Exploring a cherubism bone phenotype outside the craniofacial region.

Cherubism is a rare paediatric bone disease caused by gain-of-function mutations in the SH3BP2 gene. This condition is characterized by osteolysis of the jaw bone, which can be sometimes massive, whereby bone is replaced by fibrous tissue containing osteoclast-like multinucleated giant cells. Recently, a patient with a severe cherubism was reported to have, in addition to the craniofacial cherubism features, a low bone mass phenotype. To determine whether this patient was the exception or the rule, the cherubism phenotype was explored in ten more patients outside the cranio-facial. For the fir

Extracellular Vesicles and Their Role in Osteogenesis.

Extracellular vesicles (EVs) have emerged as promising cell-free therapeutic agents for bone regeneration due to their ability to modulate osteogenesis, angiogenesis, and immune responses. This review summarizes the current knowledge on EV biology, including their classification, biogenesis, and molecular cargo, with a particular focus on their role in bone healing. EVs derived from mesenchymal stromal cells and other cell types promote osteoblast differentiation, inhibit osteoclast activity, enhance vascularization, and modulate inflammation through the transfer of bioactive proteins and non-

Spatial transcriptomic atlas of aggressive osteosarcomas reveals shared immune landscape and targetable surface markers.

Osteosarcoma is characterized by extensive inter- and intra-tumoral heterogeneity, contributing to treatment resistance and poor outcomes. Here, we present a comprehensive spatial transcriptomics analysis of osteosarcoma, encompassing primary tumors and local or metastatic relapses across diverse phenotypic subtypes. Despite this heterogeneity, we identify a nine-gene cell surface signature with theranostic potential, validated in independent datasets and shown by immunohistochemistry to be distributed across distinct tumor compartments, supporting multi-targeted therapeutic strategies. Analys

Pharmacological mechanism of natural products to treat osteoporosis: a focus on the autophagy.

Osteoporosis (OP) is a serious public health issue, and fragility fractures resulting from OP are a major cause of death in the elderly. Osteoblast (OB)-mediated insufficient bone formation and osteoclast (OCs)mediated abnormalities in bone destruction can result in OP. Autophagy is the process by which cells degrade and recycle their own proteins and organelles. The differentiation and activity of OBs, OCs, and other bone cells depend on autophagy activity. The regulation of autophagy has the potential to influence the metabolic processes of these cells, which may contribute to the treatment

The role of lactylation modifications in musculoskeletal diseases: From mechanism to clinical application.

Lactylation, a recently discovered histone and non-histone modification driven by lactate, has redefined the understanding of how metabolic by-products regulate gene expression. Beyond its conventional role as an energy substrate, lactate emerges as a pivotal signaling mediator that couples cellular metabolism with epigenetic regulation. In musculoskeletal disorders, aberrant lactylation has been implicated in the disruption of osteogenic-osteoclastic balance, extracellular matrix homeostasis, and regenerative capacity, thereby linking cellular metabolic stress to pathological tissue remodelin

Sirtuin 5-mediated desuccinylation of Slc25a4 inhibits osteoporosis by enhancing mitochondrial respiration.

Osteoporosis is a prevalent metabolic bone disorder that develops when osteoclast-mediated bone resorption chronically exceeds osteoblast-driven bone formation. The molecular pathways that govern osteogenic dysfunction and connect cellular metabolism to differentiation regulation remain poorly characterized. Here, we identify Sirtuin 5 (Sirt5) as a pivotal osteogenic regulator through bioinformatic screening and functional validation in Sirt5-knockout mice. Mechanistically, Sirt5 governs mitochondrial homeostasis by desuccinylating Solute Carrier Family 25 Member 4 (Slc25a4) at lysine 147 (K14

Exosomes promise better bone regeneration.

Fractures primarily result from high-energy trauma, leading to structural discontinuity of bone tissue. Contemporary therapeutic approaches continue to face persistent challenges including nonunion, infection, and inflammatory complications that pose significant clinical management difficulties. Emerging evidence demonstrates that extracellular vesicles (EVs), particularly exosomes, serve as critical mediators in diverse pathophysiological processes. Accumulating studies reveal that exosomal cargos enhance osteogenesis and angiogenesis through dynamic regulation of cellular components and mole

Beta-Catenin Positive Giant Cell Tumor of Bone with Pathological Fracture Treated with Neoadjuvant Denosumab Therapy for Visualizing Extraskeletal Lesions.

INTRODUCTION: Giant cell tumors of bone (GCTBs) are locally aggressive tumors that can induce pathological fractures. Previous reports suggest that denosumab induces intratumoral ossification in β-catenin positive GCTB. However, its effect on the complete resection of GCTB with extraskeletal lesions remains unclear. CASE PRESENTATION: A 45-year-old woman with a pathological fracture of the left distal femur was diagnosed with GCTB based on the biopsy findings, which revealed both H3.3 G34W and nuclear β-catenin positivity. Eight courses of neoadjuvant denosumab therapy were administered to vi

Spontaneous Anterior Arch Fracture of the Atlas Following C1 Laminectomy in a Patient With Osteopetrosis: A Case With Five Years of Follow-Up.

Osteopetrosis is a rare group of genetic disorders characterized by excessive bone density due to impaired osteoclast function and can lead to various complications, including fractures and immune dysfunction. We describe the case of a 63-year-old man with osteopetrosis who presented with cervical discomfort and was diagnosed with an anterior arch fracture of the atlas, a rare spontaneous fracture following C1 laminectomy. Initially, no neurological abnormalities were observed, and imaging confirmed the continuity of the transverse ligament. Thus, the decision was made to follow the natural co

Cellular mechanisms of osteoporosis: A comprehensive perspective on ferroptosis, cuproptosis and lipid metabolism abnormalities.

Osteoporosis is a prevalent skeletal disorder characterised by reduced bone mineral density and compromised bone microarchitecture, leading to increased bone fragility and increased risk of fracture. Recent studies have implicated ferroptosis, cuproptosis, and dysregulated lipid metabolism as pivotal factors in the pathogenesis of osteoporosis. Ferroptosis is an iron-dependent form of cell death that disrupts the balance between osteoblast-mediated bone formation and osteoclast-mediated bone resorption by promoting lipid peroxidation and inducing cellular injury. Cuproptosis, associated with d

Intraarticular Nodular Fasciitis of the Elbow Confirmed by UPS6::MYH9 Gene Fusion.

Nodular fasciitis is a benign, usually self-limiting myofibroblastic proliferation with a predilection for the upper extremities, trunk, and head and neck, and almost all of which harbor the USP6::MYH9 fusion. Since nodular fasciitis is not widely recognized to arise within the joints, it may therefore cause diagnostic confusion in this uncommon setting. We report an unusual tumor of an 11-year-old patient who presented with a 6-month history of right elbow swelling and pain. Histology revealed that the tumor was composed solely of proliferating bland spindle cells with osteoclast-like giant c

WHIM Syndrome-linked CXCR4 mutations drive osteoporosis.

WHIM Syndrome is a rare immunodeficiency caused by gain-of-function CXCR4 mutations. Here we report a decrease in bone mineral density in 25% of WHIM patients and bone defects leading to osteoporosis in a WHIM mouse model. Imbalanced bone tissue is observed in mutant mice combining reduced osteoprogenitor cells and increased osteoclast numbers. Mechanistically, impaired CXCR4 desensitization disrupts cell cycle progression and osteogenic commitment of skeletal stromal/stem cells, while increasing their pro-osteoclastogenic capacities. Impaired osteogenic differentiation is evidenced in primary

FTY720 Attenuates LPS-Induced Inflammatory Bone Loss by Inhibiting Osteoclastogenesis via the NF-κB and HDAC4/ATF Pathways.

Osteoclast (OC) abnormalities lead to many osteolytic diseases, such as osteoporosis, inflammatory bone erosion, and tumor-induced osteolysis. Exploring effective strategies to remediate OCs dysregulation is essential. FTY720, also known as fingolimod, has been approved for the treatment of multiple sclerosis and has anti-inflammatory and immunosuppressive effects. Here, we found that FTY720 inhibited osteoclastogenesis and OC function by inhibiting nuclear factor kappa-B (NF-κB) signaling. Interestingly, we also found that FTY720 inhibited osteoclastogenesis by upregulating histone deacetylas

Caffeic acid phenethyl ester ameliorates titanium particle-induced bone loss and inflammatory reaction in a mouse acute model.

With the increasing clinical application of dental and orthopedic implants, the problem of peri-implant osteolysis has attracted attention. The inflammatory response and osteoclast differentiation induced by wear particles play an important role in peri-implant bone loss. However, the treatment of peri-implant osteolysis is still lacking. In the present study, we investigated the effect of caffeic acid phenethyl ester (CAPE) on titanium particles induced bone loss in a mouse model. We found that CAPE significantly suppressed titanium particle-induced bone loss in vivo. CAPE treatment decreased

Oxidative Stress and Inflammation in Osteoporosis: Molecular Mechanisms Involved and the Relationship with microRNAs.

Osteoporosis is characterized by the alteration of bone homeostasis due to an imbalance between osteoclastic bone resorption and osteoblastic bone formation. Estrogen deficiency causes bone loss and postmenopausal osteoporosis, the pathogenesis of which also involves oxidative stress, inflammatory processes, and the dysregulation of the expression of microRNAs (miRNAs) that control gene expression at post-transcriptional levels. Oxidative stress, due to an increase in reactive oxygen species (ROS), proinflammatory mediators and altered levels of miRNAs enhance osteoclastogenesis and reduce ost

Quantification of Osteoprotegerin Plasma Levels in Patients with Periodontitis.

OBJECTIVE: Periodontitis (POD) is an infectious process directed at the structures supporting the teeth. Destruction of alveolar bone is considered one of the main causes of tooth loss in humans and is mediated by the host immune response. Osteoprotegerin (OPG), a protein that inhibits bone resorption by binding to the RANK ligand (RANKL), prevents osteoclastic differentiation. The aim of the study was to determine the plasma levels of OPG in patients with POD. METHODS: a case-control study with forty-nine patients with POD and 49 healthy controls were included in the study. OPG levels were d

Three Classes of Antioxidant Defense Systems and the Development of Postmenopausal Osteoporosis.

Osteoporosis is a common bone imbalance disease that threatens the health of postmenopausal women. Estrogen deficiency accelerates the aging of women. Oxidative stress damage is regarded as the main pathogenesis of postmenopausal osteoporosis. The accumulation of reactive oxygen species in the bone microenvironment plays a role in osteoblast and osteoclast apoptosis. Improving the oxidative state is essential for the prevention and treatment of postmenopausal osteoporosis. There are three classes of antioxidant defense systems in the body to eliminate free radicals and peroxides including anti

Synchronous polyostotic solid variant of aneurysmal bone cyst involving thoracic vertebrae and harboring a novel AHNAK::USP6 gene fusion. A case report and review.

Aneurysmal bone cyst (ABC) is a benign bone neoplasm that usually affects the metaphysis of long bones and the posterior elements of vertebral bodies. The rearrangement of USP6 gene is present in most of primary ABC cases. Synchronous polyostotic presentation is extremely rare. All of the eight reported cases in literature have a classic ABC histomorphology, including dilated-blood filled cystic spaces separated by fibrous septa and composed of variably cellular bland fibroblasts with scattered osteoclast-like giant cells and reactive new bone formation. Herein, we report a case of a 29-year-o

Osteoprotective effect of the marine alkaloid norzoanthamine on an osteoporosis model in ovariectomized rat.

Norzoanthamine (NZ), an alkaloid that has been isolated from the marine cnidiaria Zoanthus sp., has been shown an interesting anti-osteoporotic activity. Although its mechanism of action is not yet clear, it seems that it is different from those of currently used drugs making it particularly interesting. Previous studies have been carried out mostly in vitro. Herein, we present an in vivo study that allows to check the real potential of NZ as a protector substance by direct application into ovariectomized rat bone using a sustained delivery system. Histological and histomorphometric results in
