Prophylactic LVAD Enabling High-Risk Mitral Repair-Extending Beyond the Guidelines.

Both surgical and percutaneous mitral repair remain contraindicated in patients with severe degenerative mitral regurgitation (DMR) with severe left ventricular (LV) dysfunction because of inadequate LV reserve and increased LV work with a competent mitral valve. We report a 55-year-old gentleman who presented in cardiogenic shock with missed severe DMR and severe LV dysfunction, in whom we performed a high-risk mitral repair and insertion of a prophylactic CentriMag LV assist device. This innovative approach was found to be successful with significant patient improvement in both LV function a

Dysphagia in the setting of left ventricular assist device hemolysis.

A 69-year-old man with advanced heart failure treated with a continuous-flow left ventricular assist device presented for evaluation of dark urine and severe dysphagia. Because of evidence of ongoing intravascular hemolysis with device dysfunction, there was a clinical suspicion for pump thrombosis. He had progressive end-organ dysfunction and was therefore treated with tissue plasminogen activator with prompt resolution in hemolysis and dysphagia. Although symptoms of smooth muscle dystonia could represent worsening heart failure in the setting of device failure, the observation may also be r

The Role of Cardiac Magnetic Resonance Imaging in Severe Anorexia Nervosa.

Objective Anorexia nervosa (AN) patients are at an increased risk of developing cardiac complications including bradyarrhythmias, systolic dysfunction, pericardial effusions, and sudden cardiac death. Although previous echocardiographic studies in AN patients have demonstrated a reduction in overall left ventricular (LV) mass, systolic dysfunction, and silent pericardial effusions, little is known about the role of cardiac magnetic resonance imaging (CMR) in assessing this patient population. The objective of this study was to assess cardiac indices and the presence of myocardial fibrosis in A

Coronary steal from an anomalous circumflex artery diagnosed 25 years after ventricular septal defect closure: a case report.

BACKGROUND: Association of anomalous origin of a coronary artery arising from pulmonary artery (PA) with other congenital heart defects, such as ventricular septal defects (VSDs), is uncommon. In such cases, coronary anomalies may be overlooked and underdiagnosed. CASE SUMMARY: We report a case of late diagnosis of a circumflex coronary artery arising from the right PA, 25 years after VSD-closure during infancy with a complicated post-operative course with severe left ventricular dysfunction. Eventually being discharged, she remained asymptomatic until reaching adulthood, when she developed a

Association Between Sacubitril/Valsartan Use and Risk of Major Adverse Cardiovascular Events in Patients With Durable Left Ventricular Assist Devices: A Multi-Institutional Real-World Cohort Study.

BACKGROUND: Durable left ventricular assist devices improve survival in patients with advanced heart failure (HF), yet long-term outcomes remain limited by recurrent decompensation and end-organ dysfunction. Evidence on the prognostic association of angiotensin receptor-neprilysin inhibitor (ARNI) therapy after left ventricular assist device implantation remains limited. METHODS: We conducted a retrospective multicenter cohort study using the TriNetX Research Network. Adults undergoing durable left ventricular assist device implantation between 2015 and 2025 were identified. Patients receivin

Epicardial Adipose Tissue in Duchenne Muscular Dystrophy Cardiomyopathy.

BACKGROUND: Duchenne muscular dystrophy (DMD) has unique characteristics when compared with other forms of non-DMD dilated cardiomyopathy. We sought to describe epicardial adipose tissue (EAT), the fat layer surrounding the myocardium, and investigate its relationship to longitudinal cardiac changes in DMD. METHODS: This single-center, retrospective, cohort study included all DMD patients with ≥5 cardiac magnetic resonance studies between 2015 and 2022. EAT thickness was measured in the right atrioventricular groove during end diastole. Adiposity data were obtained from dual-energy x-ray abso

Determinants of prognosis of patients with aortic regurgitation who undergo aortic valve replacement.

Insidious and potentially irreversible left ventricular dysfunction may develop in patients with aortic regurgitation. To determine whether preoperative variables can predict postoperative outcome, 113 consecutive patients with aortic regurgitation who underwent surgical correction between 1962 and 1977 were studied and survivors were followed up for 4.6 +/- 3.3 years. Clinical and hemodynamic examinations were made in all patients before the operation. Echocardiograms were performed in 44 patients preoperatively and in 36 patients postoperatively. Perioperative or postoperative death due to c

Impaired right and left ventricular diastolic myocardial mechanics and filling in asymptomatic children and adolescents after repair of tetralogy of Fallot.

AIMS: After tetralogy of Fallot (TOF) repair patients have right ventricular (RV) dysfunction and reduced exercise tolerance. Diastolic dysfunction may be important but is as yet poorly characterized. The early diastolic strain rate (SR) is a measure of ventricular relaxation, and may be useful to assess diastolic mechanics in TOF. We hypothesized that children after TOF repair have diastolic dysfunction and dyssynchrony by this measure, and sought to determine their relationship with pulmonary regurgitation (PR), RV enlargement, and aerobic exercise capacity. METHODS AND RESULTS: We prospect

Bail-out transcatheter aortic valve implantation to reduce severe acute aortic regurgitation in a failing homograft secondary to HeartMate II ventricular assistance device.

Left ventricular assistance with surgically implanted pump devices like the HeartMate may be crucial in selected patient with end-stage heart failure. However, mainly due to its high aortic output, the pump itself may induce severe aortic regurgitation that could result in paradoxycal worsening of the anterograde perfusion after the initiation of the support. Surgical or percutaneous occlusion of the aortic valve has proved useful in these kinds of patients. Here we present a successful case of bail-out CoreValve implantation after HeartMate II positioning complicated by acute severe aortic re

Serious takotsubo cardiomyopathy: an autopsy case presenting severe irreversible myocardial damage after frequent episodes of recurrence.

BACKGROUND: Takotsubo cardiomyopathy is characterized by transient dysfunction of the medial to apical segment of the left ventricle. Recurrence within a few months or years has been reported and serious complications, including arrhythmia, acute cardiac shock and cardiac rupture, can arise; however, recurrence is rare and takotsubo cardiomyopathy is typically a reversible functional disorder. CASE PRESENTATION: A 91-year-old Japanese woman with a past medical history of angina pectoris, hypertension and uterine carcinoma noted bilateral axillary pain and presented herself to an emergency roo

Shared Decision-Making-Guided Prehabilitation for High-Risk Anomalous Aortic Origin of the Left Coronary Artery.

OBJECTIVE: To describe shared decision-making-guided prehabilitation as a bridge to repair in unrepaired high-risk anomalous aortic origin of the left main coronary artery. KEY STEPS: A young woman developed cardiogenic shock after hysteroscopic hemorrhage, required extracorporeal membrane oxygenation, and was found to have severe ostial compression of an anomalous left main coronary artery. After initial weaning, oral feeding was followed by recurrent shock requiring renewed support. Because repair was deferred for ventricular dysfunction, malnutrition, frailty, and emotional distress, she u

Hypertrophic cardiomyopathy overview: Genetics, screening, and treatment.

Hypertrophic cardiomyopathy (HCM) is a common genetic cardiac disorder marked by unexplained asymmetrical left ventricular wall thickening. This condition exhibits histopathological hallmarks including enlarged heart cell muscles, fibrosis, and associated with left ventricular diastolic dysfunction and outflow tract obstruction. Hypertrophic cardiomyopathy results from sarcomere protein mutations and can cause significant cardiac symptoms, although some, especially young individuals, may remain asymptomatic or have mild symptoms. However, sudden cardiac death remains a significant risk for und

Sudden death in young people with apparently isolated mitral valve prolapse.

Ventricular electrical instability and sudden death in mitral valve prolapse (MVP) have been related to mitral valve regurgitation and left ventricular dysfunction, autonomic nervous system abnormalities, or underlying cardiomyopathy. The aim of the present study was to assess the frequency, nature and pathophysiologic significance of histologic myocardial abnormalities in young patients with apparently isolated MVP and sudden cardiac death. Among 163 cases of sudden cardiovascular death in young people, MVP was the only cardiac pathology found at postmortem gross examination in 17 (10%) (12 f
