Percutaneous Image-guided Core Needle Biopsy for Upper Tract Urothelial Carcinoma.

OBJECTIVE: To better understand the safety and diagnostic yield of percutaneous core-needle biopsy (PCNB) for upper tract urothelial carcinoma (UTUC). METHODS: Of 444 patients undergoing radical nephroureterectomy (RNU) for UTUC between 2009 and 2017 at our institution, 42 who had PCNB prior to RNU were identified for analysis. Endpoints included safety, diagnostic yield, and concordance with RNU pathology. PCNB specimens were deemed histologically concordant with RNU specimens for cases when cytologic evaluation of biopsy specimen and corresponding pathologic evaluation of RNU specimen both

Long-term results with M-VAC for advanced urothelial cancer: high relapse rate and low survival in patients with a complete response.

OBJECTIVE: To describe the long-term results of treating patients with advanced urothelial cancer using a combination of methotrexate, vinblastine, doxorubicin and cisplatin (M-VAC). PATIENTS AND METHODS: Fifty-one patients (37 men and 14 women, median age 66 years, range 41-82) with inoperable or metastatic carcinoma of the bladder, ureter or renal pelvis were treated with M-VAC and their responses evaluated for up to 7 years. Relative dose intensity (RDI) was calculated by dividing the actual dose intensity by the projected total dose intensity and related to outcome. Overall survival was a

Assessment of NGAL, eGFR, and tubular injury markers for early detection of chemotherapy-induced nephrotoxicity in pediatric oncology patients.

Acute kidney injury is a common complication in children with oncological diseases. Its etiology is multifactorial, with key risk factors including drug-induced nephrotoxicity, tumor lysis syndrome, and infection-including sepsis during periods of aplasia. The classical diagnostic criteria for AKI rely on monitoring serum creatinine levels and detecting oliguria. In the context of drug-induced nephrotoxicity, the predominant pathophysiological mechanism is tubular injury, which is typically non-oliguric and may not cause changes in serum creatinine, even in cases of severe damage.

Deceased donor kidney transplantation in candidates with pre-transplant hematological malignancies: a literature review and recipient allocation proposal in Singapore.

Deceased donor kidneys are a scarce national resource, and principles of utilitarianism and justice govern allocation. Kidney transplant recipients with a prior history of cancer show an increased risk of malignancy- and non-malignancy-related mortality compared to their counterparts without a previous history of malignancy. The inferior survival of a recipient with pre-transplant malignancy questions the allocation of a scarce resource to a population at anticipated poorer patient and graft survival. However, patient survival has significantly improved with advances in therapeutics for hemato

Renal Cell Carcinoma With Hemangioblastoma-Like Features and Leiomyomatous Stroma: A Rare Case Report and A Potential Pathological Diagnostic Pitfall.

Renal cell carcinoma (RCC) with hemangioblastoma-like features, which was not included in the 2022 edition of the World Health Organization Classification of Tumors of the Urinary System and Male Genital Organs, is an extremely rare RCC reported in recent years. Due to the morphological and immunophenotypic overlap of RCC with hemangioblastoma-like features and renal hemangioblastoma, this might be a potential pitfall or conundrum in histopathological diagnosis. To reinforce the knowledge of this rare tumor, we present an example of RCC with hemangioblastoma-like features with leiomyomatous st

Dual diagnosis of autosomal dominant polycystic kidney disease and sickle cell disease in a teenage male.

BACKGROUND: Sickle cell disease (SCD) and autosomal dominant polycystic kidney disease (ADPKD) are relatively common genetic conditions with considerable overlap in clinical presentation. In addition to similarities between the signs and symptoms in sickle cell nephropathy and ADPKD, more than half of SCD patients have kidney cysts. The co-occurrence of these two diseases has not been previously reported in the literature. CASE DIAGNOSIS/TREATMENT: A 16-year-old Black male with SCD had bilateral kidney enlargement and multiple simple cysts on ultrasound. Although kidney cysts are significantl

Antigen Retrieval and Its Effect on the MALDI-MSI of Lipids in Formalin-Fixed Paraffin-Embedded Tissue.

Formalin-fixed paraffin-embedded (FFPE) tissue represents the primary source of clinical tissue and is routinely used in MALDI-MSI studies. However, it is not particularly suitable for lipidomics imaging given that many species are depleted during tissue processing. Irrespective, a number of solvent-resistant lipids remain, but their extraction may be hindered by the cross-link between proteins. Therefore, an antigen retrieval step could enable the extraction of a greater number of lipids and may provide information that is complementary to that which can be obtained from other biomolecules, s

Plasmacytoid urothelial carcinoma of renal pelvis with positive zinc finger E-box-binding homeobox 1: a case report.

BACKGROUND: The tumor transformation mechanism of a plasmacytoid urothelial carcinoma remains unexplained. We describe the case of a plasmacytoid urothelial carcinoma of the renal pelvis in which the expression of zinc finger E-box-binding homeobox 1 (ZEB1), a key nuclear transcription factor in an epithelial-mesenchymal transition, is involved in tumor transformation. CASE PRESENTATION: The patient had a left nephrectomy with the clinical diagnosis of left pelvic renal cancer. The resected specimen showed that the tumor surface comprised a noninvasive papillary urothelial carcinoma with the

Renal Splenosis: Renal Mass Biopsy Diagnosis of a Tumor Clinically Mimicking Renal Cell Carcinoma.

Renal mass biopsy is increasingly used to guide conservative tumor management, placing increasing importance on pathologists' ability to diagnose small tumor samples. A 66-year-old man with a history of prior splenectomy for trauma presented for urologic evaluation after identification of a left 5.8 cm renal mass. Partial envelopment of the mass by renal parenchyma and equivocal enhancement on computed tomography raised concern for renal cell carcinoma. Needle-core biopsy revealed blood, subtle vasculature, few aggregates of lymphocytes, and rare renal tubules. Immunohistochemical staining rev

Efficacy and Feasibility of Low-Dose Rate Brachytherapy for Prostate Cancer in Renal Transplant Recipients.

BACKGROUND: In young patients with localized prostate cancer, radical prostatectomy is the treatment of choice in the general population. Radiotherapy, such as low-dose rate (LDR) brachytherapy or intensity-modulated radiotherapy, is a viable alternative as well. However, in transplant patients, irradiation is not proposed as often as it is in healthy adults because of the risk of post-radiation ureteral stenosis and gastrointestinal toxicity as the result of fragile tissue. The objective of the study was to assess the efficacy and feasibility of LDR brachytherapy for prostate cancer in renal

Morphologic, Molecular, and Taxonomic Evolution of Renal Cell Carcinoma: A Conceptual Perspective With Emphasis on Updates to the 2016 World Health Organization Classification.

Molecular and morphologic interrogation has driven a much-needed reexamination of renal cell carcinoma (RCC). Indeed, the recently released 2016 World Health Organization classification now recognizes 12 distinct RCC subtypes, as well as several other emerging/provisional RCC entities. From a clinical perspective, accurate RCC classification may have important implications for patients and their families, including prognostic risk stratification, targeted therapeutics selection, and identification for genetic testing. In this review, we provide a conceptual framework for approaching RCC diagno

Wilms Tumor in a Child With Bilateral Polycystic Kidneys and PHACE Syndrome: Successful Treatment Outcome Using Partial Nephrectomy and Chemotherapy.

Congenital anomalies may be associated with Wilms tumor either as isolated anomalies or as part of a congenital malformation syndrome. Nephroblastoma occurring in association with polycystic kidneys is very rare. The optimal surgical management of nephroblastoma in the setting of polycystic kidneys is not defined because of the rarity of this presentation. PHACE syndrome includes posterior fossa anomalies, hemangioma, arterial lesions, cardiac abnormalities/coarctation of aorta, and eye abnormalities. We report a 17-month-old baby with bilateral polycystic kidneys and PHACE syndrome who develo
