Hemodynamically stable children with duodenal obstruction caused by retroperitoneal hematoma after a seat belt injury can be successfully managed conservatively. Careful clinical and imaging monitoring allows nonoperative treatment and may help avoid complications associated with surgical intervention.

Severe acute pancreatitis (SAP) is a life-threatening inflammatory condition that can occasionally result in rare and devastating extra-pancreatic complications. Among these, gastric or duodenal wall necrosis is exceptionally uncommon but carries a high risk of mortality. We report the case of a 56-year-old man with idiopathic SAP who initially improved with conservative management. After systematic exclusion of common etiologies, including biliary, alcoholic, metabolic, infectious, and drug-induced causes, the pancreatitis was classified as idiopathic. Three weeks after admission, he develope

BACKGROUND: Bevacizumab-induced gastrointestinal perforation is a potentially fatal adverse event. However, duodenal perforations are rare. Conversely, patients with cancer often have concomitant rheumatoid arthritis and may be treated with a combination of anti-rheumatic drugs and symptomatic medications; however, the effects of these concomitant medications on bevacizumab-related perforation remain understudied. This report describes the case of a patient with ovarian cancer and a history of anti-rheumatic drug treatment who developed early-onset duodenal perforation during bevacizumab-conta

BACKGROUND AND STUDY AIMS: It is currently unclear whether obstruction location affects clinical and procedure outcomes after endoscopic ultrasonography-guided gastroenterostomy (EUS-GE) with a lumen-apposing metal stent (LAMS) in patients with malignant gastric outlet obstruction (GOO). Therefore, we compared clinical outcomes of EUS-GE for malignant GOO located in the stomach with obstruction located in the duodenum.
PATIENTS AND METHODS: In this nationwide, multicenter, retrospective study, we included consecutive patients who underwent EUS-GE as palliative treatment for malignant GOO. Mai

BACKGROUND: Recent work suggests an altered duodenal mucosa-associated microbiota (d-MAM) in patients with functional dyspepsia (FD) when compared to controls. This may reflect alterations in host-microbiome homeostasis. Given the specific mucosal immune signatures identified in FD, we hypothesize that these signatures are associated with specific microbial changes. We aim to profile the d-MAM to identify microbes associated with known changes in FD mucosal and peripheral immunity.
METHODS: Upper gastrointestinal biopsies were collected from 11 outpatient controls and 17 FD patients. Specific

Congenital duodenal obstruction (CDO) is a recognised anomaly frequently associated with Down syndrome (DS). While complete obstruction is typically diagnosed in the neonatal period, incomplete obstruction may present with milder, non-specific symptoms, often delaying diagnosis. We report a boy in early adolescence with DS and autism spectrum disorder (ASD) in whom CDO was diagnosed in adolescence. Since early childhood, his eating behaviour was restricted to finely chopped foods, initially interpreted as a behavioural feature related to DS and ASD. This likely masked gastrointestinal symptoms

BACKGROUND: Strongyloides stercoralis is an intestinal nematode with a unique autoinfective life cycle that enables decades-long persistence in the human host. Its gastrointestinal manifestations may resemble inflammatory bowel disease (IBD), creating a clinically important diagnostic pitfall. Misdiagnosis is particularly hazardous because corticosteroids or other immunosuppressive therapies may precipitate Strongyloides hyperinfection syndrome or disseminated disease.

INTRODUCTION: Upper gastrointestinal bleeding (UGIB) remains a critical clinical challenge, with peptic ulcer disease accounting for up to two-thirds of cases. While most bleeding duodenal ulcers respond to endoscopic therapy, refractory hemorrhage, particularly in the presence of arterial involvement or vascular anomalies, may necessitate escalation to interventional radiology (IR) or surgery.
CASE PRESENTATION: We present the case of a 64-year-old woman with a bleeding duodenal ulcer arising from an aberrant branch of the superior mesenteric artery in the setting of severe celiac artery ste

Perforated duodenal ulcers represent a surgical emergency requiring prompt operative intervention. The Graham omental patch is the most commonly performed procedure for emergent repair; however, postoperative findings suggestive of leakage remain a serious concern and are associated with significant morbidity and mortality. Traditionally, suspected postoperative leaks have prompted early surgical re-exploration due to the risk of peritonitis and sepsis. However, select patients with concerning postoperative findings may be managed successfully with appropriate drainage, decompression, and nutr

BACKGROUND AND AIMS: A 69-year-old male patient with a previous duodenal switch surgery presented with choledocholithiasis and cholangitis. Initially, interventional radiology (IR) performed biliary stent placement with a metal stent left in situ. One year later, the patient presented again with cholangitis, now requiring definitive treatment via EUS-directed transduodenal ERCP (EDDE).
METHODS: An anastomosis was created between the first portion (superior portion) of the duodenum (D1) and the excluded second portion (descending portion) of the duodenum (D2) using a lumen-apposing metal stent

INTRODUCTION: Duodenal duplication is a rare congenital anomaly with nonspecific symptoms, and preoperative diagnosis is often difficult. Complete resection is recommended, but lesions adjacent to the bile or pancreatic duct are challenging to treat safely in pediatric patients. We report a pediatric case of duodenal duplication successfully managed by laparoscopic mucosal resection with selective full-thickness dissection under intraoperative ultrasonographic guidance.

BACKGROUND AND AIMS: Small intestinal bacterial (SIBO) or fungal overgrowth (SIFO) involves excessive microbial growth in the small intestine. While jejunal aspirate is the gold standard for diagnosis, duodenal aspiration is easier to perform. This study determined and compared the diagnostic yield of tandemly performed duodenal and jejunal aspirates and cultures.
METHODS: Patients with gas and bloating symptoms and suspected SIBO/SIFO underwent enteroscopy, during which duodenal and jejunal aspirates were sequentially collected using a 2 mm Liguory catheter under aseptic conditions. Cultures

BACKGROUND: Endoscopic papillectomy (EP) via endoscopic retrograde cholangiopancreatography has emerged as a less invasive alternative to surgery for duodenal papillary adenomas (DPAs), which is traditionally associated with notable postoperative risks.
AIM: To compare quality of life (QoL) and outcomes between DPA patients undergoing EP vs surgical resection, and to assess the influencing factors of QoL and complications.
METHODS: We conducted a retrospective, single-center analysis involving patients treated for DPA at the Drum Tower Hospital of Nanjing University Medical School from 2011

AIMS: Duodenal Mucosal Resurfacing (DMR) is an endoscopic ablation technique aimed at improving glycemia in patients with type 2 diabetes mellitus (T2DM). Although the exact underlying mechanism is still unclear, it is postulated that the DMR-induced improvements are the result of changes in the duodenal mucosa. For this reason, we assessed macroscopic and microscopic changes in the duodenal mucosa induced by DMR + GLP-1RA.
METHODS: We included 16 patients with T2DM using basal insulin that received a combination treatment of a single DMR and GLP-1RA. Endoscopic evaluation was performed befor

BACKGROUND: The complex interactions between host genetics and the gut microbiome are well documented. However, the specific impacts of gene expression patterns and microbial composition on each other remain to be further explored.
RESULTS: Here, we investigated this complex interplay in a sizable population of 705 hens, employing integrative analyses to examine the relationships among the host genome, mucosal gene expression, and gut microbiota. Specific microbial taxa, such as the cecal family Christensenellaceae, which showed a heritability of 0.365, were strongly correlated with host geno

Non-traumatic duodenal perforation is a rare and potentially life-threatening condition, typically caused by peptic ulcer disease, and traditionally requires urgent surgical intervention. However, in resource-limited settings, conservative management can be an effective alternative for selected patients, particularly those with localized contamination and comorbidities that make surgery high-risk. This case report describes the successful non-operative management of a 78-year-old woman with poorly controlled type 2 diabetes, hypertension, dyslipidemia, and bronchial asthma, who presented with

The upper oesophagogastrointestinal (UEGI) tract histology, intestinal morphometry and lymphocyte subpopulations of healthy people is scarcely known. In research studies of inflammation involving the UEGI tract, there is a lack of adequate healthy controls. Aims: To evaluate the histology of the UEGI tract and the duodenal lymphocyte subpopulations of healthy volunteers and patients with gastroesophageal reflux disease (GERD), the latter to assess if it could replace healthy subjects. Healthy individuals were excluded if they had symptoms, comorbidities, pregnancy, toxics, medications or abnor

Duodenal perforations are rare but critical conditions that often present with significant morbidity and mortality. These injuries may result from peptic ulcer disease, trauma, or foreign bodies, with posterior perforations posing particular challenges due to their retroperitoneal location. Such perforations may lead to complications, including retroperitoneal abscesses, necrotizing pancreatitis, and fistula. The management of these cases often requires a multidisciplinary approach. This case report describes a highly unusual instance of a complicated posterior duodenal perforation in a 76-yea
