Youth Advisors Driving Action: hearing the youth voice in mental health systems of care.

The Children's Mental Health Initiative (CMHI) is funded by the Center for Mental Health Services within the Substance Abuse and Mental Health Services Administration. CMHI assists communities in developing comprehensive, coordinated services for children with serious emotional disturbances and their families. Broadly speaking, these systems are designed to be child centered, youth guided, family driven, community based, and culturally competent. To assure implementation of the "youth-guided" core value in the national evaluation, an advisory group of youth coordinator/youth teams representing

Experiences of parents of children with congenital limb differences with health care providers: a qualitative study.

UNLABELLED: This article describes a qualitative, participatory action research study based in grounded theory, in which an online survey was developed and utilized to explore and generate suggestions for further research about the needs and health care experiences of parents of children with congenital limb differences (CLD) during the first year of the child's life. PARTICIPANTS: Fifty parents completed an online survey that was developed through review of themes in the literature and input from people with CLD and their families. Primarily with open-ended questions, the survey targeted the

Ethnicity and the use of health services in Belize.

Data from the 1991 Belize Family Health Survey show differentials in the use of maternal and child health services between ethnic groups (Creole, Mestizo, Maya/Ketchi and Garifuna). Multivariate analysis is used to explore whether such differentials can truly be attributed to ethnicity or to other characteristics that distinguish the ethnic groups. Health services considered are: family planning, place of delivery (hospital/other), postpartum and newborn check-ups after a birth, and immunisations for children. The language usually spoken in the household is found to be important for interpreti…

What makes a pediatric or young adult patient an appropriate transplant candidate?

A 3-year-old child with chronic granulomatous disease was brought to the transplant clinic by his parents. The patient has a history of Aspergillus fumigatus pneumonia, which required mechanical ventilation, and sepsis, resulting in several intensive care stays. He has failure to thrive and developmental delay. His parents are seeking guidance whether allogeneic hematopoietic cell transplantation (HCT) is a reasonable treatment option given concerns about his upfront major health limitations. Based on the original HCT-Comorbidity Index (CI), this child's risk for nonrelapse mortality (NRM) wou

A review of the intriguing interaction between testosterone and neurocognitive development in males with 47,XXY.

PURPOSE OF REVIEW: Although 47,XXY (Klinefelter syndrome) was first discovered more than 50 years ago, there have been limited comprehensive studies on this disorder. The present review explains the study of neurodevelopmental dysfunction and the impact of testosterone replacement at specific junctions in the life of males with 47,XXY. The intricate relationship between testosterone, neurodevelopment, health, and well being warrants an in-depth investigation in order to achieve optimal outcomes. RECENT FINDINGS: Current literature suggests that the implementation of biological treatment has a

Designing new growth charts for low-birth weight babies: need of the hour in India.

Literature reveals theories explaining low birth weight (LBW) babies' unplanned catch-up growth is related to several non-communicable diseases (NCDs) in adult-life. Is the current growth chart as per World Health Organization child growth standards of healthy breast feed infants and young child is applicable for even LBW or small for gestational age babies? There are high chances that these LBW babies who are growing "in their centiles" will be diagnosed as mild or moderate protein energy malnutrition (PEM) in clinical settings as the current diagnostic criteria for PEM do not include birth w

Sleep disorders in children.

Sleep disorders are common in childhood, and may affect multiple aspects of a child's life and the lives of other family members. A sleep disorder assessment should begin with detailed sleep history and a review of interrelated health issues. Factors contributing to disturbed sleep may be discovered or confirmed by a thorough physical examination. Thereafter, appropriate ancillary testing can provide support for a specific clinical diagnosis. The spectrum of childhood sleep disorders includes OSA, narcolepsy, RLS/PLMD, sleep onset association disorder, and parasomnias. Diagnosing sleep disorde

Caregiving and developmental factors differentiating young at-risk urban children showing resilient versus stress-affected outcomes: a replication and extension.

This study tested hypotheses from an organizational-developmental model for childhood resilience. In this model resilience reflects a child's mastery of age-salient objectives, in the face of substantial adversity, by drawing on internal and external resources that enhance processes of adaptation specific to each developmental stage. Interviews were conducted with parents of 122 7- to 9-year-old urban children exposed to multiple risk factors, 69 classified as resilient and 53 as maladjusted. Consistent with predictions generated by the model: (1) characteristics of a child's caregiving system

Dietary vitamin A intake and the incidence of diarrhea and respiratory infection among Sudanese children.

The relationship of vitamin A deficiency and child survival has been documented in a number of studies but not in others, yet the relationship of vitamin A with child morbidity remains controversial. We prospectively examined the relationship of dietary vitamin A intake and the incidences of diarrhea and respiratory infection among 28,753 Sudanese children between the ages of 6 mo and 6 y. Total dietary vitamin A intake was strongly and inversely associated with the risk of diarrhea (multivariate risk in top relative to bottom quintile = 0.58, 95% confidence interval = 0.47-0.72); we also obse

Psychological health of young adults who experienced early parent death: MMPI trends.

Psychological effects that result from childhood parent loss by death are of substantial clinical and research importance. Parent loss has been linked with various psychopathology, though little is known of its effects on subsequent psychological health (a) as a function of the child's sex; (b) in normal individuals; (c) where early parent separation also has occurred. This study provides data on the Minnesota Multiphasic Personality Inventory-168 (MMPI-168) that address these issues. Ss were normals (16 males and 16 females per group) divided into three groups: Early bereaved (up to age 7), l

A study of an urban health centre: factors influencing contact with mothers and their babies.

Some insight into the use of primary care team was provided by a study of encounters with families with infants. The demand for primary care was large, and varied greatly depending on the circumstances of the family. Using statistical models, five possible influences on contact were examined - the presence of other children in the family, distance from the surgery, the family's socioeconomic grouping, maternal age, and maternal depression. Having more than one child and living close to the surgery increased the likelihood of a home visit by the doctor. There were proportionately more visits by

Co-designing engagement principles for involving children and young people in palliative care research in low- and middle-income settings.

BACKGROUND: Engaging children in research processes is recognised as best practice, as it ensures studies are relevant and responsive to their needs. Limited evidence exists on effective approaches for engaging children with serious illnesses, particularly in palliative care and in resource-limited settings. This study aimed to work with children and young people with serious physical illness to co-design processes and practices for meaningful engagement and involvement in palliative care research within resource-limited settings. METHODS: This qualitative, participatory study used three co-d

Persistent cloaca and partial caudal duplication: a case report.

Persistent cloaca and caudal duplication are 2 rare anomalies of embryogenesis that can present with a wide variety of pelvic malformations. Here we present the rare case of a female born with both abnormalities. The infant was born with a single introitus, an imperforate anus, a didelphys uterus, a duplicated cervix and vagina, and accessory limb and coccyx. Multiple surgeries were performed to correct for the anomalies that would have otherwise had important health and lifestyle consequences for the child.

[A new outlook on adenoid hypertrophy: adenoids or adenoid disease?].

Hypertrophy of the pharyngeal tonsil affects functions of some organs and systems producing negative effects on physical and mental development of the child. The existing classification based of the tonsil size does not reflect the complexity of the pathological process. "Adenoid disease" is thought to be a more adequate term. A new classification of adenoid disease is proposed. Indications for conservative and surgical treatments can be well formulated with the new classification. The authors prefer adenotomy as, in their opinion, adenectomy is not safe for health.

Custody and visitation. Problems and perspectives.

Child custody and visitation remain vexing problems requiring input from mental health professionals as public policy decisions are formulated. Difficult problems do not have easy solutions, but through continued exploration of the issues using clinical and research methods, perhaps innovative programs will be developed and evaluated. Ultimately, this may lead to better ways of helping families cope with the trauma and the consequences of the family break-up and the resulting attempt to establish custody and visitation.

Changing epidemiology, implications, and recommendations for hepatitis C in women of childbearing age and during pregnancy.

Despite the remarkable advances in HCV treatment brought about by the advent of direct-acting antivirals, HCV remains a global public health concern. One particular concern relates to the rising prevalence of HCV in women of childbearing age. Active HCV during pregnancy is associated with cholestasis of pregnancy as well as the risk of mother-to-child transmission. Guidelines are increasingly recommending universal screening during pregnancy, while the treatment of HCV during pregnancy is an area of ongoing research.
