We have evaluated the immediate heart rate response to standing and lying and in 100 Diabetic subjects aged 43 +/- 10 years who underwent five other cardiovascular autonomic tests. Using a specially devised scoring system the patients were divided into three groups: a) 58 subjects without autonomic neuropathy, b) 15 borderliners, c) 27 with autonomic neuropathy. The results were compared with 50, sex and age matched controls. We studied SL1 = ratio between R-R mean before lying and R-R maximum between the 20th to 25th beat and R-R minimum over the first 5 beats after lying. In controls SL1 was

GI motility changes little--if at all--with age in healthy patients. However, a variety of diseases, including diabetes and Parkinson's disease, may cause autonomic neuropathy that is manifest as a motility disorder in the GI tract. Autonomic neuropathy can cause dysmotility in the esophagus, stomach, and gut. Symptoms are often nonspecific, including difficulty in swallowing, nausea, vomiting, heartburn, indigestion, diarrhea, and constipation. Nonpharmacologic treatment includes management of underlying diseases, avoidance of anticholinergic medications, and dietary changes. Agents with prok

Carcinomatous autonomic neuropathy is a syndrome of autonomic dysfunction associated with malignant neoplasm. It is most often associated with small-cell lung cancer. Auto-immune mechanism is suspected to underlie this syndrome. Among the symptoms of autonomic dysfunction, intestinal pseudo-obstruction is most frequently seen. The autonomic dysfunction often precedes the discovery of the cancer. Several autoantibodies have been reported in paraneoplastic neuropathies. Anti-Hu antibody, which was originally reported in paraneoplastic sensory neuronopathy, has also been detected in some cases wi

Multiple system atrophy (MSA) is a heterogeneous group of central neurological degenerations often associated with diffuse deterioration of the hypothalamic cholinergic neurons. In the hypothesis of an altered cholinergic regulation of vasopressin release, we evaluated vasopressin response to metoclopramide (20 mg i.v.), a cholinomimetic agonist, in 12 MSA patients. In the same patients the hemodynamic and osmolal control of vasopressin was also evaluated. We found that MSA patients had significantly lower basal plasma vasopressin values and higher plasma osmolality than control subjects. Howe

Microcirculation disorders may cause functional deviation in gray matter cells of the spinal cord. One of the setting moments of the disorders is the subluxation of a vertebra as a result of the disturbance in carrying ability of the spinal disc in case of spinal osteochondrosis. In this position the soft tissues of the spinal motional well innervated segment are stretched, which induces irritation in the segmental part of the spinal cord including vegetative nervous structures. Subluxation of a vertebra causes changes in the structures and in the microcirculation vessels which grow simultaneo

Previous studies have shown that stimulation of the rat anterior pretectal nucleus (APtN) strongly depresses a spinal reflex to noxious heat without causing significant aversion or depression of other motor responses. It is not known if APtN stimulation can similarly reduce the aversiveness of electrical stimulation of the brain, nor is it known if APtN stimulation is itself rewarding or aversive. This study used a simple switch-off paradigm to examine the rewarding properties of APtN stimulation at different sites throughout the nucleus and also used the tail-flick test to determine if the st

Treatment with L-threo-3,4-dihydroxyphenylserine (L-threo-dops), a synthetic precursor of norepinephrine, significantly increased upright blood pressure in patients with multiple system atrophy but had no effect on the upright blood pressure of patients with pure autonomic failure. These results suggest that the site of action of L-threo-dops is central and that its pressor effect requires intact peripheral sympathetic neurons.

Acute coronary artery disease (CAD) is the leading cause of death in the Federal Republic of Germany. In spite of major advances in means of acute treatment, more than half of the patients die as a direct consequence of the disease; in the majority they die prior to hospitalization. One important reason appears to be the lack of scientific insight into the acute pathophysiologic mechanisms and, therefore, of specific and effective means of prevention. The present article presents a model of the triggering mechanisms of acute CAD based on findings of recent clinical-epidemiologic, pathological,

Orthostatic dysregulation (OD), originally a German-Scandinavian term partially corresponding to an Anglo-American concept of sympathotonic orthostatic hypotension, is characterised by altered cardiovascular control on standing, and its clinical features include dizziness, palpitation and, occasionally, orthostatic hypotension. The symptomatology suggests presence of cardiovascular adrenoceptor dysfunction, although the aetiology of OD has not been elucidated. The above situation prompted us to investigate autonomic nervous function in OD. The subjects were 8 patients with OD (20 +/- 2 years o

The results of examining patients aged 10 to 15 years suffering from vegetovascular dystonia (VVD) associated with the tetanic syndrome (TS) are provided. Patients with VVD not associated with the TS, and healthy patients of the comparable age made up the control. Use was made of the electrophysiological techniques (EEG, EMG, REG), vegetological methods (background, reactivity, activity support), neuropsychological (MMPI, Spielberger and Izenk tests), polygraphic and biochemical methods. The authors describe a number of disease characteristics (predominance of the latent forms of the TS, the n

A woman was referred with severe orthostatic hypotension at the age of 21. Ptosis, skeletal muscle hypotonia, and recurrent hypoglycaemia had been noticed in early childhood. There was noradrenergic denervation and adrenomedullary failure but baroreflex afferents, cholinergic innervation, and adrenocortical function were intact. Noradrenaline and adrenaline were undetectable in plasma, urine, and cerebrospinal fluid (CSF), but dopamine was 7-fold to 12-fold normal in plasma, 4-fold normal in urine, and 20-fold normal in CSF. Measurements of catecholamine metabolites showed further evidence for

Vesical function has been studied in vivo in alloxan-diabetic rats. Rhythmic contractions evoked as a micturition reflex by a controlled dis … ition reflex and improved the rate of bladder contractions. It is suggested that alterations of the autonomic nervous system can be expressed as functional vesical dysfunction in experimental diabetes.

Infant botulism is a rare disease with previous reports in Australia originating from its southern states. The first case to be recognized from Queensland is described in a four-month-old white girl. Complications included respiratory failure, the syndrome of inappropriate antidiuretic hormone secretion, urinary tract infection and autonomic dysfunction.

The author studies 109 patients with schizophrenia combined with cerebrovascular pathology (vegetovascular crises, essential hypertension, cerebral atherosclerosis). The effect of vascular disorders on the clinical picture and course of different forms of schizophrenia was found to be heterogeneous, being dependent on the severity of vascular disease, as well as the type of the course and progression of the schizophrenic process. Changes in the psychopathological structure of schizophrenic manifestations were the greatest in patients with periodic, shift-like, and slowly progressive schizophre

Five simple tests of autonomic nerve function were studied in 38 children (12.0 years, s.d. = 3.3) with insulin-dependent diabetes mellitus and a further 38 age and sex matched controls in order to validate the use of these tests in children. The five tests included one test (immediate heart rate response to standing) previously reported. Compared to controls, diabetics showed a significantly faster heart rate at rest (P = 0.022), a reduced heart rate variation during quite respiration (P = 0.003), a smaller change in heart rate following a single deep breath (P = 0.006), a lower Valsalva rati
