We report a 13-year-old Indian boy with nephrotic syndrome caused by renal AA amyloidosis. Workup of the AA amyloidosis revealed chronic hepatitis B. Laser microdissection of the Congo-red-positive glomeruli and vessels followed by liquid chromatography and tandem mass spectrometry confirmed the presence of serum amyloid A (SAA) protein and ruled out hereditary and familial amyloidosis. Furthermore, mass spectrometry also detected a variant of SAA protein (SAA W71R).

A case of markedly elevated 24-hour urine-free cortisol levels, mainly due to interference with carbamazepine when measuring with high performance liquid chromatography but also due to increased fluid intake, is presented. Moreover, the 1 mg overnight dexamethasone suppression test was positive and the midnight serum cortisol level was slightly increased, probably as a result of carbamazepine-induced activity of CYP3A4 and increased levels of cortisol binding globulin.

Here, we described the expression and characterization of the recombinant toxin LTx2, which was previously isolated from the venomous cDNA library of a Brazilian spider, Lasiodora sp. (Mygalomorphae, Theraphosidae). The recombinant toxin found in the soluble and insoluble fractions was purified by reverse phase high-performance liquid chromatography (HPLC). Ca2+ imaging analysis revealed that the recombinant LTx2 acts on calcium channels of BC3H1 cells, blocking L-type calcium channels.

Ninety-two cerebrospinal fluids were analyzed by a simple gas-liquid chromatography method for lactic acid. The presence of greater than 30 mg/dl of lactic acid correlated with proven bacterial or cryptococcal meningitis. Elevated lactate levels were also found in two patients with suspected but not proven bacterial meningitis. In 78 patients who did not have bacterial meningitis the cerebrospinal fluid contained less than 30 mg/dl of lactic acid.
