Cannabis Use and Prolonged Opioid Utilization After Head and Neck Free Flap Reconstructive Surgery.

GOAL: This study investigates the risk of prolonged postoperative opioid use and functional outcomes of free flap reconstruction in head and neck cancer patients with a history of cannabis use. METHODS: Patients undergoing head and neck free flap reconstruction (2005-2025) were identified using the TriNetX database and stratified by cannabis use. Cohorts were propensity score-matched for demographics, substance use, and tumor characteristics. The primary outcome was postoperative opioid use at 3-12 months; secondary outcomes included tracheostomy and gastrostomy dependence. Multivariable Cox

A novel treatment for the fixation of displaced femoral neck fractures in young patients: A case report.

Displaced femoral neck fractures in physiologically young patients remain a significant treatment challenge due to high complication rates associated with internal fixation, including osteonecrosis, non-union, and hardware failure. This case report describes the successful use of a novel implant, the SimpliFix Hip System, in the surgical management of a 30-year-old female marathon runner who sustained a displaced transcervical femoral neck fracture. Following closed reduction on a fracture table, three partially threaded cannulated screws were inserted and cross-locked with headless cross scre

Long-Term Opioid Use After Treatment of Head and Neck Cancers: A Systematic Review and Meta-analysis.

OBJECTIVE: We determined the rates of long-term opioid use in head and neck cancer patients to better understand what characteristics increase the odds of long-term opioid use in these patients. DATA SOURCES: Cochrane Central, EMBASE, MEDLINE, PsycINFO, CINHAL, PubMed, and the gray literature were searched from July 2004 to July 2024. REVIEW METHODS: We included meta-analyses examining the relationship between head and neck cancers and long-term opioid use. We calculated pooled rates and compared factors using confidence intervals, chi square, and odds ratios. RESULTS: Of 90 articles retrie

Conservative treatment of 3- and 4-part proximal humeral fractures: Can poor outcomes be predicted?

PURPOSE: 3- and 4-part proximal humeral fractures are frequently treated conservatively. This study aims to combine radiographic, and patient reported outcome data to identify factors leading to poor outcomes following conservative treatment. METHODS: A retrospective local database analysis identified 3- and 4-part fractures. Radiographic and functional outcomes including Oxford Shoulder Score (OSS), QuickDash (QD), Subjective Shoulder Score (SSV) and VAS pain scores were collected. RESULTS: 104 patients were included at mean follow-up of 55 months. Analysis highlighted significant OSS diffe

Occipital neuralgia as an initial manifestation of atypical Lemierre syndrome: A case report.

Lemierre syndrome is a life-threatening but treatable septic thrombosis of the internal jugular vein (IJV) derived from a craniocervical bacterial infection. Metastatic septic embolization is common. However, diagnostic delay and poor prognosis remain a problem, largely due to the diverse clinical presentations and unclear symptomatology of Lemierre syndrome. In contrast, occipital neuralgia is a common disease typically treated with symptomatic therapy in emergency settings, as most cases are not life-threatening. The present case involved a 70-year-old female with Lemierre syndrome stemming

A Systematic Review and Meta-Analysis of the Efficacy of Buddy Taping Versus Reduction and Casting for Non-operative Management of Closed Fifth Metacarpal Neck Fractures.

A closed fifth metacarpal neck fracture is a frequently encountered upper limb fracture that occurs when the bone breaks right below the little finger's knuckle. At the moment, there is no agreement on the best way to treat these fractures. This research seeks to look at the efficacy of buddy taping versus reduction and casting for non-operative management of uncomplicated closed fifth metacarpal neck fractures. A systematic review of PubMed, Medical Literature Analysis and Retrieval System Online (MEDLINE), PubMed Central (PMC), and the Cochrane Library databases was carried out using the Pre

Osteoradionecrosis of the Occipitocervical Junction After Radiation for Head and Neck Cancer: A Report of 8 Cases.

BACKGROUND: Osteoradionecrosis (ORN) refers to the degenerative changes seen in bone after local radiation, particularly in head and neck cancer. ORN can present as neck or facial pain and may be confused with tumor recurrence. Magnetic resonance imaging (MRI) and positron emission tomography (PET) scans are often inconclusive, requiring percutaneous biopsy to differentiate ORN from infection and recurrent disease. We reviewed the utility of preprocedural imaging in guiding the decision to biopsy in cases of ORN. CASE DESCRIPTION: Eight patients with a history of prior head and neck cancer, r

Ayurvedic management in cervical spondylotic myelopathy.

The age related spondylotic changes may result in direct compressive and ischemic dysfunction of the spinal cord known as cervical spondylotic myelopathy (CSM). Symptoms often develop insidiously and are characterized by neck stiffness, unilateral or bilateral deep aching neck, arm and shoulder pain, and possibly stiffness or clumsiness while walking. The management available in current mainstream medicine is not satisfactory. Various Ayurvedic treatments have been in use for these manifestations. We present a case of CSM, which was treated with a combination of Panchakarma procedures and Ayur
