Ulcerative colitis and colorectal cancer. A population-based study.

BACKGROUND: The risk of colorectal cancer is increased among patients with ulcerative colitis. The magnitude of this increase in risk and the effects of the length of follow-up, the extent of disease at diagnosis, and age at diagnosis vary substantially in different studies. METHODS: To provide accurate estimates of the risk of colorectal cancer among patients with ulcerative colitis, we studied a population-based cohort of 3117 patients given a diagnosis of ulcerative colitis from 1922 through 1983 who were followed up through 1984. RESULTS: Ninety-two cases of colorectal cancer occurred in

Major colorectal cancer aetiological hypotheses do not explain mortality trends among Maori and non-Maori New Zealanders.

New Zealand colorectal cancer mortality rates are presented for the period 1947-1980. Mortality has been increasing and is now the highest in the world for both males and females in the age range 35-64; indeed New Zealand mortality rates for those aged 35-44 are approximately twice those of other countries with high mortality. By contrast, colorectal cancer mortality rates among Maoris, the indigenous New Zealanders of Polynesian descent, have been decreasing so that they are now less than half the non-Maori mortality rates. These findings cannot be explained by ethnic differences in consumpti

Polyps and diverticulosis of large bowel in autopsy population of Akita prefecture, compared with Miyagi. High risk for colorectal cancer in Japan.

A necropsy study of polypoid lesions and diverticula of the large bowel in the Japanese prefectures of Akita and Miyagi indicated that adenomatous polyps were more numerous in Akita (30%) than Miyagi (18.3%). They also were larger and showed more severe atypia in Akita. The sigmoid colon and rectum showed the most severe atypia in both prefectures. Hyperplastic polyps and diverticula were very uncommon in both prefectures. These results are discussed in relation to the different levels of risk to colorectal carcinoma in these prefectures and in Japanese migrants to Hawaii.

Diagnostic Performance of CT, MRI, and ERUS in Preoperative T- and N-Staging of Colorectal Cancer: A Retrospective Single-Center Analysis.

PURPOSE: This study evaluates the accuracy of computed tomography (CT), magnetic resonance imaging (MRI), and endorectal ultrasound (ERUS) in preoperative staging of colon and rectal cancers, comparing imaging results with histopathological findings. The aim is to identify strengths and limitations of each modality to optimize therapeutic strategies, particularly for neoadjuvant therapy in locally advanced cases. METHODS: This retrospective study analyzed 487 patients treated surgically for colon and rectal cancers between 2012 and 2018. Patients with incomplete imaging or histopathological d

Effect of prolonged interval between neoadjuvant chemoradiotherapy and surgery on pathological and oncological outcomes in locally advanced rectal cancer: a randomized controlled trial.

BACKGROUND: The optimal interval between neoadjuvant chemoradiotherapy (nCRT) and total mesorectal excision (TME) in locally advanced rectal cancer (LARC) remains controversial. This randomized controlled trial evaluated whether prolonging the interval between nCRT and surgery improves pathological response and oncological outcomes. METHODS: Adult patients with histologically confirmed, MRI-staged II-III rectal adenocarcinoma located within 12 cm of the anal verge were randomized to undergo surgery at 8 weeks or 12 weeks after completion of long-course nCRT (50 Gy with concurrent 5-fluorourac

Stoma Rate and Oncological Outcomes of Primary TaTME vs Completion TaTME in Patients With Early-Stage Rectal Cancer.

BACKGROUND: Local excision as a first step in the treatment of early rectal cancer has gained interest. However, in the presence of histopathological risk factors, (inter)national guidelines recommend completion total mesorectal excision. Although oncologically safe, completion total mesorectal excision is associated with an increased end colostomy rate compared to primary total mesorectal excision, especially in distal lesions. Transanal total mesorectal excision may facilitate lower anastomoses, potentially reducing end colostomy rates. OBJECTIVE: To compare the end colostomy rate and oncol

Neoadjuvant Chemotherapy Versus Chemoradiotherapy for Locally Advanced Rectal Cancer: A Multicenter Propensity Score-Matched Study.

BACKGROUND: Risk-adapted treatment strategies have been recommended for patients with rectal cancer. In our previous phase II study, neoadjuvant chemotherapy with capecitabine and oxaliplatin resulted in substantial tumor responses and encouraging survival outcomes for patients with locally advanced rectal cancer. OBJECTIVE: The aim of this study was to compare the survival outcomes of neoadjuvant chemotherapy and neoadjuvant chemoradiotherapy in patients with locally advanced rectal cancer. DESIGN: Retrospective cohort analysis. SETTING: This study was conducted at 2 high-volume hospitals

Clinical nodal staging to assess the risk of local recurrence after rectal cancer resection: A nationwide cross-sectional study.

AIM: In rectal cancer, neo-adjuvant (chemo)radiotherapy (n(C)RT) should be used selectively, due to related toxicity. The decision to offer n(C)RT is mainly based on preoperative MRI staging. cN category has limited accuracy and its use when deciding on n(C)RT is controversial. This population-based study aimed to assess the association between cN category and local recurrence (LR) rate. METHOD: Data from a national cross-sectional cohort of patients who underwent curative resection of primary rectal cancer in the Netherlands in 2016 were used. Patients were subdivided by neo-adjuvant treatme

Minimally invasive approaches for early-stage rectal cancer: A comparative analysis from the TriNetX network.

BACKGROUND: Minimally invasive approaches to manage early-stage rectal cancer have become popular, offering effective treatment with reduced morbidity. Among them, transanal endoluminal surgery (TES) and endoscopic submucosal dissection (ESD) are widely utilized, yet their comparative outcomes from large-scale real-world data remain limited. METHODS: We conducted a retrospective cohort study using the TriNetX United States Collaborative Network. Adults with T1 rectal cancer undergoing either TES or ESD were identified. Propensity score matching was performed on demographic and clinical variab
