Optimizing risk-reducing surgery and aspirin decision aids for Lynch syndrome carriers using the person-based approach: A think-aloud interview study.

Lynch syndrome "carriers" carry a germline pathogenic variant conferring gene-, sex-, and organ-specific increased cancer risks. They are presented with difficult, interrelated choices over their lifetime. This study was part of a larger project to codesign a health intervention, Lynch Choices™ https://canchoose.org.uk to provide an information hub and decision support for carriers, their family members, and clinicians. This study aimed to answer the research question: What content, framing, and design elements of a decision aid for genetic cancer risk management are important to Lynch syndrom

An individualized patient-reported outcome measure (PROM) based patient decision aid and surgeon report for patients considering total knee arthroplasty: protocol for a pragmatic randomized controlled trial.

BACKGROUND: While the rates of total knee arthroplasty (TKA) continue to rise worldwide, there are concerns about whether all surgeries are appropriate. Guidelines for appropriateness suggest that patients should have realistic expectations for total knee arthroplasty (TKA), and that the patient and their surgeon should agree that the potential benefits outweigh the potential harms. The objective of this study is to evaluate whether routinely collected pre- and post-TKA patient-reported outcome measures (PROMs) could be integrated into a patient decision aid to better inform these appropriaten

Healthcare provider perspectives on their role in perpetrating and perpetuating reproductive coercion: a qualitative study.

BACKGROUND: Reproductive coercion (RC) refers to behaviours that interfere with an individual's autonomy over their reproductive health and decision-making. While traditionally attributed to partners or families, emerging research has highlighted the potential role healthcare providers may play in perpetrating and perpetuating RC. This study aims to explore the perspectives of Australian healthcare providers who deliver reproductive healthcare, focusing on their understanding of provider bias and coercion in the context of reproductive decision-making, both within their own practice and among

Complex Limb Injuries: Limb Salvage Versus Amputation-A Mini Review and Meta-Analysis.

Introduction: The management of complex limb injuries can be very challenging, and it demands a multidisciplinary approach to treatment. Amputation and limb reconstruction are the two options that clinicians must choose. This study aims to comprehensively synthesize existing tools and resources from the literature that can assist clinicians in the decision-making process. Evaluation: The initial resuscitation and the prehospital care are the first important steps in the management of these injuries, while the immediate transfer to trauma centers is recommended for complex cases. After the stab

Spinal Muscular Atrophy Update in Best Practices: Recommendations for Treatment Considerations.

BACKGROUND AND OBJECTIVES: Spinal muscular atrophy (SMA) is an autosomal recessive disorder caused by biallelic variants of the Survival Motor Neuron 1 gene (SMN1) that affects approximately 1 in 15,000 live births. Availability of 3 SMN-enhancing treatments for SMA has led to urgency to review how clinicians and patients use these treatments for SMA, while additional research and real-world data and experience are being collected. This work describes important factors to assist with decision-making for SMN-enhancing treatments. METHODS: A systematic literature review was conducted on SMN-enh

Medical and ethical considerations in twin pregnancies discordant for serious cardiac disease.

OBJECTIVE: Prenatal ultrasound has led to the early diagnosis of major anomalies. However, the ready availability of this technology has led to increasing challenges for physicians counseling affected parents, which is all the more difficult in a twin pregnancy with only one affected fetus. This paper reviews the medical and ethical considerations in twin pregnancies discordant for a serious cardiac condition. STUDY DESIGN: Six recent twin pregnancies discordant for a serious cardiac condition and their outcomes are presented. Options considered in the management of the pregnancy were to cont

Nonsize Criteria for Surgical Intervention on the Ascending Thoracic Aorta.

For decades, aortic surgery has relied on size criteria for intervention on the ascending aorta. While diameter has served well, diameter alone falls short of an ideal criterion. Herein, we examine the potential application of other, nondiameter criteria in aortic decision-making. These findings are summarized in this review. We have conducted multiple investigations of specific alternate nonsize criteria by leveraging our extensive database, which includes complete, verified anatomic, clinical, and mortality data on 2,501 patients with thoracic aortic aneurysm (TAA) and dissections (198 Type

The role of blood nitric oxide level in predicting return of spontaneous circulation: a prospective case-control study.

OBJECTIVE: The aim of this study was to investigate whether there is a difference in serum nitric oxide levels between patients who return spontaneously after cardiopulmonary resuscitation and those who do not. We also examined the potential of using serum nitric oxide levels as a marker to make an accurate decision about patient survival. METHODS: We included 100 consecutive patients who were brought to the emergency clinic due to cardiac arrest. Blood samples were taken from these patients at admission, 30 min after admission, and when resuscitation was terminated. RESULTS: We found that t

A randomized controlled trial to evaluate innovative decision support in the context of fall prevention.

Falls are a major cause of injuries among older people, with medication being a key risk factor. The SNOWDROP intervention introduces a clinical decision support system for general practitioners (GPs) offering personalized deprescribing advice, and a patient portal containing information and a question prompt list. This study evaluates the intervention's effectiveness through a cluster randomized controlled trial in six general practices, with 84 patients (Mage = 78.01, SDage = 5.71). Patients discussed their medication-related fall risk with their GP. Data were collected via questionnaires an

Induction of labour in low-risk pregnancies before 40 weeks of gestation: A systematic review and meta-analysis of randomized trials.

This systematic review of 16 randomized trials, which included 8796 pregnancies, did not demonstrate a reduction in caesarean births or perinatal morbidity from induction of labour (IoL) at 39-40 weeks for non-medical indications. However, IoL at 39-40 weeks may be associated with a lower incidence of maternal hypertensive disorders, a shorter duration of the first stage of labour, fewer instances of meconium-stained amniotic fluid, lower mean birth weights, longer duration of (maternal) hospitalization, and higher epidural usage. Until regional data on clinical benefits, resource implications

Consensus Decision-Making for the Management of Antiplatelet Therapy before Non-Cardiac Surgery in Patients Who Underwent Percutaneous Coronary Intervention With Second-Generation Drug-Eluting Stents: A Cohort Study.

Background Although antiplatelet therapy (APT) has been recommended to balance ischemic-bleeding risks, it has been left to an individualized decision-making based on physicians' perspectives before non-cardiac surgery. The study aimed to assess the advantages of a consensus among physicians, surgeons, and anesthesiologists on continuation and regimen of preoperative APT in patients with coronary drug-eluting stents. Methods and Results A total of 3582 adult patients undergoing non-cardiac surgery after percutaneous coronary intervention with second-generation stents was retrospectively includ

Current progress and controversies in thoracic ossification of the ligamentum flavum: pathogenesis, diagnosis and surgical management.

Thoracic ossification of ligamentum flavum (TOLF) is the primary pathological driver of thoracic spinal stenosis (TSS), characterized by insidious clinical onset, high surgical complexity, and a elevated risk of permanent neurological deficits. This systematic review synthesizes the epidemiological landscape and genetic, mechanical, and inflammatory pathogeneses ofTOLF. We provide a comprehensive evaluation of current radiological classification systems and critically appraise different decompressive procedures-ranging from classic open laminectomies to emerging minimally invasive biportal and

Patient experience data as a methodological and strategic innovation in clinical trial design.

Patient experience data (PED) are increasingly recognised as a methodological and strategic innovation in drug development, yet their implementation in clinical trial design remains inconsistent. Drawing on a structured literature review and multistakeholder interviews, this analysis examines how PED inform endpoint decision-making. Although clinical development is shifting towards more patient-informed measures, challenges persist due to limited standardisation, validation, methodological gaps and operational constraints. Opportunities include early patient involvement, validated disease-spec
