Successful slow pathway ablation in a patient with a rare unroofed type coronary sinus.

We report a case of atrioventricular nodal reentrant tachycardia coexistent with a coronary sinus (CS) anomaly. During a standard electrophysiological study, the CS could not be cannulated despite several attempts. A persistent left superior vena cava angiogram through the left brachial vein confirmed an unroofed type CS. Successful slow pathway ablation from the right posterior paraseptum lesion was achieved using an anatomical approach.

A case of difficult temporary pacing.

A 55-year-old lady came with recurrent syncope due to complete heart block. A temporary pacing lead could not be advanced via the inferior vena cava (IVC) to the right atrium. A venogram demonstrated that the IVC had no connection to the right atrium. The temporary lead could then be advanced through the azygos and superior caval vein to the right atrium and ventricle. The patient had no congenital heart disease or visceral heterotaxy.

Shut that door: a man with contemporaneous multisite paradoxical embolization.

Patent foramen ovale (PFO) has been recognized as mediator of paradoxical embolism. Concurrent multisite paradoxical embolization is a quite rarely reported occurrence. We present a case of multisite paradoxical embolism (pulmonary, cerebral, upper limb) mediated by a large right-to-left shunt mediated by a PFO with associated a redundant Eustachian valve, successfully treated with PFO transcatheter closure and vena cava filter placement.

Unusual presentation of idiopathic retroperitoneal fibrosis: case report.

Idiopathic retroperitoneal fibrosis (IRF) is an uncommon entity described as progressive proliferation of connective tissues leading to a fibrous plaque-like lesions that encases the aorta and inferior vena cava inferior to the level of the renal arteries. Mass forming retroperitoneal fibrosis is rare. We present a rare case of a unilateral focal retroperitoneal mass simulating a tumour encasing the middle third of the left ureter, with involvement of the ipsilateral scrotum.

Retrotracheal aberrant left brachiocephalic vein: CT findings.

We present a child with double aberrant left brachiocephalic vein (ALBCV) that was an incidental finding on CT. The anterior and thin branch was above the aortic arch and behind the truncus brachiocephalicus and drained into the superior vena cava (SVC). The posterior and thick branch of the ALBCV coursed posterior to the trachea and oesophagus and joined with the azygos vein before draining into the SVC. To our knowledge, retrotracheal ALBCV has not been previously described.

Catheter-based closure of an atrial septal defect in scimitar syndrome.

We report a patient with abnormal systemic blood supply to the right lung and right-sided anomalous pulmonary venous drainage to the inferior vena cava (scimitar syndrome or pulmonary venolobar syndrome). In addition, she had an atrial septal defect, underdeveloped right pulmonary artery, an aberrant right bronchus, and tracheobronchomalacia. She improved markedly after palliative interventional closure of her atrial septal defect.

Extensive primary cardiac liposarcoma with multiple functional complications.

Cardiac liposarcoma in a patient manifested multiple cardiac functional complications. Three dimensional reconstruction of the heart with multidetector row computed tomography made apparent each of the cardiac complications that resulted from the tumour's invasion. On the basis of these findings, the floating mass in the pulmonary artery and the compressing mass around the superior vena cava and interatrial septum were successfully resected. Pathological examination of the mass was consistent with well differentiated liposarcoma.

Pulmonary embolism-the initial manifestation of renal cell carcinoma in a young woman.

We report a case of massive pulmonary embolus demonstrated on CT in a young woman presenting with dyspnea, with no known risk factors for embolism. Abdominal CT on further investigation showed a renal tumor invading the left renal vein and the inferior vena cava as the cause of the pulmonary embolus. In a patient presenting with pulmonary artery embolism without venous thrombosis, the differential diagnosis should include an occult tumor as the cause of the embolus.

Repair of total anomalous pulmonary venous connection without cardiopulmonary bypass.

Two infants with asplenia syndrome and pulmonary venous obstruction underwent repair of total anomalous pulmonary venous connection without cardiopulmonary bypass in 1988. Using a side-biting clamp, an anastomosis between the left upper pulmonary vein and left superior vena cava-atrial junction was satisfactorily created without undue instability. Furthermore, both cases had the advantage of efficient growth in the anastomotic site postoperatively.

Incontinentia pigmenti: a case associated with cardiovascular anomalies.

Various cutaneous and developmental defects of the eyes, teeth, skeleton, and central nervous system have been detected in infants with incontinentia pigmenti. We report an isolated case of incontinentia pigmenti in a 6-month-old girl in association with tricuspid insufficiency, an abnormal shunt of the right pulmonary vein into the superior vena cava, and pulmonary hypertension. We believe that our findings will help to confirm the association of cardiovascular anomalies in IP.

Diffuse pulmonary arteriovenous malformation in a child with polysplenia syndrome.

Pulmonary arteriovenous malformation (PAVM) is usually seen as a well-circumscribed cystic mass. In this communication we describe a diffuse arteriovenous malformation in a 5-month-old infant. The lesion was seen only at the microscopic level and was associated with abdominal heterotaxy, atrial situs solitus, polysplenia, interrupted inferior vena cava, atrial septal defect, and atrioventricular canal. This uncommon association may be within the spectrum of polysplenia syndrome.

[Venous thromboembolic disease].

Prevention and treatment of venous thrombosis and pulmonary embolism have improved dramatically in the last few years owing to: 1) better control of the coumarin activity with the International Normalized Ratio system; 2) appearance on the market of the low-weight molecular heparins and enlargement of their field of prescription; 3) indications of thrombolysis in severe pulmonary embolism; 4) availability of inferior vena cava filters, effective in preventing recurrent pulmonary embolism.

[Protein C deficiency and the P pill].

A 18-year old woman was hospitalized because of vena cava inferior thrombosis, renal vein thrombosis and pulmonary embolism due to inherited protein C deficiency and three months of contraceptive treatment with oestrogen hormones. Protein deficiency was detected in 12 other members of the family. The father had had a pulmonary embolism 12 years earlier, and two other members may have died of thromboembolism.

Renal cell carcinoma: presenting with congestive hepatosplenomegaly and rare haematological complications.

We report here a case of right-sided renal cell carcinoma who presented with hypertension and multi-organ metastases. Haematological manifestations noted were erythrocytosis, thrombocytosis and leukaemoid reaction. Of these leukemoid reaction and thrombocytosis are very rare. The patient had hepatosplenomegaly which was found to be congestive in origin due to the pressure of the tumour on the hepatic vein and the inferior vena cava. These rare features make it an unusual case.

Stenosis of pulmonary veins with total left anomalous pulmonary venous return.

A case of congenital stenosis of the pulmonary veins and total left anomalous pulmonary venous return is reported. There was also obstruction between the left pulmonary veins and left vena cava superior. The diagnosis was based on 2-dimensional and Doppler echocardiography and was confirmed by angiocardiography. The 6-month-old baby with these anomalies was operated upon successfully. These cardiac malformations are very rare separately and their combination has not been previously reported.

Atresia of the right atrial ostium of the coronary sinus.

A case of asymptomatic congenital occlusion of the ostium of the coronary sinus is described. The myocardial venous drainage was maintained via a persistent left superior vena cava as well via ectatic, widened atrial veins of the dorsal wall of the left atrium. The study shows that complete ostial occlusion of the coronary sinus does not reduce cardiac venous drainage. The view of the literature allows a comparison with the comprehensive classification of coronary sinus anomalies.

Atresia of the coronary sinus orifice.

The cases of two children are reported in whom atresia of the right atrial orifice of the coronary sinus was diagnosed at cardiac catheterization. Only one previous case diagnosed in life has been found in the literature, though there are 34 necropsy reports. In nearly half the cases a persistent left superior vena cava has afforded the only outflow channel for the coronary sinus, and occasionally this may have surgical implications.

Congenital absence of the portal vein.

A 4 1/2-year-old girl is described in whom the portal vein is absent and the venous return from the gastrointestinal tract, spleen, and pancreas is through the left renal vein into the inferior vena cava. She had a hepatoblastoma, which was treated surgically by right hepatic lobectomy. Regeneration occurred in the remaining left lobe within 3 months after surgery. This is the second such case documented in the literature.

[Retroperitoneal fibrosis with cholestatic onset 4 years before malignant lymphoma].

A 9 year old boy presented an "idiopathic" retroperitoneal fibrosis. Fibrosis extended to the liver, compressing the major biliary tree, the portal vein, the urinary tract and the inferior vena cava. The process led two successive bilio-digestive shunts and was controlled after one year corticosteroid treatment. A malignant lymphoma was responsible for the death which occurred 4 years after the initial symptoms of retroperitoneal fibrosis.

Locally advanced adrenal tumor invading the stomach revealed by upper gastrointestinal hemorrhage: a case report.

INTRODUCTION AND IMPORTANCE: Adrenocortical carcinoma (ACC) is a rare and highly aggressive malignancy, often diagnosed at an advanced stage. Direct invasion into adjacent organs is possible, but gastric involvement is exceedingly uncommon. We report a rare case of a locally advanced adrenal tumor invading the stomach and presenting with upper gastrointestinal bleeding. CASE PRESENTATION: A 42-year-old man presented with hematemesis and melena evolving over 24 h. Physical examination revealed pallor, hypotension, and a palpable left upper quadrant mass. CT imaging showed a large left adrenal
