[The effect of their initial functional status on the radiation reaction of the neurons in the rat spinal cord].

The influence of CNS functional state and structural changes of neurons in spinal cord following local exposure to 38 or 76 Gy X-radiation. Morphological analysis show, that stimulation of peripheral nerves increase, but hypoxia or barbiturates decrease destruction of spinal neurons by radiation. Value destruction also depends of neurons volumes.

Genesis of spontaneous rhythmic motor patterns in the lumbosacral spinal cord of neonate mouse.

The isolated spinal cord of the neonatal mouse spontaneously generates two different motor patterns of continuous rhythmic bursting: one in lumbar ventral roots in earliest postnatal preparations (P0-2) and another at the sacral level at later postnatal times (P3-5). Lumbar rhythmic motor discharges clearly alternate on contralateral roots and are in a frequency range (approximately 1 Hz) usually described for locomotor-like activity, while sacral motor sequences include mixed synchrony and irregular bilateral alternation that differ from typical locomotor-like activity. A significant decrease…

A surgical approach for concomitant spinal cord and brachial plexus surgery: an anatomical study.

Spinal cord surgery is not the current treatment for brachial plexus avulsion injuries. However, several experimental and a few clinical cases have been reported with promising results. This surgical strategy in the near future, might prove to be useful. Different simultaneous anatomical approaches to the brachial plexus and spinal cord were studied in attempt to discover the best route to be used in the surgical reconstruction of avulsion lesions of the brachial plexus by spinal cord surgery. Eleven fresh subjects were used to compare: a) simultaneous dorsal approaches to the brachial plexus …

A study comparing sterile and nonsterile urethral catheterization in patients with spinal cord injury.

This study was designed to determine the effect of sterile and nonsterile intermittent catheterization on the incidence of urinary tract infection (UTI) in patients after spinal cord injury. The study included 29 patients with neurogenic bladder dysfunction treated with intermittent catheterization. One group of 14 patients was on sterile catheterization; another group of 15 patients was on nonsterile catheterization. On a weekly basis, urine samples were obtained and analyzed. A total of 122 urine samples were analyzed. The patients on sterile catheterization had a 28.6% UTI incidence; the gr…

Diversion colitis: a cause of abdominal discomfort in spinal cord injury patients with colostomy.

Diversion colitis is thought to result from nutritional deficiencies secondary to fecal diversion. Symptoms include hemorrhagic purulent rectal discharge, abdominal pain, and tenesmus. 5-Aminosalicylic acid (5-ASA) and N-butyrate enemas have been reported to help this condition non-spinal cord injury (SCI) patients. We report the case of a 49-year-old C6 ASIA B tetraplegic man who had received colostomy because of intractable ileus 10 years earlier. He presented with a 2-week history of rectal pain and bleeding. Abdominal and rectal examination on admission were unremarkable. Colonoscopy showe…

Regional distribution of the locomotor pattern-generating network in the neonatal rat spinal cord.

The regional distribution of spinal cord networks producing locomotor-like, as well as non-locomotor-like, activity was studied with the use of an in vitro neonatal rat preparation. Rhythmic activity was induced by bath application of either serotonin (5-HT), acetylcholine (ACh), N-methyl-D,L-aspartate (NMA), or combined 5-HT/NMA, and was monitored via hindlimb flexor (peroneal) and extensor (tibial) electroneurograms (ENGs) or ventral root recordings. In some experiments, synchronous patterns were produced by the addition of inhibitory amino acid (IAA) receptor antagonists. Selective applicat…

Effects of 4-aminopyridine on motor evoked potentials in patients with spinal cord injury.

The potassium (K+) channel-blocking agent 4-aminopyridine (4-AP) is currently being investigated for its potential therapeutic value in patients with spinal cord injury (SCI). The present study was designed to test the hypothesis that 4-AP ameliorates central motor conduction deficits in individuals with SCI. Oral 4-AP (10 mg) was administered to 19 (n = 19) SCI subjects with stable neurological deficits. Their response to the drug was monitored using motor evoked potentials (MEPs) following transcranial magnetic stimulation of motor cortex and various measures of segmental or peripheral refle…

Prevention of urinary tract infection in patients with spinal cord injury--a microbiological review.

The importance of urinary tract infection (UTI) in patients with spinal cord injury cannot be understated. Many patients with significant bacteriuria are considered to be colonised rather than infected, and treatment should be reserved for those with clinical symptoms or other signs of infection. Published research on the prevention and management of UTI in patients with spinal cord injury often has limitations due to differences in definitions of UTI, studies on groups using different urinary drainage appliances, the mixture of newly injured and longstanding injured patients and studies being…

A comparison of two methods of sterile urethral catheterisation in spinal cord injured adults.

The purpose of this study was to compare the incidence of urinary infection following sterile catheterisation after either (1) a handwash of 30 s and double gloving, (the shorter sterile technique) or (2) after a 3 minute wash from fingertips to elbows and the wearing of a sterile gown and one pair of gloves, (the longer sterile technique). Forty-six patients were randomly assigned to the two groups. No significant difference in urinary infection rates was found and as a result the more complicated technique has been discontinued.

The role of renal scintigraphy in the evaluation of spinal cord injury patients with presumed urosepsis.

PURPOSE: We attempted to differentiate pyelonephritis, defined as upper urinary tract parenchymal infection, from fever due to other sources in patients with spinal cord injury by radioisotope renal scintigraphy. MATERIALS AND METHODS: A total of 13 consecutive spinal cord injury patients 21 to 54 years old (level of injury C4 to cauda equina) was hospitalized with urosepsis. The diagnosis was based on medical history, physical examination and laboratory evaluation, including blood, urine, sputum and wound fluid culture and sensitivity, and a 99mtechnetium glucoheptonate renal scan for pyelon…

Spinal cord compression due to extramedullary hematopoiesis in the proliferative phase of polycythemia vera.

Extramedullary hematopoiesis is a common accompaniment of a variety of hematologic diseases such as hereditary spherocytosis, thalassemia and myelofibrosis. The association of extramedullary hematopoiesis with polycythemia vera in the proliferative phase is much less usual. We report a patient who presented with paraplegia due to spinal cord compression; clinical investigation revealed a paravertebral hematopoietic tumor, and the diagnosis of polycythemia vera was then established.

Value of magnetic resonance imaging of the lumbosacral spinal cord in patients with voiding dysfunction.

PURPOSE: We evaluated the role of magnetic resonance imaging (MRI) of the lumbosacral spinal cord in patients with voiding complaints and abnormal urodynamic findings but normal neurological and lower spine examinations. MATERIALS AND METHODS: We studied 17 women and 13 men 17 to 50 years old (mean age 32.2) who presented with voiding complaints. All patients had a completely normal neurological examination and no evidence of prostatic enlargement. Of the 30 patients 18 had incontinence (17 urge, and 1 stress and urge), 9 frequency and urgency alone, and 3 urinary retention. Seven patients ha…

Pathology of spinal cord injuries due to fracture-dislocations of the thoracic and lumbar spine.

The pathology of intervertebral disc injuries, hyper-extension injuries and spinal cord injuries due to fracture-dislocations of the cervical spine has been reported previously. Between 1957 and 1987, autopsy was performed on 4 patients with fracture-dislocation of the thoracic spine and on one patient with that of the lumbar spine. This paper presents the results of a clinicopathologic study conducted on two patients with a fracture-dislocation of the thoracic spine and on one patient with that pathology of the lumbar spine. The methods employed in the present study are the same as those that…

Neurotransmitters in the spinal cord dorsal horn in a model of painful neuropathy and in nerve crush.

We tested the hypothesis that neurochemical changes in the spinal cord dorsal horn associated with neuropathic pain states differ from those seen in association with non-painful neuropathies. Immunohistochemistry was performed on spinal cord sections from rats with a chronic constriction injury (CCI), which develop hyperalgesia, and from animals with a nerve crush injury, which do not develop hyperalgesia or other signs of a painful syndrome. Immunohistochemistry was quantified by computer-assisted densitometry. Calcitonin gene-related peptide (CGRP) immunoreactivity and substance P (SP) immun…

Synaptic excitation in the thoracic spinal cord from expiratory bulbospinal neurones in the cat.

1. Synaptic actions in the thoracic spinal cord of individual expiratory bulbospinal neurones were studied in anaesthetized cats by the use of two techniques: (i) the monosynaptic connections to motoneurones were assessed by cross-correlations between the discharges of the neurones and efferent discharges in the internal intercostal nerves of several segments bilaterally; and (ii) distributions of terminal and focal synaptic potentials were measured by extracellular spike-triggered averaging in the thoracic ventral horn. 2. Monosynaptic connections were identified by both the durations and tim…

[Interactions between opioids and local anesthetics at the level of the dorsal horn of the spinal cord].

For many years, local anaesthetic drugs, given spinally, have been used to control pain after surgery or during parturition. For nearly 20 years, we have known that opioids given by the same way have an analgesic effect. However the later group do not provide satisfactory analgesia in several pain contexts but also give troublesome side effects; local anaesthetics are associated also with side effects. Experimental and clinical studies have shown that the combination of local anaesthetics with opioids provides some degree of synergism, associated with a reduction of side effects. Further studi

Effects of nicorandil on the recovery of reflex potentials after spinal cord ischaemia in cats.

1. The pathophysiological significance of ATP-sensitive K+ (KATP) channels in the central nervous system is not fully understood. In this study the effects of nicorandil (a hybrid vasodilator having a dual mechanism of action as a K+ channel opener and a nitrate) on the recovery of the spinal cord reflex potentials after spinal cord ischaemia were examined and compared with those of pinacidil and nitroprusside in anaesthetized spinal cats. 2. Spinal cord ischaemia was produced by occlusion of the thoracic aorta and the bilateral internal mammary arteries for 10 min. Regional blood flow in the …
