Vagus nerve stimulation for pediatric patients with intractable epilepsy between 3 and 6 years of age: study protocol for a double-blind, randomized control trial.

BACKGROUND: Recent clinical observations have reported the potential benefit of vagus nerve stimulation (VNS) as an adjunctive therapy for pediatric epilepsy. Preliminary evidence suggests that VNS treatment is effective for seizure reduction and mental development in young participants between 3 and 6 years of age who suffer from intractable epilepsy. However, robust clinical evidence for quantifying the difference of the efficacy and safety of VNS treatment in this specific patient population has yet to be reported. METHODS/DESIGN: A two-armed, multicenter, randomized, double-blind, prospec

Flexible Multielectrode Array for Skeletal Muscle Conditioning, Acetylcholine Receptor Stabilization and Epimysial Recording After Critical Peripheral Nerve Injury.

Complete re-innervation after a traumatic injury severing a muscle's peripheral nerve may take years. During this time, the denervated muscle atrophies and loses acetylcholine receptors, a vital component of the neuromuscular junction, limiting functional recovery. One common clinical treatment for atrophy is electrical stimulation; however, epimysial electrodes currently used are bulky and often fail due to an excessive inflammatory response. Additionally, there remains a need for a device providing in vivo monitoring of neuromuscular regeneration and the maintenance of acetylcholine receptor

Increased O2 consumption in excitation-contraction coupling in hypertrophied rat heart slices related to increased Na+ -Ca2+ exchange activity.

The goal of our study was to evaluate the origin of the increased O(2) consumption in electrically stimulated left ventricular slices of isoproterenol-induced hypertrophied rat hearts with normal left ventricular pressure. O(2) consumption per minute (mVO(2)) of mechanically unloaded left ventricular slices was measured in the absence and presence of 1-Hz field stimulation. Basal metabolic mVO(2), i.e., mVO(2) without electrical stimulation, was significantly smaller, but mVO(2) for the total Ca(2+) handling in excitation-contraction coupling (E-C coupling mVO(2)), i.e., delta mVO(2) (=mVO(2)

[Influence of hemisphere lesions on the contralateral blink reflex].

Electrically elicited blink reflexes were investigated in 28 patients with unilateral hemispheral lesion. The reflex evoked by electrical stimulation of the supraorbital nerve contralateral to the side of the hemispheral lesion was absent or depressed in 14 out of 28 cases. The abnormal patterns of the blink reflexes were devided into four different types (Type AB, Type AD, Type EB, and Type AB-EB). The Type AB, recorded in 4 cases, consisted of bilateral absence of late component (R2) when the supraorbital nerve on the non-lesion side was stimulated. In contrast, the reflex responses were alm

[Electrophysiology of the atrioventricular node during atrial fibrillation. I. Ventricular rhythm].

Atrial fibrillation (AF) occurs in 0.9% of the population, in 6% of persons over 65 and in 10% of persons over 80. It is an important independent risk factor for thromboembolism, especially cerebral infarctions. The functions of the atrioventricular (AV) node are: (a) optimal adjustment of the time between the contractions of atria and ventricles; (b) protection of the ventricles against excessively high frequencies of atrial tachycardia; (c) a pacemaker function in case of atrial arrest. AF is an irregular, disorganized electrical activity of the atria. On the ECG, P waves are absent and the

Electromagnetic field induced activation of amyloid-β degrading enzyme, neprilysin, for accelerated Alzheimer's disease therapy.

BackgroundAmyloid-β accumulation and misfolding are key features of Alzheimer's disease (AD), contributing to synaptic dysfunction, neuroinflammation, and neuronal loss. Normally, amyloid-β is cleared by chaperone-assisted folding, enzymes such as neprilysin and insulin-degrading enzyme, and autophagy pathways. In AD, these mechanisms become impaired, leading to toxic aggregation. Enhancing the activity of endogenous degrading enzymes offers a promising therapeutic approach, with electromagnetic (EM) fields emerging as a potential noninvasive method to promote amyloid-β clearance.ObjectiveThis

How Standardized Is Transcranial Magnetic Stimulation Treatment for Depression? Large-Cohort Modeling Reveals Systematic Dosimetric Variability.

INTRODUCTION: Repetitive transcranial magnetic stimulation (rTMS) is a widely utilized, noninvasive brain stimulation technique for treating neurological and psychiatric disorders worldwide. The efficacy of rTMS is influenced by individual head morphology, which shapes the induced electric field (E-field) in the brain. The standard practice for dosing rTMS involves adjusting the intensity to a defined percentage of the motor threshold (MT), a method exemplified by the globally adopted, US FDA-approved protocol of using 120% MT for depression. Since head morphology varies systematically across

Percutaneous auricular neuromodulation to treat pain after ambulatory breast surgery: A randomized, double-masked, sham-controlled pilot study.

INTRODUCTION: Percutaneous auricular neuromodulation involves implanting electrodes around the ear and administering an electric current. A device is currently available that is cleared to treat symptoms from opioid withdrawal, and multiple reports suggest a possible postoperative analgesic effect. This randomized, controlled pilot study aimed to (1) assess the feasibility of a postoperative auricular neuromodulation protocol and (2) provide an estimate of its treatment effects on postoperative pain and opioid consumption. METHODS: Adults undergoing unilateral or bilateral ambulatory breast s

Precise mimicry of physiological Ca2+ oscillations for mammalian oocyte activation by nanosecond pulsed electric field.

Oocyte activation deficiency is a primary cause of fertilization failure following intracytoplasmic sperm injection, a problem that can potentially be overcome through artificial oocyte activation (AOA). However, concerns persist regarding the safety and efficacy of AOA in clinical practice. We demonstrated that single-pulse nanosecond pulsed electric field (nsPEF) stimulation induced Ca2+ signaling patterns that depend on intensity in both mouse and human oocytes, facilitating parthenogenetic activation and blastocyst formation. The sperm-initiated physiological Ca2+ oscillations were effecti

Darts fast-learning reduces theta power but is not affected by Hf-tRNS: A behavioral and electrophysiological investigation.

Sports trainers have recently shown increasing interest in innovative methods, including transcranial electric stimulation, to enhance motor performance and boost the acquisition of new skills during training. However, studies on the effectiveness of these tools on fast visuomotor learning and brain activity are still limited. In this randomized single-blind, sham-controlled, between-subjects study, we investigated whether a single training session, either coupled or not with 2 mA online high-frequency transcranial random noise stimulation (hf-tRNS) over the bilateral primary motor cortex (M1)…

Tracking the current in the Alzheimer's brain - Systematic differences between patients and healthy controls in the electric field induced by tDCS.

BACKGROUND: Several studies on patients with Alzheimer's disease (AD) have used transcranial direct current stimulation (tDCS) to enhance neural excitability in the left dorsolateral prefrontal cortex (lDLPFC). Interindividual differences in brain anatomy in AD patients pose a challenge to efficiently target the lDLPFC using scalp-based coordinates, calling for new and more precise tDCS protocols. OBJECTIVE: The purpose of this study was to explore how AD-related neuropathology affects the tDCS-induced electric field (EF) across different DLPFC montages using computational modeling. METHOD:

Individual differences in neuroanatomy and neurophysiology predict effects of transcranial alternating current stimulation.

BACKGROUND: Noninvasive transcranial electrical stimulation (tES) research has been plagued with inconsistent effects. Recent work has suggested neuroanatomical and neurophysiological variability may alter tES efficacy. However, direct evidence is limited. OBJECTIVE: We have previously replicated effects of transcranial alternating current stimulation (tACS) on improving multitasking ability in young adults. Here, we attempt to assess whether these stimulation parameters have comparable effects in older adults (aged 60-80 years), which is a population known to have greater variability in neur

TGR5-dependent hepatoprotection through the regulation of biliary epithelium barrier function.

OBJECTIVE: We explored the hypothesis that TGR5, the bile acid (BA) G-protein-coupled receptor highly expressed in biliary epithelial cells, protects the liver against BA overload through the regulation of biliary epithelium permeability. DESIGN: Experiments were performed under basal and TGR5 agonist treatment. In vitro transepithelial electric resistance (TER) and FITC-dextran diffusion were measured in different cell lines. In vivo FITC-dextran was injected in the gallbladder (GB) lumen and traced in plasma. Tight junction proteins and TGR5-induced signalling were investigated in vitro and

Giant early components of somatosensory evoked potentials to tibial nerve stimulation in cortical myoclonus.

Enlarged cortical components of somatosensory evoked potentials (giant SEPs) recorded by electroencephalography (EEG) and abnormal somatosensory evoked magnetic fields (SEFs) recorded by magnetoencephalography (MEG) are observed in the majority of patients with cortical myoclonus (CM). Studies on simultaneous recordings of SEPs and SEFs showed that generator mechanism of giant SEPs involves both primary sensory and motor cortices. However the generator sources of giant SEPs have not been fully understood as only one report describes clearly giant SEPs following lower limb stimulation. In our s

Regulation of insulin secretion by cAMP in rat islets of Langerhans permeabilised by high-voltage discharge.

Adenosine 3',5-cyclic monophosphate (cAMP) was shown to stimulate insulin secretion from electrically permeabilised islets of Langerhans incubated in Ca2+/EGTA buffers. cAMP-induced insulin secretion occurred in the presence of either sub-stimulatory (50 nM) or stimulatory (greater than 100 nM) concentrations of Ca2+. Similar effects on secretion were obtained in response to 8-bromo-cAMP (8-Br-cAMP) or the phosphodiesterase inhibitor, 3-isobutyl-1-methylxanthine. Forskolin (0.2-20 microM) increased adenylate cyclase activity and enhanced insulin secretion from the permeabilised islets. These r

Impedance and electrically evoked compound action potential (ECAP) drop within 24 hours after cochlear implantation.

Previous animal study revealed that post-implantation electrical detection levels significantly declined within days. The impact of cochlear implant (CI) insertion on human auditory pathway in terms of impedance and electrically evoked compound action potential (ECAP) variation within hours after surgery remains unclear, since at this time frequency mapping can only commence weeks after implantation due to factors associated with wound conditions. The study presented our experiences with regards to initial switch-on within 24 hours, and thus the findings about the milieus inside cochlea within
