Evaluation of Hospital-Acquired Anemia and Contributing Factors in ICU Patients: A Retrospective Cohort Study.

Objectives: Hospital-acquired anemia (HAA) is an important clinical problem that can adversely affect outcomes, particularly in critically ill patients. This study aimed to determine the incidence of hospital-acquired anemia in adult ICU patients and to evaluate its relation with diagnostic phlebotomy procedures and other etiologic factors. Methods: Data from 1170 adult ICU admissions between 1 January and 31 December 2024 were retrospectively reviewed and 321 patients were included in the analysis. The median age of the study cohort was 72 years (range: 18-102), with 45.2% females (n = 145) a…

Carbimazole-Induced Severe Acquired Aplastic Anemia in a Patient with Graves' Disease: A Case Report.

Background and Clinical Significance: Carbimazole is a widely used medication to treat Graves' disease, although it can rarely cause acquired aplastic anemia, a complication that happens in less than 0.01% of people. Case Presentation: We report a 36-year-old woman treated with supratherapeutic dose carbimazole who, after approximately six months of treatment, developed high grade fever, severe menorrhagia, spontaneous epistaxis, and pancytopenia. Her bone marrow biopsy showed severe bone marrow failure with only 5% cellularity and trilineage hypoplasia. Other potential causes were ruled out. …

Differential diagnosis of microcytic hypochromic anemia in dogs: the role of hepcidin and reticulocyte hemoglobin indices.

Microcytic hypochromic anemia in dogs represents a persistent diagnostic challenge, particularly when differentiating between absolute iron deficiency and inflammation-driven functional iron restriction. Conventional biomarkers, including serum iron and ferritin, present important limitations in terms of sensitivity, specificity, and temporal resolution, restricting their ability to accurately reflect dynamic changes in iron metabolism. This review critically evaluates the diagnostic performance and clinical relevance of hepcidin and reticulocyte hemoglobin indices (CHr/RET-He) as advanced bio…

Fatal Presumed Rhino-Orbito-Sinus Mucormycosis in a Child With Fanconi Anemia: A Case Report.

We report the case of a 10-year-old male patient with bone marrow aplasia secondary to Fanconi anemia who was initially admitted for febrile purulent tonsillitis (no throat culture was performed before antibiotic initiation). Despite several courses of broad-spectrum antibiotic therapy, the clinical course was marked by persistent fever and progressive right-sided orbitofacial cellulitis. Imaging revealed extensive involvement of the right maxillary sinus with orbital extension. The appearance of a necrotic black palatal lesion (eschar) in the setting of profound immunosuppression is highly ch…

Severe Chronic Anemia and Tumor-Mimicking Colonic Lesions as an Atypical Presentation of Crohn's Colitis.

Crohn's colitis may rarely present with severe bleeding and tumor-mimicking colonic lesions, creating concern for malignancy. A 20-year-old bedridden man with cerebral palsy had recurrent hematochezia and transfusion-dependent microcytic anemia, with hemoglobin 2.8 g/dL. Colonoscopy showed diffuse raised island-like plaques with vascular-pattern loss, rectal sparing, edematous ileocecal valve, and an oozing transverse-colon lesion requiring clipping. Multisite biopsies showed chronic active colitis with crypt distortion, cryptitis, and crypt abscesses, without granulomas, dysplasia, or maligna…

The association between the degree of social vulnerability and maternal anemia and other adverse perinatal outcomes.

OBJECTIVE: Maternal anemia in pregnancy is associated with many adverse maternal, fetal, and neonatal outcomes. The role of social determinants of health (SDOH) in the development of maternal anemia and other adverse outcomes is continually evolving. One such newer tool, the Social Vulnerability Index (SVI), measures an individual's community-level SDOH, with higher SVI denoting higher vulnerability. Our objective was to evaluate the association between SVI, maternal anemia, and other adverse perinatal outcomes. METHODS: Retrospective cohort study of pregnant individuals delivering singletons…

Acute hemolytic anemia and methemoglobinemia in a pediatric oncology patient with G6PD deficiency: a case report.

BACKGROUND: Glucose-6-phosphate dehydrogenase (G6PD) deficiency is a common enzymatic disorder that is often asymptomatic until exposure to oxidative stress, such as certain medications. Rasburicase, used for tumor lysis syndrome (TLS) prophylaxis, can trigger hemolysis in G6PD-deficient patients. CASE PRESENTATION: We report a nine-year-old Asian boy with acute myeloid leukemia (AML) and hyperleukocytosis who developed severe hemolytic anemia and methemoglobinemia shortly after receiving rasburicase for TLS. Enzymatic testing suggested G6PD deficiency but was unreliable due to hyperleukocyto…

Febrile neutropenia in pediatric acquired aplastic anemia: a 20-year analysis of infections and mortality.

Acquired aplastic anemia (AA) is a life-threatening bone marrow failure disorder, and febrile neutropenia (FN) episodes in children with AA pose significant risks due to infections. The study aimed to investigate the incidence, types, and outcomes of infections during FN episodes in pediatric AA patients. This single-centre retrospective cohort study included 39 pediatric patients with AA diagnosed between January 2004 and January 2024. Data from medical records, including clinical characteristics, laboratory findings, immunosuppressive therapy details, microbiological results, and infection o…

The curious case of aplastic anemia: a fatal triple co-infection with Aspergillus, Mucorales, and Nocardia.

BACKGROUND: Concurrent opportunistic infections are rare but can lead to life-threatening complications in patients with hematologic disorders. Immunosuppression, particularly severe neutropenia, significantly increases the risk of invasive fungal and bacterial infections. We report a 34-year-old female with chronic hepatitis B initially presented with pancytopenia. Subsequent bone marrow evaluation revealed aplastic anemia, and she subsequently received immunosuppressive therapy. During hospitalization, she developed multiple infectious episodes, including fever, pulmonary nodular lesions, an…

Frailty in lung transplant recipients is associated with anemia and telomere dysfunction but independent of epigenetic age.

Frailty is a syndrome of vulnerability to stressors linked to worse outcomes before and after lung transplantation. However, the biological basis of this association is unknown. Biological correlates of aging include epigenetic reprograming, chronic inflammation, telomere dysfunction, and anemia. We hypothesized that these aging-associated biological processes would be associated with frailty in lung transplant recipients. In a nested case-control study, we compared 43 lung transplant recipients who were frail pretransplant and posttransplant with 43 nonfrail matched controls. We quantified pe…

Lack of association between the TMPRSS6 gene polymorphism (rs855791) and anemia: a comprehensive meta-analysis.

BACKGROUND: Anemia affects around 1.6 billion people worldwide and presents a significant challenge for healthcare providers. Despite the hemoglobin concentration being commonly used for diagnosis, identifying underlying causes remains challenging, particularly in vulnerable groups like children under five and pregnant women. Genetic factors, notably variations in the TMPRSS6 gene, are implicated in iron deficiency anemia, yet the precise relationship with anemia remains unclear. METHODS: A thorough literature search was conducted across databases, including Embase, Google Scholar, and PubMed…

Exploring the association between blood donation and anemia: A cross‑sectional study based on the NHANES.

BACKGROUND: Blood donation is essential for maintaining an adequate blood supply, but repeated donation can lead to iron depletion and anemia. Evidence from nationally representative populations is limited. METHODS: We conducted a cross-sectional analysis of adults (≥18 years) from the National Health and Nutrition Examination Survey (NHANES) 2011-2018. Blood donation within the past 12 months was self-reported. Outcomes included anemia (hemoglobin <13.0 g/dL in men, <12.0 g/dL in women), iron deficiency (serum ferritin <70 µg/L), and iron deficiency anemia (IDA, both conditions). Secondary o…
