What is the most appropriate method for the measurement of the range of motion in the lumbar spine and how does surgical fixation affect the range of movement of the lumbar spine in adolescent idiopathic scoliosis? A systematic review protocol.

INTRODUCTION: Adolescent idiopathic scoliosis (AIS) is a three-dimensional rotational change in the normal shape of the spine which affects children aged 10 to 18 years. Both the condition and its management can have significant impact on functional ability. Currently, expected restriction in spinal motion is experience based, rather than evidence based, and discussions to inform patient expectations pre-operatively can be difficult. The aim of this review is to evaluate the evidence pertaining to measurement of spinal motion and whether this is altered following surgery, dependent on the anat…

Cervical magnetic resonance imaging abnormalities not predictive of cervical spine instability in traumatically injured patients. Invited submission from the Joint Section Meeting on Disorders of the Spine and Peripheral Nerves, March 2004.

OBJECT: Identifying instability of the cervical spine can be difficult in traumatically injured patients. The goal of this study was to determine whether cervical abnormalities demonstrated on magnetic resonance (MR) imaging are predictive of spinal instability. METHODS: Data in all patients admitted through the Level I trauma service at the authors' institution who had undergone cervical MR imaging were retrospectively reviewed. The reasons for MR imaging screening were neurological deficit, fracture, neck pain, and indeterminate clinical examination (for example, coma). Abnormal soft-tissue…

Sustained increases in lumbar spine and hip BMD with denosumab therapy beyond 10 years.

INTRODUCTION: Denosumab therapy results in a sustained increase in BMD at the lumbar spine and hip and lower fracture rates during 10 years of treatment. However, data on efficacy and safety beyond 10 years are limited. We present the case of a female who demonstrated a large and continued rise in BMD after 10 years of denosumab therapy. CASE: An 80-year-old woman was referred to our Bone Health Clinic for advice regarding osteoporosis diagnosed 1 year earlier on routine DXA: lumbar spine T-score of -4.2 (0.585 g/cm2) and total hip T-score of -4.0 (0.503 g/cm2). Her GP had commenced her on al…

Linking functional and structural dendritic spine remodeling during fear learning and extinction in vivo.

Structural plasticity of dendritic spines has been observed during different learning paradigms, but how the functional dynamics of dendritic spines changes with memory processing and how these patterns relate to structural plasticity and dendritic integration remain unclear. Here, we perform longitudinal functional and structural in vivo imaging of the frontal association cortex in mice subject to fear conditioning and extinction over several days. We show that fear learning induced responsive spines that are more likely to be synchronous and clustered, which are consolidated over the followi…

Comparative Cost Analysis: Uniportal Endoscopy, Biportal Endoscopy, Microtubular, and Open Spine Surgery [Retrospective Review].

STUDY DESIGN: A retrospective review. OBJECTIVE: To perform a comparative cost analysis of unilateral biportal endoscopic surgery (UBE), uniportal endoscopic surgery, microtubular surgery, and open surgery in outpatient and inpatient settings. SUMMARY OF BACKGROUND DATA: Endoscopic spine surgery techniques have gained traction for their benefits. However, the relative costs of biportal and uniportal endoscopic approaches compared with traditional methods, such as microtubular and open spine surgery, remains a debated topic. An evaluation of cost implications is critical for health care provi…

Stop guessing: A clinician's guide to demystifying spine-related arm pain in primary care.

Pain in the arm could arise from a wide range of conditions, from non-specific musculoskeletal disorders to serious spinal pathology. Spine-related arm pain (SAP), i.e., pain that is best explained by processes in the spine or spinal nerves, poses a particular diagnostic challenge in primary care due to its diverse overlapping symptoms and evolving presentations. This paper proposes a structured, literature-informed, consensus-based guide to assist clinicians in classifying SAP according to predominant pain mechanisms and stratifying the level of clinical concern. The guide takes the form of a…

The Utility of Routine Postoperative Radiographs Following Surgical Treatment of Traumatic Cervical Spine Injuries.

Background/Objectives: Postoperative cervical spine radiographs are routinely obtained during in-hospital and follow-up period. We aim to evaluate the utility of postoperative radiographs for identifying instrumentation failure and the subsequent need for revision surgery in patients with traumatic cervical spine injuries. Materials and Methods: A retrospective chart review of patients who had surgical treatment for traumatic cervical spine injury was conducted. Clinical notes and radiographic reports were evaluated. Postoperative radiographs were obtained prior to discharge from the hospital,…
