The Trauma Recovery and Empowerment Model (TREM): conceptual and practical issues in a group intervention for women.

This article describes the Trauma Recovery and Empowerment Model (TREM), a manualized group intervention designed for women trauma survivors with severe mental disorders, and discusses key issues in its conceptualization and implementation. TREM recognizes the complexity of long-term adaptation to trauma and addresses a range of difficulties common among survivors of sexual and physical abuse. Focusing primarily on the development of specific recovery skills and current functioning, TREM utilizes techniques shown to be effective in trauma recovery services. The group's content and structure ar

A case of pathological gambling--its features and mangement.

Pathological gambling (PG) represents the end spectrum of gambling behaviours. It is characterised by constantly recurring gambling behaviour which is maladaptive. This case illustrates how an Asian woman pathological gambler progresses through the different stages of development of the gambling habit, and how a multi-modal treatment approach has helped her regain psychosocial function. In addition, the maintenance factors, typical and atypical associated features, and difficulties in the management of such a case are highlighted.

Body image in eating disorders and analysis of its relevance: a preliminary study.

The purpose of the present study was to assess body perception in eating disorders and its meaning in therapy. A total of 44 patients, 25 bulimia nervosa and 19 anorexia nervosa, admitted at a specialized in-patient treatment center participated in the present study. Body size estimation procedures (video-distortion and image-marking techniques) and self-report questionnaires were used for the assessment. No significant differences were found in actual body size estimation between the two eating disorder groups, either before or after therapy. It emerged after therapy that the outcome was not

Is the reexperience of painful emotion therapeutic?

Recently, a great deal of attention has been focused on the health promoting benefits that can accrue from revisiting painful emotion. The rationales for revisiting painful emotions include those that assume reexperiencing emotion per se can be health-promoting. Another view stipulates that revisiting painful emotion will only yield benefit if there is some recasting/restructuring of the emotional memory. Research pertinent to the various rationales is discussed. Then research on the impact of emotional expression and outcomes studies of therapies designed to enhance emotional experience are r

Sampling of empirically supported psychological treatments from health psychology: smoking, chronic pain, cancer, and bulimia nervosa.

Interventions in health psychology and behavioral medicine represent an integral area of research for the development of psychological therapies to enhance health behaviors, manage symptoms and sequelae of disease, treat psychological symptoms and disorders, prolong survival in the face of a life-threatening illness, and improve quality of life. A sampling of interventions in health psychology and behavioral medicine is offered that meet the criteria for empirically supported treatments for smoking cessation, chronic pain, cancer, and bulimia nervosa. Guided imagery treatment to promote self-soothing in bulimia nervosa. A theoretical rationale.

Bulimia nervosa (BN) has been described as involving impairment in affect regulation and in self-soothing. Such a conceptualization suggests the need to design treatments that specifically target these problems in order to assist individuals with BN in comforting themselves. A model of guided imagery therapy suggests that imagery therapy has multiple levels of action and can assist these individuals in the regulation of affect by providing an external source of soothing and also by enhancing self-soothing. The authors illustrate the model with a case example and report the results of a study i

A review of post-traumatic stress disorder. Part II: Treatment.

This paper describes the history of the development of understanding of psychological responses to traumatic life-events and their treatment. One major response, post-traumatic stress disorder (PTSD), is a recognized condition which has appeared relatively recently in diagnostic manuals. PTSD is a condition of major significance, not only to mental health professionals, but also to trauma surgeons and allied professionals. This paper focuses on the current definition of PTSD in the International Classification of Diseases (ICD-10, 1992, World Health Organization) and the Diagnostic and Statist

Jenny's story: reinventing oneself through occupation and narrative configuration.

Two life history interviews were conducted to discover how one women, Jenny, experienced a traumatic head injury, rehabilitation, and recovery. Narrative analysis of the transcribed interviews revealed a rich story of how Jenny had fashioned her identity and character through childhood occupations, including studying classical literature and music, and of how she drew upon resources developed in childhood to engineer her recovery. It also illustrated how Jenny used a recursive process of narrative construction and engagement in self-devised graduated occupations, including studying, playing mu

The neural correlates of inner speech and auditory verbal imagery in schizophrenia: relationship to auditory verbal hallucinations.

BACKGROUND: Auditory verbal hallucinations are thought to arise from the disordered monitoring of inner speech (thinking in words). We tested the hypothesis that a predisposition to verbal auditory hallucinations would be associated with an abnormal pattern of brain activation during tasks which involved the generation and monitoring of inner speech. METHOD: The neural correlates of tasks which engaged inner speech and auditory verbal imagery were examined using positron emission tomography in (a) schizophrenic patients with a strong predisposition to auditory verbal hallucinations (hallucina

Treatment of sleep disturbances in depressed patients.

Sleep is disturbed in 90% of patients with major depression. Disordered sleep physiology may persist after clinical remission of depression, suggesting either that sleep disruption is a trait characteristic of recurrent depression or that depressed patients acquire new habits that perpetuate sleep-related problems. This article reviews the data suggesting a common pathophysiology between sleep and depression. It then focuses on a strategy for evaluating and treating sleep disruption in depressed patients. Treatment must have a conservative goal of restoring sleep quality to the pre-episode lev

International study of expert judgment on therapeutic use of benzodiazepines and other psychotherapeutic medications: III. Clinical features affecting experts' therapeutic recommendations in anxiety disorders.

Our objective was to assemble expert clinical experience and judgment in the treatment of anxiety and related disorders in a systematic, quantitative manner. Psychiatric aspects of liver transplantation.

Psychiatry plays an essential role in the selection and preoperative care of liver transplant patients. Unique to liver transplantation are the preponderance of candidates with alcoholic end-stage organ failure and also the reversibility of encephalopathy with postoperative normalization of hepatic function. The expense of liver transplantation necessitates documentation of quality of life among recipients. It can also be postulated that a "ripple effect" does occur, in which favorable operative outcomes provide a meaningful and beneficial effect on social networks of transplant candidates and

Dimensions of multiple personality disorder.

Research on multiple personality disorder (MPD) has burgeoned, and large-scale investigations indicate that a typical MPD patient is a woman, a victim of childhood abuse (especially sexual abuse), a person whose symptoms meet criteria for other psychiatric disorders, and a person who would employ many psychological defenses. Treatment approaches have frequently included hypnotherapy, which requires skill and caution.
