Comparison of drinking and smoking patterns during pregnancy over a six-year interval.

Changes in the drinking and smoking habits of pregnant women in Seattle, Washington, were examined in the 6-year period that followed the first American announcement of the fetal alcohol syndrome. Between 1974/1975 and 1980/1981, the number of women who reported any alcohol use around the time of the first prenatal visit dropped from 81% to 42%, whereas the number of smokers dropped from 25% to 22%; binge and heavier drinking also declined. However, among drinkers, there was no decrease in the proportion of women who reported heavier drinking. Although the proportion of women who drink during

Divorce in clinical practice.

The family physician today has many families in his/her practice who have decided on divorce and who turn to the family physician for guidance. The effect of divorce for children from infancy to age three years is primarily related to the mother's emotional adjustment to the divorce. Preschool and school aged children are most at risk for personality disturbances because of their emerging sense of identity and need for both parents as figures of identification. The adolescent is initially the most painfully distressed by the divorce but, in fact, is in time the least affected of all the age gr

From silenced to connected: measuring language-based identity expansion through digital media.

BACKGROUND: Social media have become a central arena for identity construction, particularly for minority communities whose languages and cultures have been historically marginalized. Drawing on self-expansion theory, this study examines how social media facilitate language-based identity expansion within the Kurdish community in Türkiye. Despite long-standing suppression of the Kurdish language and culture, digital platforms provide new spaces for linguistic reconnection, cultural expression, and the reinforcement of collective identity. While self-expansion theory has traditionally focused o

Effectiveness of telehealth in preventive care: a study protocol for a randomised controlled trial of tele-exercise programme involving older people with possible sarcopenia or at risk of fall.

BACKGROUND: Continuous loss of muscle mass and strength are the consequences of the ageing process, which increase the risk of falls among older people. Falls can lead to severe consequences such as bone fractures and hampered physical and psychological well-being. Regular exercise is the key to reversing muscle atrophy and relieving sarcopenia. However, the frailty of older people and the recent COVID-19 pandemic may affect their confidence to leave home to attend classes in the community. A feasible and effective alternative should be explored. METHODS: The primary objective is to evaluate

Improving the effectiveness of psychological interventions for depression and anxiety in the cardiac rehabilitation pathway using group-based metacognitive therapy (PATHWAY Group MCT): study protocol for a randomised controlled trial.

BACKGROUND: Anxiety and depression are prevalent among cardiac rehabilitation patients but pharmacological and psychological treatments have limited effectiveness in this group. Furthermore, psychological interventions have not been systematically integrated into cardiac rehabilitation services despite being a strategic priority for the UK National Health Service. A promising new treatment, metacognitive therapy, may be well-suited to the needs of cardiac rehabilitation patients and has the potential to improve outcomes. It is based on the metacognitive model, which proposes that a thinking st

BACKGROUND: Parkinson's is a neurodegenerative condition that has a critical impact on every aspect of the affected individual's life and also on their caregiver. A neurosurgery procedure called deep brain stimulation (DBS) has improved motor symptoms in some patients. DBS is one of the options offered to eligible candidates affected by Parkinson's; however, this pathway may cause both physical and psychological problems for patients and their caregivers. AIMS: To explore the experiences of caregivers of patients with Parkinson's who have undergone DBS, to further understand their needs, and

Virtual mindfulness-based wellness coaching to support mental health during the COVID-19 pandemic: Thematic analysis of participants' experiences.

The COVID-19 pandemic and associated disruptions adversely impacted physical and mental health. The shift to remote delivery of medical and psychological services provided opportunities to disseminate effective community interventions, unimpeded by needs for local in-person meetings. This study focuses on the responses of older adults to the virtual community Mindfulness-based Wellness Coaching (MBWC) intervention that was delivered by the YMCA of Greater Toronto. We focus on the thematic analysis of semi-structured interviews of 23 participants who were regular attendees. Themes identified pe

From initial acceptance to reflective adjustment: psychological adjustment patterns among Taiwanese adolescents encountering misinformation.

This qualitative study explores adolescents' psychological adjustment when encountering online misinformation. Based on in-depth interviews with Taiwanese secondary school students, it examines how participants moved from initial acceptance of inaccurate information toward questioning, verification, recognition, and later adjustment. The analysis identified five interrelated patterns: initial acceptance and confusion, emerging doubt, verification, recognition and reflective reassessment, and subsequent adjustment. Although these patterns often appeared in a broadly temporal relationship, they

Pregnancy experiences of expectant parents with Neurofibromatosis type 1: a qualitative interview study.

Pregnancy in the context of Neurofibromatosis 1 (NF1) may be emotionally complex due to uncertainties surrounding inheritance and the condition's variable presentation. This study aimed to explore how expectant parents with NF1 experience pregnancy and relate to their unborn child. Fourteen participants took part in individual semi-structured interviews, and data were analysed using reflexive thematic analysis. Participants described how decisions around conception and genetic testing were influenced by personal and medical history, perceived severity of NF1, and concerns about potential impac

Storied Bodies and Temporal Lives: A Narrative Inquiry Exploring the Experiences of Young Adults Living with an Eating Disorder.

An eating disorder is a type of mental illness, characterized by a disturbance in eating related behaviour, resulting in significant physical and psychological impairments. Despite this recognition, eating disorders remain poorly understood and plagued by high mortality and low recovery rates. Furthermore, individuals living with an eating disorder rarely have opportunities to shape study design or inform treatment. This study used narrative inquiry to explore the experiences of two young adults, aged 18 to 30, living with Anorexia or Bulimia Nervosa, and self-identified as living with an eati

Metamorphosis: A qualitative study exploring menopause through body-based practices.

Menopause is a multidimensional life transition, yet UK healthcare and public discourse often frame it narrowly around hormones and medical treatment. This qualitative study explored how 18 women aged 40-60 used body-based practices, including strength training, yoga and cold-water immersion, to navigate menopause. Interviews were analysed using reflexive thematic analysis within a critical realist framework. Menopause was interpreted as an individual biopsychosocial-cultural experience, with spiritual and experiential dimensions for some. Psychological symptoms were typically more disruptive

Associations Between MMPI-3 Scales and Dimensional Personality Disorder Models in a Community Mental Health Sample.

As the field adopts dimensional models of personality disorders (DSM-5 Alternative Model of Personality Disorders [AMPD]; ICD-11 Personality Disorder), it is imperative that clinical assessment tools are available to assess them. The Minnesota Multiphasic Personality Inventory-3 (MMPI-3) is one such measure, as it is routinely used in clinical practice and assesses a variety of psychological symptoms. The current study aimed to build upon previous literature using a community mental health sample and a multi-method approach. Specifically, a sample of 289 individuals enrolled in various mental

The carceral state beyond bars: Pretrial surveillance as a structural determinant of health.

The expanding reach of the carceral state has led to a rise in surveillance-based reforms marketed as humane alternatives to incarceration. Electronic monitoring (EM), an increasingly common condition of pretrial release, is often portrayed as a non-punitive solution to mass incarceration. Drawing on 40 in-depth interviews with individuals on pretrial EM in Cook County, Illinois-home to one of the largest and most restrictive EM programs in the U.S.-this paper critically examines the health harms that EM imposes on legally innocent people. We identify three mechanisms through which EM operates
