Endoscopic indigo carmine spraying for evaluation of intestinal mucosal permeability: Prospective pilot study.

BACKGROUND AND STUDY AIMS: "Leaky gut," caused by increased mucosal permeability, plays a pivotal role in various diseases. However, few methods are available to evaluate intestinal mucosal permeability in the living human body. We established a novel method for evaluation of mucosal permeability using indigo carmine (IC). PATIENTS AND METHODS: Subjects undergoing colonic endoscopy for screening of colon polyps or evaluation of ulcerative colitis (UC) severity were enrolled. IC was endoscopically sprayed in the cecum, and blood samples were obtained before spraying and at 30 and 60 minutes af

Integrating silver diamine fluoride into school-based oral health programs: A pilot study.

OBJECTIVE: Current approaches to school-based caries prevention can increase access to oral healthcare, but are often limited by costs and other logistical challenges. Evidence from large pragmatic trials support the use of silver diamine fluoride (SDF) to prevent and control caries in school dental programs. In this pilot, we developed implementation strategies and integrated SDF into an existing school-based dental program, Smiles For Students (SFS). METHODS: This was a single-group observational pilot. Using a school-based SDF implementation toolkit, SFS clinicians received training in cli

Phase 1B pilot study of itacitinib with alemtuzumab in patients with T-cell prolymphocytic leukemia.

T-cell prolymphocytic leukemia (T-PLL) is a mature T-cell neoplasm with an aggressive clinical course. Overall prognosis is poor, and treatment relies on alemtuzumab because of inadequate response to conventional chemotherapy. Three-quarters of cases harbor activating mutations in the JAK-STAT pathway (JAK1, JAK3, STAT5B, IL2RG). We report safety and efficacy from a phase 1B study evaluating the combination of the JAK1 inhibitor itacitinib with alemtuzumab. Patients (N = 15) were aged >18 years, with treatment-naïve (n = 8) or relapsed/refractory (n = 7) T-PLL with adequate organ function, Eur

Impact of Chemotherapy Alone and in Combination with Immunotherapy on Oral Microbiota in Cancer Patients-A Pilot Study.

The oral cavity harbors a highly intricate and dynamic microbial ecosystem of multiple microhabitats supporting diverse microbial populations. As the second most complex microbiome in the human body, surpassed only by the gut, the oral microbiome comprises over 1000 species. Disruptions in the microbial balance have been associated with an increased risk of both oral diseases (dental caries and periodontitis) and systemic conditions, including inflammatory diseases and certain types of cancers. In our pilot study, we purified bacterial DNA from pre-treated, saponin-based, host-depleted saliva

Inversion time optimization in postmortem 1.5 tesla FLAIR brain imaging: a pilot study.

INTRODUCTION: Postmortem magnetic resonance imaging (PMMR) has gained importance during the last decade in forensic pathology. While many clinical radiology sequences are applicable for the evaluation of the brain, the 3D FLAIR sequence shows different contrast in postmortem cases compared to living patients. Two factors-the temperature and the interval between official declaration of the death and PMMR (DC-PMMR interval) are suspected to influence the optimal inversion time (TI) needed to achieve living patient-like image contrast. This study aimed to investigate if our empirical approach had

Standardized ultrasound assessment with a classification system in greater trochanteric pain syndrome: a pilot study.

BACKGROUND: Ultrasound (US) plays an important role in identifying the underlying cause of greater trochanteric pain syndrome (GTPS). However, establishing standardized US protocols and diagnostic criteria is essential for reproducible grading of GTPS severity. Such standardization would enhance radiological assessment, facilitating more informed treatment decisions by clinicians and patients. In this study, we performed standardized US examination and interpretation, and proposed a US classification system to better understand GTPS progression and severity. METHODS: In this prospective cross

Mutational Analysis of Bile Cell-Free DNA in Primary Sclerosing Cholangitis: A Pilot Study.

BACKGROUND: Primary sclerosing cholangitis (PSC) is a chronic liver disease characterised by inflammation and fibrosis of the bile ducts, conferring an increased risk of cholangiocarcinoma (CCA). However, detecting CCA early in PSC patients remains challenging due to the limited sensitivity of conventional diagnostic methods, including imaging or bile duct brush cytology during endoscopic retrograde cholangiopancreatography (ERCP). This study aims to evaluate the potential of bile cell-free DNA (cfDNA) mutational analysis, termed the Bilemut assay, as a tool for CCA detection in PSC patients.
