The six classic exanthems of childhood have many similar physical findings. Familiarity with them, as well as with important laboratory data, allows early and accurate diagnosis of these often confusing diseases. Recognition and treatment of possible sequelae also improve prognosis. In this review we discuss epidemiology, etiology, clinical manifestations, pathologic and laboratory findings, differential diagnosis, therapy, and prevention of each of the exanthems.

In 480 patients with hypsarrhythmia (H) and also in a matched control group (180), evidence for some recent infection was determined as a possible etiology for the H. Evidence for infection was found in 38% of the H patients, significantly (p less than 0.0005) different from the 23% in the control group. Although the H group more often showed specific infections with a known organism, a greater difference was seen in nonspecific infections with temperatures of at least 103 degrees F. In the developing world today, less than 20% of children are immunized against diphtheria, pertussis, tetanus, tuberculosis, measles, and poliomyelitis. Consequently, these diseases kill 10 children and disable 10 more each minute. To address this problem, the World Health Organization created the Expanded Programme on Immunization (EPI), with the goal of reducing morbidity and mortality by making immunization services available for all children of the world by 1990. A method based on a single serological test enabling the diagnosis of congenital toxoplasmosis immediately after birth is described. The concentration of anti-toxoplasmosis antibody from the CSF is compared to that of rubeola used as reference. It is thus possible to distinguish between the passive passage of antibodies in the CSF and their in situ synthesis. When concentration of antibody in the CSF is higher for toxoplasmosis than for rubeola, the existence of a congenital infection of toxoplasmosis can be suggested.
