Clinical Outcomes and Complications Following Limited Open Achilles Repair Without an Instrumented Guide.

BACKGROUND: We report the clinical outcomes and complications following our limited open incision Achilles tendon repair technique without instrument guides. METHODS: A total of 33 patients were included in this study. We recorded pre- and postoperative scores on the Foot and Ankle Disability Index (FADI), visual analog scale (VAS), and the Foot and Ankle Outcome Score (FAOS). Subgroup analyses were performed for acute (<2 weeks) and subacute (2-6 weeks) Achilles tendon repairs. A P value <.05 was considered significant for all statistical analyses. RESULTS: The median time from injury to su

Pyruvate and lactate as local prognostic biomarkers of patient outcome after achilles tendon rupture.

BACKGROUND: Acute Achilles tendon rupture (ATR) is a frequently disabling injury, which exhibits unclear variability in long-term functional and patient-reported outcomes. Biomarkers from early healing, which have been shown to be prognostic of long-term outcome would facilitate the development of improved treatment methods. HYPOTHESIS/PURPOSE: The aim of this study was to assess essential metabolites pyruvate and its product lactate, as early biomarkers in relation to long-term functional- and patient-reported outcome after ATR. STUDY DESIGN: Prospective cohort study. METHODS: A total of 1

Negative effect of zoledronic acid on tendon-to-bone healing.

Background and purpose - Outcome after ligament reconstruction or tendon repair depends on secure tendon-to-bone healing. Increased osteoclastic activity resulting in local bone loss may contribute to delayed healing of the tendon-bone interface. The objective of this study was to evaluate the effect of the bisphosphonate zoledronic acid (ZA) on tendon-to-bone healing. Methods - Wistar rats (n = 92) had their right Achilles tendon cut proximally, pulled through a bone tunnel in the distal tibia and sutured anteriorly. After 1 week animals were randomized to receive a single dose of ZA (0.1 mg/

Negative pressure wound therapy in the management of late deep infections after open reconstruction of achilles tendon rupture.

Infection is a major complication after open reconstruction of Achilles tendon ruptures. We report on the use of vacuum-assisted closure (VAC) therapy in the treatment of late deep infections after open Achilles tendon reconstruction. Six patients (5 males [83.33%], 1 female [16.67%]; mean age, 52.8 [range 37 to 66] years) were been treated using an identical protocol. Surgical management consisted of debridement, lavage, and necrectomy of infected tendon parts. The VAC therapy was used for local wound preconditioning and infection management. A continuous negative pressure of 125 mm Hg was ap

The efficacy of ultrasound in the diagnosis of long head of the biceps tendon pathology.

The use of shoulder ultrasound as an imaging modality has recently gained widespread attention; however, the ability of ultrasound to diagnose long head of the biceps tendon pathology accurately still remains unclear. The biceps tendons in 71 patients were prospectively evaluated by comparison of standard ultrasonographic and arthroscopic examinations. Arthroscopic examination was used as the gold standard comparison. Ultrasound showed a 100% specificity and 96% sensitivity for subluxation or dislocation. Ultrasound detected all complete ruptures of the biceps tendon but detected none of the 2

Strength analysis and comparison of the Teno Fix Tendon Repair System with the two-strand modified Kessler repair in the Achilles tendon.

BACKGROUND: Various options exist for repair of Achilles tendon ruptures, yet a method that consistently provides sufficient gap resistance to allow early mobilization remains elusive. This study compares the mechanical performance of a modified Kessler repair with that of the Teno Fix Tendon Repair System (Ortheon Medical, LLC) in matched pairs of cadaver Achilles tendons. METHODS: The two-strand modified Kessler repair was selected as the control construct on the basis of literature review and physician interviews which identified it among the many options as a popular method for repair of

Differential expression of neural cell adhesion molecule (NCAM) after tenotomy in rabbit skeletal muscle.

Tenotomy is a commonly encountered event in orthopaedic surgery. In 23 rabbit extensor digitorum longus (EDL) muscles, within 24 h after tenotomy, a marked drop in maximum force production occurred. This was not explainable based on architectural changes and histological examination using standard markers for muscle injury, i.e., haematoxylin and eosin morphology, developmental myosin heavy chain (MHC) immunolabeling, and quantitation of muscle fiber type percentage, area and distribution. The expression of neural cell adhesion molecule (NCAM), a glycoprotein expressed during muscle developmen

How to examine a foot and what to do with a bunion.

The foot is a remarkable structure that provides stable support for the body, lessens impact loading during gait, and allows for forward motion that is energy efficient. Because of the enormous loads concentrated at the joints in the foot and ankle with normal activities, frequent injuries occur and present to the primary care provider. Systemic conditions and improper footwear also contribute to the number of foot injuries that require medical attention. This article provides the clinician with the practical tools necessary for a complete examination of the foot and ankle. This article also i

Biomechanical and histological observations of the dog patellar tendon after removal of its central one-third.

The use of a central one-third patellar tendon as an autograft for surgical reconstruction of a damaged cruciate ligament is common. Few complications of its use have been reported. However, recent clinical studies indicate that decreased quadriceps strength, decreased range of motion, decreased thigh circumference, and patellofemoral problems can be associated with this procedure. Some of these complications may result from alterations in the biomechanical properties of the remaining patellar tendon. The objective of this study was to examine biomechanically and histologically the fate of the

Fingertip Injuries and Amputations: A Review of the Literature.

The fingertip is defined as the part of the digit distal to the insertion of the extensor and flexor tendons on the distal phalanx. Devastating injuries to the hand occur every year that lead fingertip amputations in thousands of people. The highest incidence rates are usually seen in children less than five years old and in adults over the age of 65. There are various presentations of injury that may end up with post-traumatic fingertip amputation, including lacerations, avulsions, and crush injuries. The fingertip is vital for sensation, as it has a high concentration of sensory receptors, a

Diagnostic imaging of upper extremity trauma.

The radiologic evaluation of upper extremity trauma relies primarily on standard as well as optional radiographic projections. Certain injuries are more fully evaluated with fluoroscopy, arthrography, ultrasound, computed tomography, or magnetic resonance imaging. The imaging approach to upper extremity trauma is presented, with emphasis on subtle or commonly overlooked lesions. The indications for evaluation by fluoroscopy, arthrography, and advanced imaging modalities are discussed.
