Recurrent leishmaniasis in kidney transplant recipients: report of 2 cases and systematic review of the literature.

The characteristics of 8 episodes of leishmaniasis with atypical manifestations in 2 Italian kidney transplant recipients are analyzed and contextualized among those of 52 other episodes of leishmaniasis observed in 19 transplant recipients found through a systematic review of the international literature. In all the patients, the initial episode was visceral leishmaniasis, which was associated with mucocutaneous involvement in 2 cases. With the exception of 1 case of post kala-azar dermal leishmaniasis, 2 episodes of Leishmania endophthalmitis, and 3 episodes of mucocutaneous leishmaniasis, a

Comparison of PCR, virus isolation, and indirect fluorescent antibody staining in the detection of naturally occurring feline herpesvirus infections.

Cats with clinical signs suggestive of ocular infection with feline herpesvirus type 1 (FHV 1) and cats without such signs were assayed by 3 methods to detect FHV. Comparison of polymerase chain reaction (PCR), virus isolation, and indirect fluorescent antibody staining techniques for the detection of FHV demonstrated higher sensitivity of PCR in detecting this common infectious agent of cats. Compared with PCR, sensitivity and specificity for virus isolation was 49% and 100%, respectively, and those of indirect immunofluorescence were 29% and 96%, respectively. FHV was detected in 13.7% of cl

[Recombination of alpha-viruses during mixed infection in cultured cells. Formation of hybrid forms of RNA of Venezuelan equine encephalomyelitis virus and Karelian fever virus in the region of genes for nucleocapsid and envelope proteins].

A recombination of the genomes of VEE and KF viruses, two alphaviruses of different serotypes, takes place in mixed infection of chick embryo fibroblasts and Vero cells with vaccine strain VEE-230 and KF-9298 strain. This recombination resulted in the production of recombinant virions with altered antigenic properties. Polymerase chain reaction (PCR) rapidly and effectively detected the hybrid sites of the genome in recombinant viruses. Short nucleotide sequences (approximately 20 n. p.) of a known primary structure in the RNA genomes, specifically reacting with the complementary PCR primers,

Chronic Meningitis.

Chronic meningitis may be caused by a large number of infectious agents, including spirochetes (Treponema pallidum, Borrelia burgdorferi), Mycobacterium tuberculosis, and fungi (primarily Cryptococcus neoformans). The incidence of these specific causes of chronic meningitis has been impacted since the advent of HIV infection, and new information is also available on how this epidemic has affected populations in developing countries of the world. In the area of diagnostics, the development of polymerase chain reaction has been a major advance that has increased our capabilities for identifying

Association of human herpesvirus 6 infection of the central nervous system with recurrence of febrile convulsions.

To determine the relationship of human herpesvirus-6 (HHV-6) infection to febrile convulsions, cerebrospinal fluid (CSF) from patients with a history of febrile convulsion were tested by polymerase chain reaction (PCR) amplification for HHV-6 DNA. HHV-6 DNA was detected in 9 of 10 samples from patients with exanthem subitum who showed neurologic symptoms. Also, 8 of 10 CSF samples from 8 patients who had three or more febrile convulsions and 1 of 7 CSF samples from patients who had a single febrile convulsion contained HHV-6 DNA. These data suggest that HHV-6 may invade the brain during the ac

Possible reasons for failure of conventional tests for diagnosis of fatal congenital toxoplasmosis: report of a case diagnosed by PCR and immunoblot.

Diagnosis of subclinical congenital toxoplasmosis has to rely on serological methods or isolation of the parasite. We present a case of congenital toxoplasmosis, in which conventional tests failed to establish the diagnosis. It was shown that this infant developed an intrathecal antibody response that was directed only against one of two Toxoplasma gondii strains used for routine diagnosis. In contrast to conventional tests, the diagnosis of cerebral toxoplasmosis could be established by using immunoblot and polymerase chain reaction (PCR). We therefore suggest that in unclarified cases, PCR a

Virus detection in the fetal tissue of a premature delivery with a congenital varicella syndrome. A case report.

Cases of congenital varicella syndrome have been published, to date, a single case reports. Isolation attempts of Varicella-Zoster virus from fetal tissues have, thus far, been unsuccessful. This is a first report of detection of Varicella-Zoster virus in fetal tissue by means of DNA hybridization technique in a typical case of congenital varicella syndrome in a premature delivery of the 27th gestational week. The case is documented anamnestically, sonographically, pathologically and virologically. In women with primary varicella infection during pregnancy good sonographical controls are recom

Strongyloides stercoralis Infection Causing Obstructive Jaundice and Refractory Pancreatitis: A Lesson Learned from a Case Study.

A 58-year-old Japanese woman came to our institution because of leg edema and abdominal distention. She had developed acute pancreatitis 5 times in the past 3 years. Dilation of the bile duct and main pancreatic duct without obstruction was observed on computed tomography and magnetic resonance cholangiopancreatography. The presence of Strongyloides stercoralis was highly suspected from the biopsy sample from the duodenal papilla. Polymerase chain reaction amplification and sequencing of small subunit rDNA from paraffin-embedded specimens identified the worm as S. stercoralis. All of the sympt

A rare case of concomitant tuberculosis of the nose, paranasal sinuses and larynx: clinical, histological and immunohistochemical aspects. A case report.

Extrapulmonary tuberculosis is a rare condition determined by Mycobacterium tuberculosis. It can affect any organ, and has a higher incidence with the increase of HIV infection, or in countries with high pulmonary tuberculosis. Diagnosis is difficult, mostly because of non-specific symptoms and a low rate of presentation for medical consult when symptoms do occur. Complete diagnosis is usually set by histological, immunohistochemical examinations, and also with Polymerase Chain Reaction (PCR) in selected cases. The authors present a case of concomitant tuberculosis of the nose, paranasal sinus

Nine human sparganosis cases in Thailand with molecular identification of causative parasite species.

Human sparganosis is one of the neglected diseases but important food-borne parasitic zoonoses. The disease is caused by larvae (spargana) of diphyllobothriidean tapeworm. Here, we describe nine cases of human sparganosis, caused by Spirometra erinaceieuropaei in a hospital in Thailand during 2001-2012. Clinical characteristics, treatment, and outcome of cases were revealed. Diagnosis and identification of causative parasite species was made by histopathological investigations followed by a polymerase chain reaction-based molecular method using formalin-fixed paraffin embedded tissues. The DNA

Babesia odocoilei infection in a Saskatchewan elk (Cervus elaphus canadensis) herd.

An 8-year-old female elk (Cervus elaphus canadensis) cow, presented for chronic severe weight loss and unthriftiness, was diagnosed with Babesia odocoilei infection based on blood smear evaluation, polymerase chain reaction (PCR), and DNA sequence analysis. Subsequently, velvet antler from a male that died acutely on the same farm was also PCR positive for Babesia spp. Both animals originated from a game ranch of Saskatchewan-bred and -raised animals with no known history of tick exposure, but with a history of numerous sudden deaths of unknown etiology. The presence of B. odocoilei in Canada

Differential detection of human papillomavirus DNA type 6 and 11 amplified by polymerase chain reaction.

We have developed a method for differential detection of human papillomavirus type 6 and 11 with selective amplification of a segment of the viral DNAs by the polymerase chain reaction. The target sequence for amplification was a 134-bp (bp 206-339) in the E6 open reading frame of HPV 6 and 11 DNAs. DNA extracted from a paraffin-section or a minute fresh biopsy specimen of condyloma acuminatum was amplified and hybridized with an oligonucleotide probe specific for HPV 6 or 11 DNA. The method was applied to analyse 5 cases of genital condyloma acuminatum, and as a result, HPV 6 and 11 DNAs were

Cytomegalovirus-Related Gastritis in an Immunocompetent Host Presenting With Infectious Gastroparesis.

Cytomegalovirus (CMV) infection is usually asymptomatic or causes a mild mononucleosis-like syndrome, whereas severe symptoms are rarely reported. We report a case of a 70-year-old woman who was admitted to our center because of severe clinical presentation with anorexia, epigastric pain, nausea, postprandial vomiting, and significant weight loss. Esophagogastroduodenoscopy with biopsies showed ulcerative chronic gastritis with scattered large cells with inclusion bodies. Immunohistochemistry and polymerase chain reaction for CMV-DNA resulted positive. A gastric emptying of solid scintigraphy

Varicella-zoster virus (VZV) multifocal vasculopathy in a patient with systemic lupus erythematosus - a diagnostic and treatment dilemma.

Cerebral vasculopathy due to varicella-zoster virus (VZV) infection is well-documented. We report a fatal case of VZV multifocal vasculopathy in a patient with systemic lupus erythematosus (SLE) who presented with subacute changes in mental status and had multiple areas of hemorrhagic infarcts on brain imaging. However, the correct diagnosis was delayed by several confounding factors including the absence of zoster rash, normal cerebral angiography, persistently low cerebrospinal fluid (CSF) glucose and negative initial polymerase chain reaction (PCR) for VZV DNA in the CSF. Our case and liter

Hepatitis E virus-associated aplastic anaemia: the first case of its kind.

Here we report the case of a 32-year-old Pakistani male, who developed severe aplastic anaemia after a severe attack of acute hepatitis E virus (HEV) infection. His laboratory test values were not in normal ranges. The liver enzymes were elevated. Serologic and/or molecular-based tests for hepatitis A, B, C, D, G, transfusion transmitted virus (TTV) and B19 were negative, whereas anti-HEV IgM and HEV RNA polymerase chain reaction (PCR) was also detected in the patient sample. The patient received immunosuppressive therapy for 6 months; however, he did not show response to this kind of therapy.

Penile, scrotal, and gluteal vesicles.

BACKGROUND: Herpes zoster results from reactivation of the varicella zoster virus (VZV) and most frequently affects the thoracic dermatomes, especially T3 to T5. CASE DESCRIPTION: We present the case of a 61-year-old patient who presented with dysuria and grouped, partly crusted vesicles, some on an erythematous base on the glans penis, scrotum, and gluteal region. Differential diagnoses included herpes virus infection, bacterial balanoposthitis, syphilis, and contact dermatitis. A polymerase chain reaction (PCR) test for VZV was positive, confirming the diagnosis of herpes zoster. CONCLUSIO

Progressive multifocal leukoencephalopathy after natalizumab monotherapy.

We describe progressive multifocal leukoencephalopathy (PML) caused by infection with human polyomavirus JC virus in a patient with multiple sclerosis who was treated with natalizumab. The first PML symptoms appeared after 14 monthly infusions of the drug. Magnetic resonance imaging (MRI) showed a presumed multiple sclerosis lesion, and JC virus DNA was not detected on polymerase-chain-reaction (PCR) assay of cerebrospinal fluid. The patient's symptoms worsened, and the diagnosis of PML was established with a more sensitive quantitative PCR assay after 16 infusions of natalizumab. Plasma excha

Borrelia burgdorferi-associated primary cutaneous marginal-zone B-cell lymphoma: a case report.

An association between Borrelia burgdorferi with primary cutaneous B-cell lymphoma (PCBCL) has long been suspected but just recently, thanks to a polymerase chain reaction technique, it had been possible to demonstrate B. burgdorferi-specific DNA in skin lesions of patients with different PCBCL subtypes. Locating cases of PCBCL that are related to B. burgdorferi infection could be really important for therapeutic implications; in fact, there are several reports of PCBCL responding to antibiotic therapy against B. burgdorferi. We report a case of B. burgdorferi-associated primary cutaneous marg
