier analysis at day 63 was 92.2% (95% CI: 76.9-97.4).

Endovascular thoracic aortic aneurysm repair with concomitant myocardial and carotid revascularization.

A 73-year-old woman presented with a large saccular aneurysm involving the distal aortic arch. Preoperative aortography and cardiac catheterization revealed left main coronary artery and left common carotid artery stenoses. Concomitant coronary artery bypass grafting to the left anterior descending and first diagonal arteries, ascending aorta-to-left common carotid artery bypass grafting, and endovascular thoracic aortic aneurysm repair with antegrade stent-graft deployment and intentional left subclavian artery coverage were performed.

Zinc phosphide intoxication of wild turkeys (Meleagris gallopavo).

Zinc phosphide (Zn3P2) is a rodenticide used to control a variety of small mammal species. It is available over-the-counter or as a restricted-use pesticide depending on how it is to be applied. The toxicity of Zn3P2 is dependent on the species exposed, whether the animal is able to vomit or not, and whether it is ingested on a full or empty stomach. Nontarget species can be exposed through inadvertent or intentional product misapplication. In this article we describe four mortality events in which wild turkeys (Meleagris gallopavo) were believed to have been intoxicated following the ingestio…

[Review on the study of the theory of the mind in pervasive development disorders and schizophrenia].

The theory of mind (ToM) is a new concept which could be defined as the ability to make inferences on the intentions of others. This capacity has been used to explain childhood autism symptoms and recently an attempt has been made to refer it to schizophrenia. The authors aim to review the theory of mind concept, the different questions that could appear in neurocognitive sciences and the utilities that this hypothetical model has been given in the clinical practice. The authors also consider the contributions and restrictions that its use has in the field of pervasive developmental disorders …

Inherited arrhythmias in the dog: potential experimental models of cardiac disease.

Centuries of inbreeding the domestic dog has resulted in numerous spontaneous breed-specific and familial diseases of all body systems [1]. Although few animal models of spontaneous arrhythmic death exists [2] veterinary cardiologists have recognized potentially inherited arrhythmias in the dog for years. The purpose of this paper is to describe the proven inherited, and the likely inherited, arrhythmias diagnosed commonly in the dog with the intent to provide information of potential models for investigation of primary arrhythmias. Additionally, examples of common arrhythmias that are seconda

Memory bias for panic-related material in patients with panic disorder.

Two experiments investigated memory bias for panic-related material in 40 patients with panic disorder and 40 healthy control subjects. No memory bias was found on a memory task that tested intentional encoding and explicit recall of panic-related versus non-panic-related sentences. In contrast, a significant memory bias was apparent on a memory task requiring classification of panic-related and non-panic-related words to test conceptual information processing in implicit memory. Panic patients learned panic-related material better than controls.

Intimate justice. II: Fostering mutuality, reciprocity, and accommodation in therapy for psychological abuse.

This article presents part of the findings of a study of psychological abuse and physical violence in couples who voluntarily entered therapy. The study found that most of the men exhibited patterns of deception, devaluation, and dictatorial attitudes with their women partners and that these patterns were a considerable barrier to mutuality, reciprocity, and accommodation in the partnership. The researchers developed four interventions to challenge the men to change these patterns: true intentions, no free rides, the perception paradox, and the infallibility fallacy. The study was based on int…

Acute liver failure induced by alcohol and paracetamol in an HCV-infected haemophiliac.

Paracetamol (acetaminophen) is frequently used by haemophiliacs in the management of acute or chronic pain, primarily due to its lack of adverse effects when taken at therapeutic doses. We describe acute hepatic and renal toxicity of paracetamol in a 36-year-old patient with haemophilia B, chronic hepatitis C and chronic alcohol abuse. Moderate doses of paracetamol (6 g/d for 4 d), taken with therapeutic intent, resulted in life-threatening organ dysfunction which gradually recovered with full supportive care.

[Primary prevention of cerebral toxoplasmosis in patients with HIV infection: results of a double-blind randomized trial, pyrimethamine versus placebo].

To assess the efficacy of pyrimethamine (Pyr) for primary prophylaxis of cerebral toxoplasmosis (CT) in HIV patients (pts), 554 pts were randomized in a double-blind, placebo-controlled trial (mean follow-up: 14 months). The intent-to-treat analysis found no significant difference in the incidence of ct at 1 year: 12% (Pyr) versus 13% (Pcb) nor in survival. Side-effects leading to stop the study drug were more frequent in Pyr group, especially cutaneous rashes: 8% (Pyr) versus 2% (Pcb).

Congenital clubfoot. Etiology, pathoanatomy, pathogenesis, and the changing spectrum of early management.

Congenital clubfoot is a common problem of uncertain etiology with a broad spectrum of clinical severity. Much remains controversial and unsolved about this disorder. It is agreed, however, that early management should be conservative and that operative management should comprehensively address the deformities, to avoid if possible, subsequent operations. No single monograph can comprehensively review all aspects of this complex disorder. Our intent has been to touch upon the basic concepts and philosophies, to give perspective to the overall picture, for some, and to provide a stimulus and ba

Adenocarcinoma of the sigmoid colon: sites of initial dissemination and clinical patterns of recurrence following surgery alone.

Three hundred and fifty-five patients with primary adenocarcinoma of the sigmoid colon were retrospectively reviewed. One hundred and eighteen patients who had disseminated disease at diagnosis were analyzed to identify sites and mechanisms of disease spread. Two hundred and thirty-seven patients treated by surgery alone with curative intent were analyzed to determine clinical patterns of recurrence and identify prognostic factors. Operative findings in 27 patients undergoing second laparotomy at the time of initial disease recurrence are described, and implications for adjuvant therapy discus

Therapeutic Relationship: A Factor for Substantial Change in Psychodynamic Psychotherapies and Psychoanalysis.

Significant change in psychodynamic psychotherapy resides in the uncovering of unconscious conflicts as they manifest themselves in the lives of patients. My intent in this article is to illustrate the special power of the therapeutic relationship as a separate element beyond interpretation as a vehicle for a change. This is illustrated by three patients who revealed special feelings of affection for me in the context of our work together and the progress that they had made. The substrate of these feelings is a product of a consistent acceptance and valuation of the patient in all that they re

[Comment on the article "Severe coronavirus-2 infection background for cardiovascular complications"].

Current information is presented on a phenomenon that occurs in patients who had COVID-19 that could contribute to long-term cardiovascular risk. It deals with the mechanisms underlying cardiovascular and coagulation alterations that perpetuate cardiac symptoms caused by alterations induced by the virus or by myocardial inflammation, such as cardiac endothelial dysfunction, which could cause vascular microthrombosis. The intentional search for this in patients under follow-up becomes relevant since there is specific treatment and long-term complications, such as pulmonary hypertension and righ…

Case Report: Synchronous Cutaneous and Brain Metastasis in a Treated Case of Squamous Carcinoma of Left Lower Alveolus: A rare Case report.

Skin and brain metastasis in head and neck carcinoma (SCC) is extremely rare and contribute to less than 5% of cases. Cutaneous metastases of head and neck cancer usually manifests as papulonodular lesions and brain metastases presents with features of raised intracranial tension. We present a case report of 49- year male diagnosed with synchronous cutaneous and brain metastasis. Intent is usually palliative & prognosis remains poor in these cases marked by poorer outcomes. Keywords : Head and neck cancer, Brain metastases, Cutaneous metastases.

Sleep Phylogeny: Have We Jumped the Shark?

Sleep is considered to be ubiquitous in the animal kingdom. Scientists in part believe this because sleep is increasingly defined solely by behavioral measures. This has resulted in an astonishing expansion of species reported to sleep. But have we gone too far? Have scientists jumped the shark? This idiom refers to when an idea has exhausted its core intent and creates ideas that are discordant with its original purpose. In this commentary, I discuss what I see as an emerging problem in how we define sleep and the attendant implications.

Poisoning by butylated hydroxytoluene quinone methide acting as a superwarfarin: first reported case in humans.

Superwarfarins are anticoagulant rodenticides nearly 100-fold potent than the parent compound. Since their development, accidental and intentional cases of superwarfarin poisoning have been reported. We report the first human case of poisoning by butylated hydroxytoluene (BHT) quinone methide acting as a superwarfarin otherwise reported to be a well tolerated food additive and preservative and used as an antioxidant, stabilizer, anti-skinning agent in various industries. We aim to highlight the possible underlying cause of this previously unreported and potentially lethal BHT-related complicat…

Overview of Cutaneous T-Cell Lymphomas.

Non-Hodgkin's lymphoma (NHL) encompasses a diverse collection of systemic and primary cutaneous lymphomas. Cutaneous T-cell lymphomas (CTCLs) represent about 13% of all NHLs, which are further subdivided into a heterogeneous group with vastly different presentations and histologic features. Diagnosis requires integration of clinical, pathologic, and molecular features. Among CTCLs, mycosis fungoides and Sézary syndrome are the most prevalent. Treatment is aimed at limiting morbidity and halting disease progression. Hematopoietic stem cell transplantation is the only therapy with curative inten

Electroconvulsive Therapy in Geriatric Psychiatry: A Selective Review.

Electroconvulsive therapy (ECT) remains an important treatment of geriatric patients. ECT treats severe depression, mania, psychosis, catatonia, and comorbid depression and agitation in dementia. ECT also serves a crucial role in treating urgent illness requiring expedient recovery, such as catatonia, or in patients with severe suicidal ideation or intent. ECT is even more effective in the elderly than in mixed-age adult populations. ECT is a safe treatment option with few medical contraindications. Cognitive effects are largely transient, even in patients with preexisting cognitive impairment
