INTRODUCTION: Ankle fractures are increasingly common in frail patients, with hospitalisation being the principal cost driver, particularly for the elderly who often need referral to nursing facilities. This study aims to identify factors affecting resource utilisation per admission (hospital and nursing) in the fixation of low-energy ankle fractures.
MATERIALS AND METHODS: This retrospective cohort study examined patients undergoing fixation for low-energy ankle fractures. The primary outcome was the length of hospitalisation. Secondary outcomes included delays in fixation and the need for r…

BACKGROUND: Chronic ankle instability is associated with balance deficits and maladaptive cerebellar neuroplasticity across age groups. However, it remains unclear how aging and chronic ankle instability interact to influence these abnormalities, particularly in cerebellar structures that are essential for proprioception and motor coordination. This gap in understanding limits the development of age-specific rehabilitation strategies.
QUESTIONS/PURPOSES: This study aimed to: (1) determine the interaction effect of chronic ankle instability and aging on sensorimotor deficits, (2) assess whethe…

BACKGROUND: Arteriosclerosis and cardiovascular disease (CVD) can greatly affect the progression of chronic obstructive pulmonary disease (COPD). However, standardized methods for evaluating arteriosclerosis in COPD have not been established. The cardio-ankle vascular index (CAVI) is a reliable marker of arterial stiffness and a potential marker for assessing arteriosclerosis. This study aimed to examine the associations between the CAVI and clinical parameters and to evaluate its predictive value for clinical outcomes in COPD.
METHODS: This retrospective, observational study included patient…

INTRODUCTION: Describing surgical wounds accurately poses challenges due to the diverse terminology used for complications. Existing evaluation methods do not cater specifically to surgical wounds from post-ankle fracture surgery with osteosynthesis. Given the unique anatomical challenges and treatment considerations (limited tissue coverage and blood supply as well as the surgical treatment with osteosynthesis), a targeted wound score is essential for ensuring consistent evaluation and high-quality care and thereby optimizing patient outcomes and satisfaction. The study aimed to develop a wou…

This study aimed to evaluate the impacts of a 4-week transcranial direct current stimulation (tDCS), balance training (BT), and an integrated program combining tDCS with BT on static and dynamic postural control in athletes suffering from chronic ankle instability (CAI); as well as to explore whether the combined program produces superior effects compared to either single intervention. Forty athletes with CAI were randomized into four groups: tDCS group, sham tDCS (s-tDCS) group, tDCS + BT group, and s-tDCS + BT group. Twenty minutes of 2 mA anodal or sham tDCS was applied either independently…

Introduction Traumatic foot and ankle injuries are frequent presentations to the emergency department (ED). The Ottawa Foot and Ankle (F&A) Rules are to help clinicians in decision making whether an X-ray of the foot or ankle is justified. Methods A retrospective analysis of ankle and foot X-ray requests from a South London Hospital ED was performed. Consecutive requests for both foot and ankle X-rays were assessed chronologically until n=100 for each cohort. The clinical documentation of each ED clerking and the X-ray request clinical details were then compared to the Ottawa Rules of that res…

BACKGROUND/AIM: Foot and ankle trauma represents a common reason for emergency department visits. While the majority of cases involve soft tissue injuries, radiographic imaging is frequently overutilized due to concerns about missed fractures, leading to increased costs and emergency department crowding. The Shetty test, a recently introduced clinical decision rule, may serve as a simpler alternative to established tools such as the Ottawa ankle rules. This study aimed to assess the diagnostic accuracy of the Shetty test and its potential role as a supportive tool within existing triage system…

BACKGROUND: Ankle arthroscopy (AA) is a commonly used operative technique to diagnose and treat a variety of intraarticular pathologies of the ankle joint. In AA, 2 portals are commonly established to achieve visualization of the joint: the anteromedial (AM) and anterolateral (AL) portals. However, the superficial peroneal nerve (SPN) runs near the anterolateral portal site; thus, creation of the AL portal is associated with neuropraxic injuries to the SPN.When AA is combined with additional procedures, such as a Brostrom-Gould ligament repair or open reduction internal fixation (ORIF), the us…

BACKGROUND: Force steadiness can be impaired under dual-task conditions in older adults. Since this impairment is attributed to their limited attentional resources, we hypothesized that the degree of cortical activity involved in muscle contraction would affect force steadiness under dual-task conditions. To test this hypothesis, based on the premise that dorsiflexion requires more cortical resources than plantarflexion, we compared the effects of additional motor and cognitive task demands on force steadiness between dorsiflexion and plantarflexion contractions in young and older adults.

The level of dynamic mechanical instability between the bony parts of the ankle joint provides important information on biomechanical function. However, the dynamics of the distance between the talus and lateral malleolus during gait remain unclear. This study aimed to compare the distance between the talus and lateral malleolus and the ankle joint angles during the stance phase of gait between individuals with chronic ankle instability (CAI) and healthy adults. The comparison was conducted using a synchronized ultrasound (US) imaging with a three-dimensional motion analysis (MA) system. This …

Cardio-ankle vascular index (CAVI) is an innovative indicator of large-artery stiffness, which is evaluated by the pulse wave velocity (PWV) measurement. Mortality and morbidity due to cardiovascular diseases among the general public with high-risk conditions such as hypertension are usually associated with arterial stiffness. CAVI modelizes the hazard of future cardiovascular events with standard risk factors. Additionally, the "European Society of Hypertension and Cardiology" included the aortic PWV assessment in managing hypertension in their updated guidelines in 2007. We conducted this sy…

Background Ankle fractures are very common injuries seen in an emergency setting. Initial management involves the application of below-knee plaster casts. At our local trauma meetings, we have observed that below-knee casts are often applied incorrectly which can result in suboptimal outcomes for patients and increase the burden on plaster room services if re-application is required. This quality improvement project aimed to assess the quality of below-knee cast applications for ankle fractures in two local district general hospitals (DGHs). Methodology We performed a closed-loop audit utilisi…

OBJECTIVE: Peroneus Longus Tendon (PLT), a viable anterior cruciate ligament (ACL) graft, shares similar biomechanics, making it suitable for reconstruction. Controversy exists over whether PLT transplants affects the donor ankle joint. The purpose of this study was to examine the recovery of knee joint function following arthroscopic ACL restoration using autologous PLT and its influence on the donor ankle joint.
METHODS: A retrospective analysis was conducted on 65 patients with ACL rupture who underwent PLT graft reconstruction in our hospital from January 2016 to December 2021. A three-di…
