Drooling in children with neurological dysfunction indicates neurogenic failure to coordinate the muscles of the tongue, soft palate, and face which act in the first stage of swallowing. Feeding abilities in 20 individuals with Rett syndrome aged 1 1/2 to 33 years were investigated by history and clinical assessment during a meal, followed by videofluoroscopy of feeding. All were shown to have reduced movements of the mid and posterior tongue, with premature spillover of food and liquid from the mouth into the pharynx. They also showed delayed pharyngeal swallow, but otherwise pharyngeal problems were minimal. These findings were noted to be similar to those in Parkinson's disease. We report a case of achalasia presenting as acute stridor and respiratory distress in an 87-year-old woman. A mega-oesophagus was decompressed by aspiration through a naso-oesophageal tube, stiffened with paediatric endoscopic biopsy forceps before placement. Subsequent barium swallow showed mega-oesophagus secondary to achalasia causing tracheal compression at the level of the thoracic inlet. Gastric ectopias in the upper oesophagus and hypopharynx are relatively rare and often misinterpreted. They may become symptomatic due to the onset of a fistula involving adjacent structures in the neck. Drooling occurs when excessive quantities of saliva dribble from the opened mouth. This distressing condition affects many mentally handicapped patients who are unable to effectively clear their normal salivary flow by swallowing. This is the first description of a fistula arising from both the fourth branchial cleft and the fourth pharyngeal pouch in a 62-year-old woman. This developmental anomaly ran from the piriform sinus to the skin of the left side of the neck. Fifty-one adult patients with complaints suggesting peptic oesophagitis were submitted within the same week to a radionuclide swallow test (OT), an endoscopy and a manometry in order to evaluate the incidence of oesophageal peristaltic abnormalities in patients with only minor peptic oesophagitis. In all patients with normal endoscopy, OT did not show any transit abnormality. In the patients with slight (Stage I) or moderate (Stage II) oesophagitis, OT was abnormal in 60% of the cases. One hundred and twenty five adults who were born before 1969 with oesophageal atresia or tracheo-oesophageal fistula or both and were managed at the Royal Children's Hospital, Melbourne, were reviewed. A study of the Eustachian tube, incorporating nasendoscopy, tympanometry and pressure swallow test was undertaken in patients undergoing uvulopalatopharyngoplasty. Marked negative middle ear pressure indicating impairment of tubal function was found during the first 7 postoperative days. Spontaneous pneumomediastinum is an uncommon disease that often presents with subtle clinical and radiographic findings. During a two-week period, three patients presented with complaints of throat and neck pain. The first complained of difficulty swallowing and throat and chest discomfort; accurate diagnosis was delayed because of the unusual presenting symptoms. The second had severe throat pain.
