Ethics questions raised by the neuropsychiatric, neuropsychological, educational, developmental, and family characteristics of 18 juveniles awaiting execution in Texas.

Eighteen males condemned to death in Texas for homicides committed prior to the defendants' 18th birthdays received systematic psychiatric, neurologic, neuropsychological, and educational assessments, and all available medical, psychological, educational, social, and family data were reviewed. Six subjects began life with potentially compromised central nervous system (CNS) function (e.g., prematurity, respiratory distress syndrome). All but one experienced serious head traumas in childhood and adolescence. All subjects evaluated neurologically and neuropsychologically had signs of prefrontal

Dexamethasone pulse therapy in pemphigus.

Pulse therapy with high-dose glucocorticoids was introduced 20 years ago as a treatment modality for autoimmune disease and transplant rejection. The most popular dermatological indication for pulse therapy is severe pemphigus. We reviewed the sequelae of 14 patients with pemphigus who were treated by pulse therapy. Seven of them reached complete remission, although three of them needed a new pulse course due to disease flare-up. Adverse events were minor and confined to 60% of all patients: temporary facial flushing during pulse administration, sleep disturbances during the first night after

Developing prosthetics to treat cognitive disabilities resulting from acquired brain injuries.

Persistent cognitive disabilities represent the most troublesome consequences of acquired brain injury. Although these problems are widely recognized, few neuroprosthetic efforts have focused on developing therapeutic strategies aimed at improving general cognitive functions such as sustained attention, intention, working memory or awareness. If possible, effective modulation of these neuropsychologic components might improve recovery of interactive behaviors. The emerging field of neuromodulation holds promise that technologies developed to treat other neurological disorders may be adapted to

AACAP Official Action. Summary of the practice parameters for child custody evaluation.

This summary is presented as a guide for clinicians evaluating the often delicate and complex issues surrounding a child custody dispute. The historical basis of child custody and the various judicial presumptions that have guided courts, as well as the differences between performing child custody evaluation and engaging in traditional clinical practice, are reviewed in the complete document. Issues that are common to all child custody disputes are presented, including continuity and quality of attachments, preference, parental alienation, special needs of children, education, gender issues, s

The HIV-positive psychiatric patient and the duty to protect: ethical and legal issues.

UNLABELLED: A small, yet significant, percentage of HIV-positive patients have a concomitant psychiatric or neurological disorder associated with impaired judgment. Clinical problems such as psychoses, certain personality disorders, and dementias are associated with disinhibition of impulses and diminished capacity for self-monitoring. These deficits in reasoning and judgment may prevent the patient from comprehending the significance of their HIV status and the consequences of sexual behavior or needle sharing. There are indications that the Tarasoff duty-to-protect may apply to this situatio

Development of consensus-based standards for a National Clinical Audit of Perioperative Care: protocol for a modified Delphi consensus study.

INTRODUCTION: Variation in perioperative care delivery and outcomes persists across healthcare systems, contributing to differences in patient experience, efficiency and outcomes. National clinical audits play a central role in benchmarking practice and supporting quality improvement. However, there is currently no unified, consensus-derived set of standards to underpin a comprehensive national audit of perioperative care across specialties in the UK. This proposed modified Delphi will develop a core set of standards to support benchmarking for the newly commissioned National Clinical Audit of

PHOENIX: a study protocol for a pilot randomised controlled trial of pre-emptive pharmacogenomic panel testing in acute care settings with health economic evaluations.

INTRODUCTION: Pharmacogenomic (PGx) panel testing may reduce adverse drug reactions (ADRs), treatment failures and downstream healthcare use by aligning prescribing with germline genetic variation. The PREPARE trial reported an approximately 30% reduction in clinically relevant ADRs using a pre-emptive multigene panel, but its open-label design, patient-reported endpoint and subsequent methodological critiques limit its use for National Health Service (NHS) decision-making. Whether panel-based PGx delivers benefit in acute NHS secondary care, where prescribing is time-critical, patients are ol

Effects of multimodal exercise programme combined with high-protein diet on glycaemic control in older adults with type 2 diabetes mellitus: study protocol for a randomised controlled trial.

INTRODUCTION: Effective management of type 2 diabetes mellitus (T2DM) in older adults requires interventions that address both metabolic control and functional capacity. Exercise improves insulin sensitivity, glucose uptake and cardiometabolic health, while high-protein diets support muscle mass preservation, satiety and glycaemic regulation. Evidence suggests that integrating structured exercise with a high-protein diet may provide additive benefits; however, research evaluating this combined approach in older adults with T2DM, particularly in low-resource settings, is limited. This study aim

Magnetic resonance-guided laser interstitial thermal therapy for mesial temporal lobe epilepsy (MINE): a single-centre, prospective, randomised, open-label, blinded-endpoint controlled trial protocol in China.

INTRODUCTION: Drug-resistant mesial temporal lobe epilepsy (mTLE) is a common and disabling focal epilepsy syndrome for which surgery offers the best chance of seizure freedom. Traditional open surgery (eg, anterior temporal lobectomy or selective amygdalohippocampectomy) is effective but may be associated with perioperative morbidity and neurocognitive sequelae. Magnetic resonance-guided laser interstitial thermal therapy (MRgLITT) is a minimally invasive alternative with encouraging observational outcomes but high-quality comparative evidence versus open surgery remains limited. The MINE tri

Protocol for the Parent Anxiety Transmission and Habits (PATH) study: a randomised controlled trial testing parent interpretation bias as a key mechanism of intergenerational transmission of anxiety.

INTRODUCTION: Anxiety disorders are the most common youth mental health problems in the USA. Most children do not receive treatment and among the few who do, many do not experience significant improvement. Thus, there is an urgent need to identify novel intervention targets to improve treatments for childhood anxiety. Past research demonstrates that parent anxiety plays a role in the development and maintenance of child anxiety, but empirical evidence for the mechanisms that drive this transmission is lacking. Interpretation bias, or the tendency to perceive threat in ambiguous situations, may

TROPION-Lung14 study protocol: a phase III study of osimertinib in combination with datopotamab deruxtecan versus osimertinib alone as first-line treatment for patients with EGFR-mutated locally advanced or metastatic non-small cell lung cancer.

BACKGROUND: Osimertinib is a first-line (1L) treatment option for patients with locally advanced/metastatic epidermal growth factor receptor (EGFR)-mutated non-small-cell lung cancer (NSCLC). Resistance to 1L osimertinib often develops and treatment options after disease progression are limited. The clinical benefit of 1L osimertinib in combination with agents which have broad antitumor activity has been demonstrated in phase III trials. Datopotamab deruxtecan (Dato-DXd), an antibody-drug conjugate composed of a humanized anti-trophoblast cell-surface antigen 2 monoclonal antibody conjugated t

Protocol for the development and validation of machine-learning models for predicting the risk of hypertriglyceridemia in critically ill patients receiving propofol sedation using retrospective data.

INTRODUCTION: Propofol is a widely used sedative-hypnotic agent for critically-ill patients requiring invasive mechanical ventilation (IMV). Despite its clinical benefits, propofol is associated with increased risks of hypertriglyceridemia. Early identification of patients at risk for propofol-associated hypertriglyceridemia is crucial for optimizing sedation strategies and preventing adverse outcomes. Machine learning (ML) models offer a promising approach for predicting individualized patient risks of propofol-associated hypertriglyceridemia. METHODS AND ANALYSIS: We propose the development

Study protocol: diagnosis of atrial fibrillation in postoperative thoracic surgery using a smartwatch, an open-label randomised controlled study (THOFAWATCH trial).

INTRODUCTION: Postoperative atrial fibrillation (POAF) affects approximately 20% of patients undergoing thoracic surgery and is associated with severe complications such as stroke, myocardial infarction, heart failure, and increased mortality. Early diagnosis is critical to mitigate these risks, but conventional monitoring is limited in detecting asymptomatic episodes. Smartwatches equipped with single-lead ECG and atrial fibrillation (AF) detection algorithms offer a novel approach for early POAF detection. This study aims to evaluate the effectiveness of smartwatch-based monitoring compared

Effectiveness of the BioFire FilmArray for the rapid detection of bloodstream infection in haematological patients with febrile neutropenia (the ONFIRE study): study protocol of a prospective, multicentre observational study at three reference university hospitals in Spain.

INTRODUCTION: Bloodstream infection (BSI) due to multidrug-resistant Gram-negative bacilli is a serious global health problem that has a profound impact on severely immunosuppressed neutropenic haematological patients. Prompt institution of appropriate antimicrobial therapy is crucial for improving outcomes in these patients, and in an era of multidrug resistance, antimicrobial stewardship programmes are mandatory. Blood cultures, the current gold standard for the diagnosis of BSI, present two main drawbacks: the prolonged time to results and their low sensitivity, especially if the patient ha

Development, validation and clinical utility of prognostic models for patients with traumatic brain injury in an Indian setting using machine learning and traditional approaches: a study protocol.

INTRODUCTION: Traumatic brain injury (TBI) remains a major public health concern in India, with high mortality and long-term disability. Existing prognostic models, mostly developed in high-income countries using traditional methods, lack generalisability to the Indian context and do not use the potential of machine learning or multicentric data. This study primarily aims to develop, compare and validate machine learning methods, including the traditional approach, to predict 30-day mortality and 6-month functional outcomes in patients with moderate or severe TBI. A secondary objective is to d

Clinical effectiveness of biofeedback for treating stress urinary incontinence in women: protocol for a randomised controlled trial.

INTRODUCTION: Pelvic floor muscle (PFM) training (PFMT) is a standard conservative first-line treatment for stress urinary incontinence (SUI) in women. However, the success of PFMT alone, without adjunctive therapies, has been hindered by the lack of adherence among women, who report the inability to contract the correct PFMs and limited motivation to perform PFMT exercises. Therefore, PFMT is often combined with adjunctive therapies, such as biofeedback (BF). The conventional BF device employed in clinical practice is invasive and involves the insertion of an intravaginal probe, causing pain

STRONG II trial: stereotactic body radiation therapy following chemotherapy for unresectable perihilar cholangiocarcinoma - a single-arm multicentre phase II study.

INTRODUCTION: For patients with perihilar cholangiocarcinoma (pCCA), surgical resection remains the sole treatment modality that can potentially result in cure. Unfortunately, the majority of patients present with unresectable tumours or are excluded from surgical treatment due to complications like cholangitis affecting their performance status. In the Netherlands, recommended first-line treatment for patients with unresectable pCCA is palliative chemotherapy with gemcitabine and cisplatin. This regimen yields an estimated median overall survival (OS) of 11.7-15.2 months, highlighting the urg

Impact of Nutritional Support for Tuberculosis on Intermediate and Terminal Undernutrition and Treatment Outcomes (INSTITUT) study: a protocol for a quasi-experimental study to assess the impact of nutritional support on tuberculosis treatment outcomes in Benin and Togo.

INTRODUCTION: Undernourished persons with tuberculosis (TB) (PWTB) are at increased risk of mortality, treatment failure and relapse. Nutritional support for PWTB has the potential to mitigate the risk of undernutrition. However, given ethical concerns, well-powered randomised control trials of macronutrient support are unavailable to inform policymakers, clinicians and researchers about the impact of nutritional support. However, there is potential to understand the impact of macronutrient nutritional support through a natural experiment. Benin and Togo are neighbouring countries in West Afri
