Inferior vena cava filter insertion and retrieval patterns in a tertiary referral centre in Ireland.

BACKGROUND: Inferior vena cava (IVC) filters are widely used in clinical practice to prevent large, clinically significant pulmonary emboli. Modern filters are designed to be retrievable within a specific time window; however, many become lost to follow-up. AIM: To examine the insertion and retrieval patterns of IVC filters in a tertiary referral teaching hospital in Ireland. METHODS: A retrospective review of all IVC filter insertions and retrievals was performed in a tertiary referral university teaching hospital which incorporates the national referral centre for pelvic and acetabular tra

Sustained change in didactic skills--does teacher training last?

Teacher training programmes are necessary assets in faculty development. Few data exist on their long-term effects on participants' teaching skills. Our aim was to study participants' didactic competencies up to four years after attending a newly established faculty development workshop at Hamburg Medical School. Of the 322 participants who attended our teacher training between 2006 and 2009, 313 received a self-assessment and evaluation questionnaire in 2010. This follow-up self-assessment (t2) was compared with their self-assessment of the same didactic competencies before (t0) and directly

Complications of midline-open tracheotomy in adults.

OBJECTIVES/HYPOTHESIS: Percutaneous tracheotomy is progressively replacing open tracheotomy as a consequence of promising results of comparative studies. However, this comparison has four considerable weaknesses: 1) selected indications (high-risk patients excluded for percutaneous tracheotomy); 2) varying spectra of complications included in different studies; 3) varying operative settings (experienced surgeons exclusively, surgeons in training, or both); and 4) missing differentiation between different surgical techniques. Our study was performed to collect complete datasets of unselected pa

Trends in surgical management of femoral neck fractures in the United States.

UNLABELLED: We examined trends in utilization of open reduction and internal fixation (ORIF), THA, and hemiarthroplasty (HA) for femoral neck fractures. Closed femoral neck fractures managed with ORIF or hip arthroplasty (n = 162,257) were extracted from 1990 to 2001 Nationwide Inpatient Samples. Trends were examined during three periods (1990-1993 [Period I], 1994-1997 [Period II], and 1998-2001 [Period III]). Utilization of HA increased from 67.8% in Period I to 75.3% in Period III. In the same period, utilization of THA decreased from 11.6% to 6.6%. The trend of decreased use of THA was con

Are schools a good setting for adolescent sexual health promotion in rural Africa? A qualitative assessment from Tanzania.

African adolescents are at high risk of poor sexual health. School-based interventions could reach many adolescents in a sustainable and replicable way, if enrolment, funding and infrastructure are adequate. This study examined pupils', recent school leavers', parents' and teachers' views and experiences of rural Tanzanian primary schools, focusing on the implications for potential sexual health programmes. From 1999 to 2002, participant observation was conducted in nine villages for 158 person-weeks. Half of Year 7 pupils were 15-17 years old, and few went on to secondary school, suggesting t

Patterns of care and survival for adolescents and young adults with acute leukaemia--a population-based study.

We report a population-based study of patterns of care and survival for people with acute leukaemia diagnosed at age 15-29 years during 1984-94 in regions of England and Wales covered by specialist leukaemia registries. There were 879 patients: 417 with acute lymphoblastic leukaemia (ALL) and 462 with acute myeloid leukaemia (AML). For ALL, actuarial survival rates were 43% at 5 years after diagnosis and 37% at 10 years. Survival improved significantly between 1984-88 and 1989-94 for those aged 15-19 at diagnosis. Patients entered in national clinical trials and those not entered had similar s

Acute bacterial meningitis: where do children die?

The case-fatality rate for acute bacterial meningitis in childhood is still disappointingly high. A review of 687 cases of acute bacterial meningitis including 72 deaths, identified in a defined population of children under ten years of age, was done to determine where patients were treated and where they died. Ten per cent of all hospital admissions were to infectious-disease hospitals, 16 per cent to teaching hospitals, and 74 per cent to general, non-teaching ("regional-board") hospitals. The case-fatality rates associated with these hospitals were three per cent, three per cent and 11 per

Intractable hiccups: How can chest X-ray help?

We describe the case of a young male presenting with hiccups refractory to antacids. Due to high index of suspicion was evaluated with a chest X-ray which revealed pericardial effusion. The effusion was drained, and the hiccups resolved. It is a teaching moment, to consider simple imaging, i.e., chest X-ray in patients with refractory hiccups to rule out more serious underlying causes.

Bringing Health Professions Education to Patients on the Streets.

This article considers strategies for illuminating health systems' structural violence toward people experiencing homelessness and for resisting incursion of moral injury to health professional learners. This article also canvasses the nature and scope of educators' obligations to teach in patient-focused ways that motivate equity and students' capacity to serve some of the country's most vulnerable residents in clinical settings or on the streets.

A case of successful embolotherapy for gastric ulcer bleeding from the intercostal artery after oesophagectomy and gastric reconstruction.

We report a successful treatment with coil embolisation of an intercostal artery for ulcer bleeding in a gastric tube in a 70-year-old man who underwent a total oesophagectomy and gastric tube reconstruction for oesophageal cancer. This case teaches us to search aberrant feeding vessels when active bleeding is suspected in reconstructed gastric tube in the patient with oesophagectomy and oesophagogastrostomy.

Care of the injured Jehovah's Witness patient: case report and review of the literature.

Care for the Jehovah's Witness patient can be a challenge and often a dilemma to clinicians because of the patient's religious beliefs and teachings against receiving blood and blood products, especially in emergency or trauma settings. We present a case of a severely injured elderly Jehovah's Witness patient who survived. We also review the literature and offer an organized approach to care for such patients.

Understanding teachers' technostress creators due to IT use at work: a mixed-methods study.

With the digitization of education, the use of information technology (IT) at work has become a requirement for teachers, rather than an option. However, IT is a double-edged sword that brings teachers great benefits and additional challenges (e.g., technostress). The present study employed a mixed-methods design to understand context-specific technostress creators and their associations with cognitive appraisal outcomes and IT use behaviors among teachers. Phase 1 identified six technostress creators-unreliability (UNR), role conflict, teaching interruption (TI), work overload (WO), informati

Measuring student perceptions of motivation and engagement with Kahoot!: a case study in primary education in Portugal.

The integration of digital tools in teaching remains a challenge for many educators, often due to resistance to change and limited training. Drawing on Self-Determination Theory, as proposed by Deci and Ryan, and Cognitive Load Theory, developed by Sweller, this exploratory study examines student motivation and engagement in a gamified learning environment. Specifically, it explores students' perceptions of the use of Kahoot! in the classroom and the extent to which the tool may add value to the teaching and learning process. The study involved 20 fourth-grade students and their class teacher

Artificial intelligence and teacher competence: a scoping review of assessment, analytics, and professional development.

This scoping review maps how artificial intelligence (AI) is being connected to teacher competence in recent research. The review was based on 33 peer-reviewed articles published in 2022-2026 and identified through a bounded Web of Science search. Its purpose was not to evaluate intervention effectiveness, but to describe the extent, range, and nature of the available evidence on AI, machine learning (ML), and learning analytics (LA) in teacher assessment, modeling, and professional development within this indexed corpus. The mapped literature suggests two broad lines of work. One uses AI, ML,
