Reconstruction of the pediatric flexible planovalgus foot by using an Evans calcaneal osteotomy and augmentative medial split tibialis anterior tendon transfer.

Seventeen consecutive pediatric patients (34 feet) underwent reconstruction of flexible pes planovalgus deformity between 1994 and 1999. The average follow-up was 24.9 months (range, 8 to 48 months). All patients underwent a medial split tibialis anterior tendon transfer/tenodesis in combination with an Evans calcaneal-lengthening osteotomy, Kidner or modified Kidner procedure, and tendo-Achilles lengthening. Various angular parameters were measured both preoperatively and postoperatively, and improvement in each was noted. The calcaneal pitch improved by an average of 26 degrees (85.88%), the

A healthy hemophilic patient without arthropathy: from concept to clinical reality.

Hemophilia is characterized by joint hemorrhage with subsequent development of arthropathy. During the past 40 to 50 years, factor VIII concentrates have been developed that enable effective treatment of bleeding. However, the development of dosing, including the implementation of prophylaxis, has been hampered by concentrate transmission of blood-borne diseases. Since the advent of viral reducing procedures that were applied to plasma-derived products and the implementation of recombinant DNA technology into the manufacturing procedure, concentrates can be considered as safe today, and hemoph

The Use of Augmented Reality for Navigation in Minimally Invasive Abdominal and Thoracic Soft-Tissue Surgery: A Systematic Review.

Surgical navigation and augmented reality (AR) are widely used in neurosurgery, spinal surgery, and orthopedics. However, their use in minimally invasive abdominal and thoracic soft-tissue surgery is limited, as tracking deformable, mobile organs is challenging. Recent advances in AR may address these challenges to improve intraoperative navigation. This systematic review, registered in PROSPERO (2024) and based on PRISMA guidelines, analyzes literature from 2014 to 2024 about AR in minimally invasive abdominal and thoracic soft-tissue surgery. It identifies target organs, describes AR hardwar

Postoperative C5 Palsy Following Posterior Cervical Decompression and Fusion: A Case Report.

INTRODUCTION: Posterior cervical decompression and fusion (PCDF) is a well-established surgical procedure for the management of multilevel cervical spinal cord compression. Although generally safe, postoperative C5 palsy is a recognized complication that can significantly affect functional recovery. CASE REPORT: We report the case of a patient with multilevel cervical compressive myelopathy who underwent posterior cervical decompression and fusion. The immediate postoperative period was complicated by new-onset weakness of shoulder abduction and elbow flexion, consistent with C5 palsy. Postop

Management of a Nonunion Type IIC Monteggia Fracture.

INTRODUCTION: Infected non-union of long bones represents one of the most challenging problems in orthopedic surgery. Conventional two-stage approaches involving debridement, antibiotic spacers, and subsequent fixation are associated with high morbidity and prolonged treatment duration. MATERIALS AND METHODS: We present a prospective case series of 10 patients with infected non-union of various long bones (tibia, femur, humerus) treated with antibiotic cement-coated intramedullary nails (AC-CIMN). The nail was coated with polymethylmethacrylate mixed with culture-sensitive antibiotics in a si

Bacterial colonization of bone substitute materials used in oral surgery: mechanisms, clinical implications, and preventive strategies-A narrative review.

The advancement of tissue engineering and the development of novel biomaterials have opened new possibilities for the effective treatment of patients with edentulism and other dental deficiencies, as well as for the prosthetic reconstruction and functional rehabilitation of the stomatognathic system. Bone substitute materials are now widely used in orthopedics, reconstructive surgery, and dentistry to support the regeneration of bone tissue lost due to trauma, inflammation, or tooth extraction. However, surgical procedures within the oral cavity inherently carry a risk of postoperative infecti

Mg-Cross-Linked Alginate Hydrogel Induces BMSC/Macrophage Crosstalk to Enhance Bone Tissue Regeneration via Dual Promotion of the Ligand-Receptor Pairing of the OSM/miR-370-3p-gp130 Signaling Pathway.

Macrophages play a pivotal role in the crosstalk between the immune and skeletal systems, while Mg-based biomaterials demonstrate immunomodulatory capabilities in this procedure. However, the mechanism of how Mg2+ promotes osteogenesis through the interplay of bone marrow-derived mesenchymal stem cells (BMSCs) and macrophages remains undescribed. Here, we demonstrated that a Mg-cross-linked alginate hydrogel exerted a dual enhancement of BMSCs osteogenic differentiation through the ligand-receptor pairing of the OSM/miR-370-3p-gp130 axis. On the one hand, Mg2+, released from the Mg-cross-linke

Exploring Comorbidities in Adolescent and Young Adults with Hypermobile Ehlers-Danlos Syndrome with and without a Surgical History: A Preliminary Investigation.

Ehlers-Danlos Syndrome (EDS) is a rare disease affecting the skin, joints, vasculature, and internal organs. Approximately 85% of those affected are categorized as the hypermobile type (hEDS), which is associated with numerous medical and psychiatric comorbidities, including chronic pain. Additionally, approximately 71% of patients with hEDS undergo at least one surgical procedure; however, indicators for surgery and pain outcomes after surgery are poorly understood. This preliminary study used a medical chart review to identify the frequency and nature of comorbidities in a cohort of adolesce

Impact of local delivery of allogeneic chondrocytes on the biological response and healing of the sternum bones after sternotomy.

Median sternotomy is the surgical method of choice for many procedures where one of the main problems is the long post-operative wound healing process leading to sternal dehiscence and the development of infection. This leads to prolonged hospital stay and increased mortality due to post-operative complications. A promising solution seems to be the use of allogeneic chondrocytes for wound treatment, whose properties in the field of cartilage reconstruction are widely used in medicine, mainly in orthopedics. In the present study, we investigated the effect of local delivery of allogeneic chondr

[Postoperative multiprofessional comprehensive treatment].

BACKGROUND: In orthogeriatric centers, postoperative, multiprofessional comprehensive treatment has proven to be an effective component in the convalescence of geriatric patients. The aim of the treatment is to minimize the perioperative risk and safely achieve individual rehabilitation goals in the acute inpatient stay. To meet the needs of geriatric patients, in addition to changes in the spatial division and design, primarily adjustments to the team composition and the procedural processes are required. THERAPEUTIC STRATEGIES: An interdisciplinary and multiprofessional team (orthopedics/tr

Long-Term Evaluation of Poly(lactic acid) (PLA) Implants in a Horse: An Experimental Pilot Study.

In horses, there is an increasing interest in developing long-lasting drug formulations, with biopolymers as viable carrier alternatives in addition to their use as scaffolds, suture threads, screws, pins, and plates for orthopedic surgeries. This communication focuses on the prolonged biocompatibility and biodegradation of PLA, prepared by hot pressing at 180 °C. Six samples were implanted subcutaneously on the lateral surface of the neck of one horse. The polymers remained implanted for 24 to 57 weeks. Physical examination, plasma fibrinogen, and the mechanical nociceptive threshold (MNT) we

[Classification for mandibular deviation and facial asymmetry and its clinical significance].

Judged from the clinical manifestations, etiological mechanisms and jaw morphological structures, malocclusions with mandibular deviation and facial asymmetry are classified into three main categories, i.e., those with mandibular positioning factors, those with mandibular condyle defects, and those with jaw congenital deformities. In the first category, three subtypes, including mandibular rotation caused by dentitional irregularities, by dental compensation and by habitual posturing, are further defined. In the second category, mandibular deviation caused by condylar asymmetrical resorption o

Wide-awake local anesthesia for clavicle fracture fixation: A case report.

INTRODUCTION: The wide-awake local anesthesia no tourniquet (WALANT) technique has been used in many orthopedic surgeries. The benefits of this technique are the avoidance of the adverse effects of general anesthesia (GA) and the overall reduction of the cost of surgery. However, a literature search revealed no published report on performing the WALANT technique for clavicular fracture surgery. PRESENTATION OF CASE: We report a case of mid-shaft clavicular fracture that was treated with plate osteosynthesis using the WALANT technique in a patient with uncontrolled atrial fibrillation. Beyond immunosuppressive effects: dual roles of myeloid-derived suppressor cells in bone-related diseases.

Myeloid-derived suppressor cells (MDSCs) are a heterogeneous population of immature myeloid cells (IMCs) with immunosuppressive functions, whereas IMCs originally differentiate into granulocytes, macrophages, and dendritic cells (DCs) to participate in innate immunity under steady-state conditions. At present, difficulties remain in identifying MDSCs due to lacking of specific biomarkers. To make identification of MDSCs accurately, it also needs to be determined whether having immunosuppressive functions. MDSCs play crucial roles in anti-tumor, angiogenesis, and metastasis. Meanwhile, MDSCs co

Surgical implications of the hip-spine relationship in total hip arthroplasty.

Total hip arthroplasty (THA) is considered the most successful orthopedic surgical procedure of the last century with excellent survivorship up to 20-years. However, instability remains a major issue representing the most common reason for revision after THA. Hip-spine relationship has gained progressive interest between arthroplasty surgeons and its understanding is crucial in order to identify high-risk patients for postoperative dislocation. Spinal deformity and abnormal spinopelvic mobility have been associated with increased risk for instability, dislocation and revision THA. Preoperative

The Medium-Term Survival Analysis of an All-Polyethylene Tibia in a Single-Series Cohort of Over 1000 Knees.

BACKGROUND: Total knee arthroplasty is a successful elective orthopedic procedure with an increasing number being undertaken. Original knee arthroplasties used an all-polyethylene tibia; however, with concerns over tibial loosening, the trend moved toward the metal-backed variety. Modern designs providing more conformity and changes in manufacturing of the polyethylene make it an equivalent but cheaper option. METHODS: We analyzed the medium-term outcome in 1092 patients with an all-polyethylene tibial component in their total knee arthroplasty. RESULTS: Twenty-six patients had further surge

Hypersensitivity to Orthopedic Implants: A Review of the Literature.

Awareness of rare etiologies for implant failure is becoming increasingly important. In addition to the overall increase in joint arthroplasties, revision surgeries are projected to increase dramatically in the coming years, with volume increasing up to seven-fold between 2005 and 2030. The literature regarding the relationship between metal allergy and implant failure is controversial. It has proven difficult to determine whether sensitization is a cause or a consequence of implant failure. Testing patients with functional implants is not a clinically useful approach, as the rate of hypersens
