Gut Microbiota as a Mediator of Dietary Salt Effects on Blood Pressure.

Dietary sodium excess is a primary driver of hypertension, yet individuals differ markedly in their blood pressure response to salt. This variation, termed salt sensitivity, cannot currently be predicted from clinical variables alone. This review examines three aspects of salt-gut physiology: intestinal sodium handling, salt-induced changes in gut microbiome composition, and microbiota-mediated effects on immune function, metabolite production, and gut barrier integrity. The intestine absorbs dietary sodium through regulated transporters whose activity adapts to luminal and hormonal conditions…

[Individual Differences in the Impact of Habitual Alcohol Drinking on Blood Pressure].

Habitual alcohol consumption is an established risk factor for hypertension. To prevent hypertension, daily alcohol consumption is generally recommended to be less than 20-30 ml in men and 10-20 ml in women. The association between alcohol consumption and hypertension is affected by various factors, including age, sex, body weight, genetic disposition, drinking patterns, smoking habits, and medication for hypertension. The difference in blood pressure between drinkers and nondrinkers tended to be more prominent in men than in women, in the elderly than in the young, in individuals with high al…

Is Prevena a better choice for patients with pressure ulcers after reconstruction?

OBJECTIVE: Prevena (3M KCI, US) is a new incisional negative pressure wound therapy system (iNPWT) for closed surgical incision management. The surgical management of pressure ulcers (PUs) remains challenging. A high rate of recurrence and overall complications have been reported. This study analysed the clinical efficacy of the iNPWT system in patients with PUs. METHOD: In this retrospective study, patients with stage 3 or 4 PUs who were surgically managed by a single plastic surgeon specialising in PUs at Tri-Service General Hospital, Taiwan, between January 2011 and June 2022 were reviewed…

Overlapping Hyperosmolar Therapy in Patients With Elevated and Refractory Intracranial Pressure.

OBJECTIVES: Elevated intracranial pressure (ICP) remains a challenge and immediate danger in patients with acute brain injury. Current literature supports the use of a single hyperosmolar agent to reduce ICP. However, the efficacy and safety of overlapping hyperosmolar agents have not been evaluated. The objectives of this study are to analyze the ICP lowering effects and evaluate the safety profile of overlapping hyperosmolar therapy. MATERIALS AND METHODS: This single-center, retrospective study included adults (≥18 y) admitted to the Neurosciences Intensive Care Unit between May 2018 and S…

Changes in Dynamic Plantar Pressure and Asymmetry in AIS Following PSSE.

UNLABELLED: Adolescent idiopathic scoliosis (AIS) may manifest with instability and abnormal plantar pressure during gait. However, there is limited objective evidence to support efficacy of short-term physiotherapeutic scoliosis specific exercises (PSSE) with or without bracing on the improvement of plantar pressure. Thus, this study aimed to (1) compare various plantar pressure parameters and their symmetry indexes during gait before and after PSSE; (2) assess those differences during gait between braced and non-braced groups. METHODS: Our study included AIS patients with surface topography…

Pro-Homeostatic Effects of Estrogen on Intraocular Pressure and the Trabecular Meshwork.

PURPOSE: Elevated intraocular pressure (IOP) results from the dysregulation of aqueous humor outflow through the mechanoresponsive trabecular meshwork (TM). Evidence suggests that low estrogen (E2) and high TGFβ2 levels are risk factors for elevated IOP and primary open-angle glaucoma. We sought to examine whether E2 signaling is involved in TM regulation of IOP homeostasis. METHODS: IOP and central cornea thickness were measured in male and female Esr1-/- mice and wild-type littermate controls from 3 to 12 months of age. Primary human TM cells (n = 10, 6 females and 4 males) were treated wit…

Influence of Pullback Pressure Gradient on Residual Angina at One Year.

BACKGROUND: The pullback pressure gradient (PPG) is a novel physiological metric that quantifies coronary artery disease patterns as focal or diffuse on a scale from 0 to 1. This study assessed the relationship between PPG and residual angina at 1 year. METHODS: PPG Global is a prospective, investigator-initiated, single-arm, multicenter study that enrolled patients with at least 1 lesion with a fractional flow reserve ≤0.80 intended to be treated with percutaneous coronary intervention. After the PPG calculation, physicians could revise treatment assignment to medical therapy or coronary art…

Magnitude of Portal Pressure Reduction and Clinical Outcomes in Randomized Trials.

BACKGROUND & AIMS: Portal hypertension is a major driver of complications of cirrhosis. Observational studies suggest that reductions in hepatic venous pressure gradient (HVPG) improve prognosis, but the magnitude of risk reduction per unit HVPG decrease has not been quantified across randomized controlled trials (RCTs). We performed a meta-regression of RCTs to quantify this relationship. METHODS: We systematically searched for RCTs testing pharmacological interventions against placebo or no treatment in portal hypertension that reported both HVPG measurements and clinical outcomes. The pred…

Imaging-based method to quantify left ventricular diastolic pressures.

AIMS: To establish an imaging-based method to quantify left ventricular (LV) diastolic pressures. METHODS AND RESULTS: In 115 patients suspected of coronary artery disease, LV pressure was measured by micromanometers and images by echocardiography. LV filling pressure was measured as LV pre-atrial contraction pressure (pre-A PLV). Based on previous observations, we hypothesized that pre-A PLV approximates the sum of minimum PLV and maximum transmitral pressure difference. Parameters used for pressure estimates included LV volumes and strain, left atrial strain, mitral flow velocities, systoli…

[Shorter stays, greater pressure? Insights into the acceleration of inpatient care].

In a context of economic pressure on hospitals, reducing the average length of stay has become essential in internal medicine. 37,218 hospitalizations at the Fribourg Hospital (2019-2024) were analyzed retrospectively using three exploratory non-validated indicators: daytime blood sampling, CT-scans performed within six hours, and admissions after 4:00 pm. All showed a significant increase, reflecting an acceleration of the clinical decision-making and care process. This evolution raises questions about organizational consequences, workload, and the meaning of the medical profession.
