Knee Offloading by Patients During Walking and Running After Meniscectomy.

BACKGROUND: Changes in knee loading have been reported after meniscectomy. Knee loading has previously been assessed during jogging and treadmill running rather than overground running, which could give altered results. PURPOSE/HYPOTHESIS: The purpose of this study was to evaluate knee function during overground running and walking after meniscectomy. It was hypothesized that the affected limb would demonstrate higher external knee adduction moment, lower knee flexion moment (KFM), and lower knee rotation moment (KRM) compared with the contralateral limb and with healthy individuals. STUDY D

Delayed Closure Is Associated with Decreased Infection Rate in Amputations after Trauma.

Amputations associated with trauma carry significant morbidity in addition to risk of limb loss due to tissue damage, contamination, and risk of infection. The timing of formal amputation closure in this situation varies among surgeons, with some allowing the wound to remain open for some time after debridement, whereas others perform formal amputation and closure at the index operation if debridement is adequate. The presence of infection after closure of an amputation has the potential to result in a higher amputation (i.e., a below-knee amputation to an above-knee amputation). Our goal was

No Growth Disturbance After Trochleoplasty for Recurrent Patellar Dislocation in Adolescents With Open Growth Plates.

BACKGROUND: Trochlear dysplasia is the most important risk factor for patellofemoral instability among adolescents; therefore, trochleoplasty to reshape the trochlear groove is the treatment of choice for patients with high-grade trochlear dysplasia. However, in the presence of open growth plates, there is a potential risk of injury to the distal femoral growth plate and subsequent growth disturbance. As such, most authors do not recommend trochleoplasty for skeletally immature patients. The effect of trochleoplasty on femoral growth when performed before closure of the distal femoral physis r

The Effect of Limited Ankle Dorsiflexion During Emergency Stop-Jump Movements on Lower Limb Biomechanics.

OBJECTIVE: The stop-jump task is a key movement in sports such as basketball and volleyball, with landing biomechanics closely linked to injury risk. Restricted ankle dorsiflexion (DF) alters lower-extremity mechanics and increases the risk of lower-limb joint injuries; however, its effects on the full stop-jump cycle remain unclear. This study examined how restricted DF influences lower-extremity biomechanics during the stop-jump task and explored the mechanisms underlying a stiff landing strategy and its potential injury risks. METHODS: Participants underwent static ankle dorsiflexion range

Paraplegia caused by a thoracic angiolipoma obscured by cord injury: a case report.

BACKGROUND: Spinal epidural angiolipomas are rare benign tumors comprising 0.5-1% of all spinal neoplasms and typically arise in the thoracic spine, causing progressive myelopathy due to spinal cord compression. They feature mature adipocytes and vascular proliferation, show heterogeneous magnetic resonance imaging (MRI) enhancement, and may mimic posttraumatic or degenerative myelopathy, particularly in patients with prior spinal cord injury. CASE DESCRIPTION: A 56-year-old man presented with a 3-month history of progressive numbness and weakness in both lower limbs. He had sustained a cervi

Sports hernia or groin disruption injury? Chronic athletic groin pain: a retrospective study of 100 patients with long-term follow-up.

INTRODUCTION AND OBJECTIVES: Chronic groin pain (athletic pubalgia) is a common problem in sports such as football, hockey, cricket, baseball and athletics. Multiple co-existing pathologies are often present which commonly include posterior inguinal canal wall deficiency, conjoint tendinopathy, adductor tendinopathy, osteitis pubis and peripheral nerve entrapment. The mechanism of injury remains unclear but sports that involve either pivoting on a single leg (e.g. kicking) or a sudden change in direction at speed are most often associated with athletic pubalgia. These manoeuvres place large fo

Surgical management of posttraumatic thoracolumbar kyphosis.

BACKGROUND CONTEXT: Spine trauma is relatively common, and each year approximately 10,000 to 17,000 people in the United States will sustain a spinal cord injury and approximately 150,000 to 160,000 will fracture their spinal column. Posttraumatic spinal deformity is a common potential complication of spinal injury and poses one of the greatest challenges in spinal surgery. PURPOSE: To provide a comprehensive and current review of posttraumatic deformity focusing on the epidemiology, clinical and radiographical presentation, treatment options, and prognosis. STUDY DESIGN/SETTING: A thorough

Effective Limb Salvage Using a Distally Based Peroneus Brevis Flap for Distal Tibial Osteomyelitis.

Post-traumatic osteomyelitis of the distal third of the tibia represents a significant reconstructive challenge due to limited soft-tissue coverage, poor vascularity, and a high risk of recurrent infection. We present the case of a 60-year-old diabetic male patient who developed chronic osteomyelitis following traumatic injury to the lower third of his leg. The patient underwent staged management, including radical debridement, culture-directed antibiotic therapy, autologous cancellous bone grafting, and soft-tissue reconstruction with a distally based peroneus brevis muscle flap. A Non-Weight Bearing Method for Measuring Hip Abduction Strength Overestimates Hip Abductor Muscle Fatigue During One-Leg Stance.

OCCUPATIONAL APPLICATIONSAssessing workers' strength capacities is a common practice prior to return to work following injury or illness, or assessing capabilities for strenuous jobs. Because it requires 50% or more of maximum strength capacity, hip abductor muscle strength is a strong predictor of both middle- and older-aged individuals' ability to reliably balance on one leg and of their risk of falls. Our results suggest subjects were able to augment their hip abductor moment during unipedal weight stance via gluteus maximus activity. Weight-bearing hip abduction strength measures are impor

The Role of Physical Therapies in Wound Healing and Assisted Scarring.

Wound healing (WH) is a complex multistep process in which a failure could lead to a chronic wound (CW). CW is a major health problem and includes leg venous ulcers, diabetic foot ulcers, and pressure ulcers. CW is difficult to treat and affects vulnerable and pluripathological patients. On the other hand, excessive scarring leads to keloids and hypertrophic scars causing disfiguration and sometimes itchiness and pain. Treatment of WH includes the cleaning and careful handling of injured tissue, early treatment and prevention of infection, and promotion of healing. Treatment of underlying cond

Tibial Plateau Fracture After Attack by an Escaped Goat.

Humans come into contact with goats in wild settings. If a goat feels threatened, it may address the perceived threat violently. While out walking, a 55-y-old man was attacked by an escaped domestic goat. Much as goats interact with each other, this goat pushed him over with its horns and then rose up on 2 legs to come back down on him with its head. The man experienced a Schatzker VI bicondylar tibial plateau fracture that required external and then internal fixation. Besides his physical injuries, he experienced acute stress disorder, which is common after traumatic events. Acute stress reac

[Robot-Assisted Partial Nephrectomy for Renal Cell Carcinoma Complicated by Leriche Syndrome : A Case Report].

A 71-year-old man was referred to our hospital for treatment of a 2 cm-sized right renal mass incidentally found by computed tomography (CT) and was diagnosed with right renal cell carcinoma cT1aN0M0. Contrast-enhanced CT revealed that the aorta was completely occluded below the inferior mesenteric artery origin, and Leriche syndrome was diagnosed. CT angiography showed several collateral arteries along the abdominal wall. A robot-assisted laparoscopic partial nephrectomy was performed to treat renal cell carcinoma. Preoperatively, we marked the collateral arteries using ultrasonography to avo

Acute methemoglobinemia after a blast injury: a case report.

Methemoglobin is a structurally modified form of hemoglobin incapable of binding oxygen, and elevated levels of methemoglobin cause tissue hypoxia. Occupational exposure to 2,4,6-trinitrotoluene, commonly called trinitrotoluene, causes methemoglobinemia. This case report describes a 27-year-old male sergeant who developed methemoglobinemia upon exposure to trinitrotoluene after a blast injury while welding the walls of tank shells. This is the first case of its kind in Korea. The patient had multiple burns in his abdomen and open fractures in his right leg. While his body temperature, heart ra

Case Report of Untreated Pediatric Femoral Neck Fracture with Osteopenia.

UNLABELLED: Pediatric hip fractures are rare. This type of injury often requires surgery within 24 hours to prevent complications such as avascular necrosis, bone non-union, and premature physeal closure. Moreover, chronic disuse from untreated fracture causes severe osteopenia. Untreated pediatric hip fracture is uncommon in the developing world due to access to surgical care. Here, we report a pediatric patient who traveled from Honduras with untreated right femoral neck fracture. The delay in her initial surgical intervention left her with chronic non-union of her right hip joint and signif

Kinesiology tape mediates soccer-simulated and local peroneal fatigue in soccer players.

To investigate the efficacy of kinesiology taping in mediating the influence of fatigue on ankle sprain risk, 12 male soccer players completed single-leg dynamic balance trials pre- and post-exercise (soccer-specific protocol, isokinetic ankle inversion/eversion protocol) in each of three counter-balanced taping conditions (no tape, zinc oxide tape ZO, kinesiology tape KT). Balance was quantified as the overall stability index (OSI) and directional stability indices of platform deflection. Soccer-specific fatigue only increased OSI in the no tape condition (p = 0.03), with ZO and KT trials neg
