Sarcopenia After Neoadjuvant Radiation Is Associated With Increased Length of Stay Following Resection of Retroperitoneal Sarcoma.

BACKGROUND: Sarcopenia, defined as loss of muscle mass and function, has been identified as a negative prognostic factor in outcomes of major abdominal cancer operations. We sought to understand the impact of sarcopenia, using the psoas muscle index (PMI) as a surrogate, during neoadjuvant radiation (NR) on perioperative outcomes in patients with retroperitoneal sarcoma (RPS). METHODS: We performed a single-institution retrospective review of patients with RPS undergoing resection between 2017 and 2024. Data were extracted for patient demographics, details of NR, surgical resection, length of

Esophageal body adaptation to Nissen fundoplication: Increased esophagogastric outflow resistance yields delayed and sustained peristaltic contractions without increased amplitude.

BACKGROUND: Improvement in lower esophageal sphincter (LES) competency after laparoscopic Nissen fundoplication (LNF) is well established, yet esophageal body physiology data are limited. We aimed to describe the impact of LNF on whole esophagus physiology using standard and novel manometric characteristics. METHODS: A cohort of patients with an intact fundoplication without herniation and no postoperative dysphagia were selected and underwent esophageal manometry at one-year after surgery. Pre- and post-operative manometry files were reanalyzed using standard and novel manometric characteris

CT-Based Body Composition and Frailty as Predictors of Survival Among Older Adults With Gastrointestinal Malignancies.

BACKGROUND: Older adults with cancer are at an increased risk of treatment related toxicities and early death. Routinely collected clinico-demographic characteristics inadequately explain this increased risk limiting accurate prognostication. Prior studies have suggested that altered body composition and frailty are independently associated with worse survival among older adults with cancer; however, their combined influence remains unclear. METHODS: We used data from a single-institution prospective cohort study of older adults (≥ 60 years) who underwent geriatric assessment (GA) at the time

Abnormal Foot Progression Angle Kinematics in Cervical Dystonia Improved After Osteopathic Manipulative Medicine: A Prospective Case Series.

Introduction Cervical dystonia (CD), a rare disorder, is the most common form of dystonia, a movement disorder. Impairments in activities of daily living and quality of life may result from chronic pain, perceived stigma, difficulty walking, and/or lack of control over movements. Studies of treatments for difficulty walking in CD have been inconclusive. Osteopathic manipulative medicine (OMM) has been used to improve gait biomechanics in other health conditions. Foot progression angle (FPA) while walking indicates functional gait abnormalities that increase the risk of knee injury and osteoart

Body composition and osteoporotic fracture using anthropometric prediction equations to assess muscle and fat masses.

BACKGROUND: Obesity is protective of bone health; however, abdominal obesity is associated with a higher fracture risk. Little is known about whether body composition protects or adversely affects osteoporotic fractures because of practical issues regarding assessment tools. This study aimed to evaluate the association of predicted body composition with fracture risk to determine the distinctive and differing effects of muscle or fat mass on bone health outcomes in the general population. METHODS: This population-based, longitudinal cohort study used 2009-2010 Korean National Health Insurance

M2 macrophage-derived exosomes promote the c-KIT phenotype of vascular smooth muscle cells during vascular tissue repair after intravascular stent implantation.

Rationale: For intravascular stent implantation to be successful, the processes of vascular tissue repair and therapy are considered to be critical. However, the mechanisms underlying the eventual fate of vascular smooth muscle cells (VSMCs) during vascular tissue repair remains elusive. In this study, we hypothesized that M2 macrophage-derived exosomes to mediate cell-to-cell crosstalk and induce dedifferentiation phenotypes in VSMCs. Methods:In vivo, 316L bare metal stents (BMS) were implanted from the left iliac artery into the abdominal aorta of 12-week-old male Sprague-Dawley (SD) rats fo

Innovative gastric endoscopic muscle biopsy to identify all cell types, including myenteric neurons and interstitial cells of Cajal in patients with idiopathic gastroparesis: a feasibility study (with video).

BACKGROUND AND AIMS: The pathophysiology of some GI neuromuscular diseases remains largely unknown. This is in part due to the inability to obtain ample deep gastric wall biopsies that include the intermuscular layer of the muscularis propria (MP) to evaluate the enteric nervous system, interstitial cells of Cajal (ICCs), and related cells. We report on a novel technique for gastric endoscopic muscle biopsy (gEMB). METHODS: Patients with idiopathic gastroparesis were prospectively enrolled in a feasibility study by using a novel "no hole" gEMB. Main outcome measures were technical success, ad

[A case of anaphylactoid shock occurring immediately after the initiation of second intravenous administration of high-dose immunoglobulin (IVIg) in a patient with Crow-Fukase syndrome].

We report a case of anaphylactoid shock occurring immediately after the initiation of second intravenous administration of high-dose immunoglobulin (IVIg) in a patient with Crow-Fukase syndrome. The patient was a 57-year-old woman, who was admitted to our hospital because of numbness and muscle weakness in the four extremities, difficulty in walking, and foot edema. On admission, her skin was dry and rough, and also showing scattered pigmentation, small hemangiomas, and hypertrichosis in both legs. She had distal dominant muscle weakness, more prominent in her legs, and was not able to walk. D

Orbital tissue-derived T lymphocytes from patients with Graves' ophthalmopathy recognize autologous orbital antigens.

Lymphocytic and other mononuclear cell infiltration of the retro-bulbar space is observed in Graves' ophthalmopathy (GO). We investigated the antigenic character of orbital adipose/connective tissue and muscle from 21 euthyroid patients with severe GO after orbital surgery. Orbital tissue proteins were separated and recovered in soluble form by means of an electroelution technique. Twenty-two protein fractions, identified according to their molecular mass ranges, were used as antigens for orbital tissue-derived and peripheral blood T lymphocytes. Seventeen T cell lines from 6 patients were est

Simulation-based evaluation of stretching behavior of NiTi wire-knitted implants for reconstructive plastic surgery.

A comprehensive study of the mechanical response of a titanium nickelide (TiNi) wire-knitted implant was conducted through tensile testing and finite element simulation using the Bergström-Boyce hyperelastic model calibrated via MCalibration and solved in ANSYS. The elongation-load curves for both the ribbon tissue scaffold (RTS) and the composite samples consisting of a RTS with biological tissues sprouted therein (RTS/bio-tissue) demonstrated nonlinear superelastic behavior. Under longitudinal tension, the biological tissue and the RTS exhibited similar deformation patterns. An evaluation of

Unmasking the "Braided" Bowel: Chronic Intestinal Pseudo-Obstruction-A Case Report.

Background and Clinical Significance: Chronic intestinal pseudo-obstruction (CIPO) clinically presents as a mechanical obstruction. When patients present with signs of impending ischemia, emergency exploratory laparotomy is indicated. Differentiating CIPO from other etiologies preoperatively is highly challenging, making intraoperative findings and definitive histopathology crucial for an accurate diagnosis. This report aims to elucidate a novel macroscopic presentation of myopathic CIPO and explore the potential biomechanical mechanisms underlying the observed structural anomaly. Case Present

[Esophageal perforation: surgical outcomes in a referral center].

BACKGROUND: Esophageal perforation is a rare and potentially life-threatening condition. Its main etiologies include iatrogenic, traumatic, and neoplastic causes. Diagnosis is based on imaging and endoscopic studies. Although some cases can be managed conservatively, most require endoscopic or surgical interventions. CLINICAL CASE: A case study was conducted analyzing the clinical characteristics, treatment, and outcomes of patients with esophageal perforation treated at a referral hospital between 2019 and 2023. Twenty patients were included, with a mean age of 52 years and a predominance of

Leiomyomatosis Peritonealis Disseminata during successful pregnancy: First report in Africa.

INTRODUCTION AND IMPORTANCE: Leiomyomatosis peritonealis disseminata is a very rare disease that involves the presence of multiple benign tumors of smooth muscle origin all over the peritoneal cavity. Due to its ability to mimic malignant pathologies, it is associated with significant diagnostic and therapeutic challenges. This case highlights a unique case of LPD to further raise awareness and provide useful information on how to manage this condition. CASE PRESENTATION: A 28-year-old female was admitted with progressive increase in abdominal distension and discomfort. Imaging studies reveal

A carboxymethyl cellulose enhanced F127 hydrogel with anti-inflammation and peritoneum regeneration activities for preventing peritoneal adhesion.

Postoperative peritoneal adhesion (PPA) is a common and severe postoperative complication that does not require satisfactory precautions by surgeons. By combining Pluronic F127 and carboxymethyl cellulose (CMC), we developed an F127- CMC hydrogel (designated as FC hydrogel), and characterized and tested its physicochemical properties, biosafety, bioactivities, and anti-PPA effects. The FC hydrogel was free-flowing and injectable below 12.5 °C and gelled in situ at body temperature. The CMC network provided anti-inflammatory and wound healing abilities to FC hydrogel, which demonstrated 35.2 %

Intravenous Leiomyomatosis of the Uterus: An Intriguing Case Revealed through Anatomopathological Examination.

INTRODUCTION: Intravenous leiomyomatosis (IVL), a rare type of uterine leiomyoma (its incidence is about 0.25% to 0.40% of patients who present uterine fibroma), is characterized by the formation and growth of benign leiomyoma tissue within the vascular wall or lymphatic lumen. Herein, we presented a case of early stage of IVL successfully treated by surgical removal and a review of actual medical recommendations. OBSERVATION: A 49-year-old woman, gravida 2 para 2, presented to our department with hypogastric pain. On physical examination, a palpable mass in the hypogastrium was noted. Pelvic
