Leadership behaviors for successful university--community collaborations to change curricula.

PURPOSE: What constitutes effective leadership in a collaborative effort to achieve enduring curricular and student career changes? This question was investigated as part of a larger evaluation of the W. K. Kellogg Foundation's Community Partnership Health Professions Education, a five-year initiative involving projects at seven sites. The goal was to produce more primary care health providers by making enduring curricular change. METHOD: Data were collected from participants with respect to predictors of project success and leaders' use of 16 behaviors via telephone interviews, mailed survey

An ecological paradigm for a health behavior analysis of "konzo", a paralytic disease of Zaire from toxic cassava.

Konzo is an irreversible paralytic disease afflicting tens of thousands of women and children in rural Zaire and throughout sub-Sahara Africa. The disease can occur where bitter, high-yield varieties of cassava that thrive in arid soils provide the basic nutritional staple. The paraparesis is related to upper motor neuron damage stemming from the consumption of insufficiently processed toxic cassava roots (manioc) and a diet poor in the sulfur-based amino acids necessary for the body to detoxify the cyanide in this plant. The ecological paradigm [Kelly (1968) Toward an ecological conception of

The Gospel Oak project 1987-1990: provision and use of community services.

This longitudinal survey of elderly people examines morbidity and service usage. 705 elderly people who lived in their own homes were interviewed as part of the Gospel Oak project in 1987/88, and in 1990, 524 (74.3%) were re-interviewed--90 (12.8%) had died, 51 (7.2%) had moved and 40 (5.7%) refused a follow-up interview. The results indicate that 60% of residents (1987/88 and 1990) had been in contact with community services within the month before the survey. Respondents with depression were mostly in contact with the health service, those with dementia were in contact with social services.

World Gastroenterology Organisation Practice Guidelines.

Significant progress on hepatitis B treatment pathways has been made since the last World Gastroenterology Organisation (WGO) guidelines in 2015. Global elimination targets have been set, prioritizing the prevention of new infections and optimizing screening and linkage to care in resource-limited settings. This has been led by the World Health Organization (WHO), which first published hepatitis B treatment and care guidelines in 2015 and updated its recommendations in 2024. The critical importance of preventing new infections was supported in 2020 by the WHO policy brief guidelines on antivir

Effects of a device-measured physical activity intervention on visual outcomes, including contrast sensitivity, and glycaemic control in individuals with abnormal glucose tolerance: a pilot randomised trial post-CarMeR study.

BACKGROUND: Abnormal glucose tolerance (AGT), a summary term for prediabetes and diabetes, is a significant public health concern in sub-Saharan Africa, where diabetes-related complications like retinopathy are underdiagnosed. Lifestyle interventions, specifically physical activity, offer a cost-effective approach to improve glycaemic and visual outcomes in resource-limited settings. However, feasibility data on the effectiveness of these interventions in low-resource settings are limited. This pilot randomised controlled trial (RCT) assessed the feasibility of a 12-week device-measured physic

Development of an Interpretable Triage Tool for Colorectal Polyp Risk Stratification Within a Population-Based Screening Program: Machine Learning Approach.

BACKGROUND: Colorectal polyps are a major source of precancerous lesions in colorectal cancer (CRC). In many population-based screening programs, a major challenge is the efficient triage of high-risk individuals for diagnostic colonoscopy amid limited endoscopic resources. OBJECTIVE: To enrich current screening frameworks, we aimed to develop an accessible, noninvasive risk stratification tool to serve as a digital triage mechanism for colorectal polyps using machine learning (ML) and routinely collected data in China. METHODS: We conducted a cross-sectional study in Wenzhou, China. A total

Severe Megaloblastic Anemia Secondary to Folate Deficiency in a 2-Year-Old Boy with Prolonged Predominant Goat Milk Feeding: A Case Report from a Low-Resource Setting.

INTRODUCTION: Megaloblastic anemia in young children is a significant but preventable condition, commonly caused by deficiencies in folate or vitamin B12. Prolonged predominant feeding with nutritionally inadequate milk substitutes, such as unfortified goat milk, increases the risk of severe nutritional anemia, particularly in low-resource settings. CASE PRESENTATION: We report a 2-year-old Somali boy who presented with a 5-month history of fatigue, recurrent fever, poor appetite, abdominal distension, progressive weight loss, and cough. The child had a history of prolonged predominant goat m

Towards unified global action on ultra-processed foods: understanding commercial determinants, countering corporate power, and mobilising a public health response.

The rise of ultra-processed foods (UPFs) in human diets is harming global public health. However, policy responses are still emerging-much like tobacco control efforts decades ago-indicating the need to understand root causes and accelerate global action. This paper, the third in a three-part Lancet Series, takes several steps to advance knowledge of these causes, and to inform a global public health response. First, we show that the UPF industry is a key driver of the problem, as its leading corporations and co-dependent actors have expanded and restructured food systems almost everywhere, in

A full response chain surge capacity test of a small rural hospital, prehospital resources and collaborating organisations.

BACKGROUND: Increased surge capacity is key in mass casualty incidents. Rural hospitals face other challenges in terms of transport capacity and available resources. The aim was to examine if a simulation system previously used to test surge capacity at large hospitals, could be used to test surge capacity at a small rural hospital. METHOD: A qualitative study was conducted to assess surge capacity at a small rural hospital using a previously validated simulation system. The simulation system was adopted to the Norwegian trauma system and local context. New simulated patient cards were develo

The Canadian VirusSeq Data Portal and Duotang: open resources for SARS-CoV-2 viral sequences and genomic epidemiology.

The COVID-19 pandemic led to a large global effort to sequence SARS-CoV-2 genomes from patient samples to track viral evolution and inform the public health response. Millions of SARS-CoV-2 genome sequences have been deposited in global public repositories. The Canadian COVID-19 Genomics Network (CanCOGeN - VirusSeq), a consortium tasked with coordinating expanded sequencing of SARS-CoV-2 genomes across Canada early in the pandemic, created the Canadian VirusSeq Data Portal, with associated data pipelines and procedures, to support these efforts. The goal of VirusSeq was to allow open access t

Automatic offline-capable smartphone paper-based microfluidic device for efficient biomarker detection of Alzheimer's disease.

BACKGROUND: Alzheimer's disease (AD) is a prevalent neurodegenerative disease with no effective treatment. Efficient and rapid detection plays a crucial role in mitigating and managing AD progression. Deep learning-assisted smartphone-based microfluidic paper analysis devices (μPADs) offer the advantages of low cost, good sensitivity, and rapid detection, providing a strategic pathway to address large-scale disease screening in resource-limited areas. However, existing smartphone-based detection platforms usually rely on large devices or cloud servers for data transfer and processing. Addition

"They are gaining experience; we are gaining extra hands": a mixed methods study to assess healthcare worker perceptions of a novel strategy to strengthen human resources for HIV in South Africa.

BACKGROUND: Lay health workers (LHWs) can support the HIV response by bridging gaps in human resources for health. Innovative strategies are needed to expand LHW programs in many low- and middle-income countries. Youth Health Africa (YHA) is a novel LHW approach implemented in South Africa that places young adults needing work experience in one-year non-clinical internships at health facilities to support HIV programs (e.g., as HIV testers, data clerks). While research suggests YHA can increase HIV service delivery, we need to understand healthcare worker perceptions to know if this is an acce

Real-world use of chemotherapy for Kaposi's sarcoma in a large community-based HIV primary care system in Kenya.

BACKGROUND: Kaposi's sarcoma (KS) is one of the most common HIV-associated malignancies in sub-Saharan Africa. Worldwide, the availability of antiretroviral therapy (ART) has improved KS survival. In resource-rich settings, survival has also benefited from chemotherapy, which is widely available. Little is known, however, about the epidemiology of chemotherapy use for HIV-associated KS in resource-limited regions such as sub-Saharan Africa. METHODS: We identified all patients newly diagnosed with HIV-related KS from 2009 to 2012 in the 26-clinic AMPATH network, a large community-based care ne

Evaluation of a Custom-Developed Computer Game to Improve Executive Functioning in 4- to 6-Year-Old Children Exposed to Alcohol in Utero: Protocol for a Feasibility Randomized Controlled Trial.

BACKGROUND: Fetal alcohol spectrum disorder (FASD) is one of the most common causes of preventable intellectual disability, and the key associated deficits are in executive function (EF). Aspects of EF can be improved using cognitive training interventions. The highest prevalence of FASD globally (at a rate of 135.1 per 1000) has been found in a South African population in the Western Cape province. There is a shortage of specialized health service personnel, and there are limited remedial services. Computer-based cognitive training, if age and culturally appropriate, could be an effective way

Patients experiences of self-management and strategies for dealing with chronic conditions in rural Malawi.

BACKGROUND: The high burden of chronic communicable diseases such as HIV/AIDS, and an escalating rise of non-communicable diseases (NCDs) in Malawi and other sub-Saharan African countries, calls for a shift in how health care services are designed and delivered. Patient-centred care and patient self-management are critical elements in chronic care, and are advocated as universal strategies. In sub-Saharan Africa, there is need for more evidence around the practice of patient self-management, and how to best support patients with chronic conditions in the African context. Our study explored sel

Challenges of controlling sleeping sickness in areas of violent conflict: experience in the Democratic Republic of Congo.

BACKGROUND: Human African trypanosomiasis (HAT), or sleeping sickness, is a fatal neglected tropical disease if left untreated. HAT primarily affects people living in rural sub-Saharan Africa, often in regions afflicted by violent conflict. Screening and treatment of HAT is complex and resource-intensive, and especially difficult in insecure, resource-constrained settings. The country with the highest endemicity of HAT is the Democratic Republic of Congo (DRC), which has a number of foci of high disease prevalence. We present here the challenges of carrying out HAT control programmes in genera
