Melatonin Prescribing in Assisted Living.

OBJECTIVES: Aging is associated with sleep problems, which are common among assisted living (AL) residents. More than 40% of residents have insomnia, and almost 70% have a sleep disturbance of some kind. Melatonin to treat sleep disorders has become common among older adults, but its use in AL is unknown, which is concerning because melatonin may have side effects and interfere with other medications. The purpose of this study was to determine the prevalence and correlates of melatonin prescribing in AL, to inform whether changes in care may be indicated. DESIGN: Cross-sectional chart abstrac

Development of a questionnaire to evaluate practitioners' confidence and knowledge in primary care in managing chronic kidney disease.

BACKGROUND: In the UK, chronic disease, including chronic kidney disease (CKD) is largely managed in primary care. We developed a tool to assess practitioner confidence and knowledge in managing CKD compared to other chronic diseases. This questionnaire was part of a cluster randomised quality improvement interventions in chronic kidney disease (QICKD; ISRCTN56023731). METHODS: The questionnaire was developed by family physicians, primary care nurses, academics and renal specialists. We conducted three focus groups (n=7, 6, and 8) to refine the questionnaire using groups of general practition

Practice variability in management of acute respiratory distress syndrome: bringing evidence and clinician education to the bedside using a web-based teaching tool.

BACKGROUND: Clinical practice often lags behind publication of evidence-based research and national consensus guidelines. OBJECTIVE: To assess practice variability in the clinical management of acute respiratory distress syndrome (ARDS) and test an evidence-based, online clinician-education tool designed to improve intensive-care clinicians' understanding of current evidence about ARDS management. METHODS: We surveyed 117 intensive care clinicians (16 critical care physician specialists, 28 resident physicians, 50 critical care nurses, and 23 respiratory therapists) with an online questionna

Understanding respiratory rate as a predictor of clinical deterioration and mortality.

Respiratory rate (RR) is a fundamental physiological observation and a key component of early warning systems used to identify deterioration in hospitalised adults. Accurate RR measurement and interpretation, alongside other clinical observations and timely escalation of abnormal findings, are essential to ensure safe patient care. However, despite its clinical importance, RR remains one of the least reliably measured vital signs in practice and it is frequently estimated rather than counted accurately. This article explores the evidence on RR as a predictor of clinical deterioration and morta

Falls prevention in hospitalised older adults with cognitive impairment: strategies and ethical implications.

Falls are the most commonly reported patient safety incident in hospitals in the UK, and a major cause of patient harm. Older adults with cognitive impairment, such as dementia or delirium, are at particular risk of falls in acute settings due, for example, to attempting to mobilise without support, inability to communicate their needs or misjudging danger. Nurses have a central role in falls prevention, balancing patient safety with preserving individuals' dignity and autonomy to ensure that practice is consistent, compassionate and ethically sound. This article explores strategies for reduci

This retrospective single-center study describes the use and content of Advance Care Planning (ACP) forms in a sample of 50 anonymized medical records of patients enrolled in a home-based primary palliative care program. ACP was documented in 62% of cases, with frequent reporting of hospitalization preferences (90.3%) and appointment of a healthcare proxy (90.3%), whereas treatment decisions and palliative sedation were less frequently addressed (45.2% and 16.1%, respectively). ACP discussions always involved the patient and the general practitioner (100%), with frequent participation of famil

Stepping it Up on Temporary Mechanical Circulatory Support: Mobilization and Prehabilitation of Patients in ICUs.

Early mobilization and prehabilitation in patients supported with temporary mechanical circulatory support (tMCS) have emerged as promising strategies to mitigate the deconditioning and sarcopenia associated with critical illness and immobility. While small studies have demonstrated that ambulation with tMCS is both safe and feasible in selected patients, significant variability exists in practice patterns across centers. Implementation of early mobility protocols requires a multidisciplinary team, including cardiology, surgery, critical care, physical therapy, respiratory therapy, nursing, an

The Evolution of Transcatheter Aortic Valve Replacement: From Novel to Normal.

Over the past decade, transcatheter aortic valve replacement (TAVR) has become a transformative intervention for patients with aortic stenosis (AS). With a minimally invasive approach, TAVR reduces overall risks to the patient compared to the traditional surgical aortic valve replacement. Improvements in technology, training, and access to registered TAVR-designated sites, as well as the high success rate of the procedure, have turned TAVR from a novel approach into the preferred, first-line treatment for patients with AS. The purpose of this review article is to educate RNs and advanced pract

Injection site abscesses caused by Mycobacterium chelonei.

Subcutaneous abscesses occurred at hip and deltoid injection sites in 13 student nurse assistants following practice injections. Mycobacterium chelonei was isolated from three cases and was the presumptive pathogen in other temporally clustered cases with similar clinical findings. A 1,000 cc bottle of 0.9 N saline used as a source for practice injection solution was the apparent source of the organism. Presenting findings included tenderness (7), swelling (7), fever (3), adenopathy (1) and weight loss (1). All cases were characterized by long incubation periods (6 to 16 weeks) and delayed res

Cardiovascular diseases, prevention, and management of complications in older adults and frail patients treated for elective or post-traumatic hip orthopaedic interventions: a clinical consensus statement of the ESC Council for Cardiology Practice (CCP), the European Association of Preventive Cardiology (EAPC), the Association for Acute CardioVascular Care (ACVC), the Association of Cardiovascular Nursing & Allied Professions of the ESC (ACNAP), the ESC Working Group on Aorta and Peripheral Vascular Diseases (WG APVD), and the ESC Working Group on Thrombosis (WG T).

Due to the aging population, focusing on healthy aging has become a global priority. Cardiovascular diseases (CVDs) and frailty, characterized by increased vulnerability to adverse stress and health events, interact synergistically in advanced age. In older adults, hip fractures are a frequent dramatic "life-transition" event. Conditions such as arrhythmias, orthostatic hypotension, heart failure, peripheral artery disease and adverse drug reactions may facilitate falls and thus bone fractures in older adults. Cardiovascular complications or the worsening of previous CVDs may increase the degr
