The Intermaxillary-Mandibular Approach to the Post-Styloid Parapharyngeal Space: An Anatomic Study.

OBJECTIVE: Lesions of the parapharyngeal space (PPS) represent a challenging surgical pathology for resection, especially when critical structures like the parapharyngeal internal carotid artery (ppICA) are difficult to visualize and access. Current approaches provide inadequate visualization and incomplete access with significant morbidity. We investigate a novel approach, the intermaxillary-mandibular (IMM) approach, and compare this approach to traditional surgical approaches for visualization and access to the PPS and ppICA. METHODS: A single-institution anatomical study with endoscopic I

Ultrastructural Analysis of Infected Absorbable Bone Fixation Material Using Scanning Electron Microscopy After Zygomatic Bone Fracture Surgery.

Absorbable bone fixation materials are commonly used in facial bone fracture surgery. However, reports on the microstructure of infected implants are rare. This study presents a case involving a 74-year-old male patient who developed an α-Streptococcus infection 2.5 months after zygomatic bone fracture surgery and the ultrastructure of an infected absorbable plate observed under a scanning electron microscope (SEM). The patient sustained a right zygomatic bone fracture due to a fall and underwent open reduction and internal fixation with SonicWeld on the seventh day after the injury. The posto

Digital Workflow Using A Free 3-D Mesh Modeling Software as a Tool in the Treatment of a Pediatric Mandibular Fracture.

The treatment of pediatric mandibular fractures is challenging due to the risk of injury to tooth germs and the potential of facial development disturbances. These cases require precise reduction and fixation to minimize such risks, while also promoting return to function. Long-term follow-up is of utmost importance, once the removal of osteosynthesis material due to plate migration might be required. Virtual surgical planning associated with 3-D printing optimizes functional and aesthetic outcomes, while reducing surgical time and intraoperative complications. This study aimed to describe the

Postmortem diagnosis of fatal venous air embolism due to sigmoid sinus rupture associated with a basilar skull fracture.

BACKGROUND: Venous air embolism (VAE) secondary to basilar skull fractures is rare but potentially fatal. While VAE is a known complication in central venous catheter management, traumatic VAE-particularly in forensic contexts-remains underreported. In forensic medicine, few case reports have detailed the use of postmortem computed tomography (PMCT) to diagnose VAE. CASE PRESENTATION: A man in his 30 s sustained a head injury at a construction site when his head was caught between a 500-kg steel plate and the ground. Despite resuscitation attempts, spontaneous circulation was not restored, an

Case report of massive spontaneous subcutaneous emphysema and pneumomediastinum after cervical discectomy.

INTRODUCTION: Anterior cervical discectomy and fusion (ACDF) is a commonly performed neurological procedure, for treating cervical spine pathologies such as cervical disc herniation. Despite the desirable results, it may cause serious, life threatening complications include pneumothorax, pneumomediastinum, and subcutaneous emphysema, which happen duo to iatrogenic injury to trachea, esophagus, or Hypopharynx. Spontaneous cases are particularly unusual. PRESENTATION OF CASE: In this report we describe a case of spontaneous Subcutaneous emphysema (SE) following ACDF. A 69-year-old female patien

Incidental neck mass in a young adult: A case report.

INTRODUCTION: While true extracranial internal carotid aneurysms (tECAAs) are extremely rare and often asymptomatic, they can lead to serious complications including rupture, thrombosis or embolization. Surgery has been the standard, even in asymptomatic patients, due to safety and long-term outcomes. However, advancements in technology have introduced endovascular interventions and observation as options. PRESENTATION OF CASE: A 32-year-old obese man presented with sudden right-sided facial numbness lasting 3 h. Personal, family, social and trauma history were unremarkable. Cardiac and neuro

Automatic detection of midfacial fractures in facial bone CT images using deep learning-based object detection models.

BACKGROUND: Midfacial fractures are among the most frequent facial fractures. Surgery is recommended within 2 weeks of injury, but this time frame is often extended because the fracture is missed on diagnostic imaging in the busy emergency medicine setting. Using deep learning technology, which has progressed markedly in various fields, we attempted to develop a system for the automatic detection of midfacial fractures. The purpose of this study was to use this system to diagnose fractures accurately and rapidly, with the intention of benefiting both patients and emergency room physicians. ME

Affection of Surgical Decompressive Scale of Optic Canal to Traumatic Optic Neuropathy.

Traumatic optic neuropathy (TON) is damage to the optic nerve that is caused by external violence to the optic nerve during cranial and facial trauma. This kind of injury may result in impaired vision, has a high risk of blindness, and significantly impairs the neurological function of the patient. The treatment of TON is controversial, and many different approaches have been suggested. No one is considered best because the traumatic mechanism is not clear. Methods: In this retrospective study, the clinical features of 37 patients diagnosed with TON without light perception who were treated at

Cerebellar Hypoperfusion in Two Patients with Cornelia de Lange Syndrome with Novel NIPBL Variants.

INTRODUCTION: Cornelia de Lange syndrome (CdLS) is a rare congenital malformation characterized by distinctive facial features, short stature, and limb defects. In addition, half of the patients with CdLS exhibit repetitive self-injurious behaviors (SIBs) related to intellectual disability with autistic traits. CdLS is caused by pathogenic variants of genes encoding the cohesin complex pathway, with 70% of these variants identified in the nipped-B-like (NIPBL) gene. CASE PRESENTATION: We report 2 patients with CdLS who exhibited repetitive SIBs. Patient 1, a 40-year-old male, carried a novel

Managing the acutely threatened airway following head and neck trauma - Requiem for cricothyroidotomy?

INTRODUCTION: Trauma to the head and neck results in acute facial trauma and swelling, which may occlude the airway and result in fatal hypoxia. The management is the establishment of a definitive airway. This paper reviews our experience with this clinical scenario. MATERIALS & METHODS: A retrospective study was conducted over a seven-year period from December 2012 to January 2020 at a major trauma centre in South Africa on all adult trauma patients who required a surgical airway procedure (surgical cricothyroidotomy (SC) or surgical tracheostomy (ST)). RESULTS: From December 2012 to Januar

Intracapsular enucleation of cervical schwannomas via retroauricular hairline incision.

INTRODUCTION: Cervical schwannomas are rare diseases treated by complete resection, leading to potential neurological deficits. Intracapsular enucleation is an acceptable alternative to preserve the involved nerve function, but the clinical outcomes by an aesthetically pleasing approach remain unrevealed. This study evaluated functional and oncological outcomes after intracapsular enucleation of cervical schwannomas via retroauricular hairline (Roh's) incision. MATERIALS AND METHODS: This prospective observational study included 17 patients with cervical schwannomas who underwent intracapsula

Idiopathic Retroperitoneal Fibrosis Presented As Urinary Tract Obstruction.

Retroperitoneal fibrosis (RPF) or Ormond's disease is a very uncommon fibro-inflammatory disease, under the umbrella of systemic autoimmune diseases. The majority of cases are idiopathic, known as idiopathic RPF (IRPF); however, diseases secondary to other causes are also seen in clinical practice. The commonest presenting features are seen due to the effects of fibrous tissue around iliac vessels, aorta and ureters, where compression of ureters is the major and most common complication. Computed tomography (CT) scans and magnetic resonance imaging (MRI) are the modalities of choice for the di

DRESS syndrome following furosemide administration: An unusual association.

INTRODUCTION: Drug reaction with eosinophilia and systemic symptoms (DRESS) is a severe and rare adverse drug reaction. Several drugs are known to induce DRESS. Furosemide, a sulfonamide loop diuretic drug, is known to induce hypersensitive reactions such as bullous eruptions, acute generalized exanthematous pustulosis and lichenoid eruptions, but rarely DRESS. We describe herein a case of furosemide-induced DRESS that recurred after bumetanide administration. CASE REPORT: A 67-year-old man was admitted to the nephrology department for hypertension, gout and chronic renal failure. He received

Two middle-aged cases of deep overbite without molar support treated by orthognathic surgery.

INTRODUCTION: Patients with severe overbite in middle age often undergo prosthetic treatment without a diagnosis of dentofacial deformity, but soft tissue trauma can occur in such patients due to the lack of inter-occlusal space. Comprehensive surgical orthodontic treatment and prosthetic treatment are indicated to correct the overbite and soft tissue injury for such patients. PRESENTATION OF CASE: We report the cases of two middle-aged patients with dentofacial deformity and severe overbite without molar support. In both cases, prosthetic treatment had been performed for many years without a

Comprehensive anatomy of the foramen ovale critical to percutaneous stereotactic radiofrequency rhizotomy: cadaveric study of dry skulls.

OBJECTIVE: Percutaneous stereotactic radiofrequency rhizotomy (PSR) is often used to treat trigeminal neuralgia, a serious condition that results in lancinating, episodic facial pain. Thorough understanding of the microsurgical anatomy of the foramen ovale (FO) and its surrounding structures is required for efficient, effective, and safe use of this technique. This morphometric study compares anatomical and surgical orientations to identify the variations of the FO and assess cannulation difficulty. METHODS: Bilateral foramina from 174 adult human dry skulls (348 foramina) were analyzed using

Bubble CPAP therapy for neonatal respiratory distress in level III neonatal unit in Amman, Jordan: a prospective observational study.

PURPOSE: Bubble continuous positive airway pressure (bCPAP), a noninvasive respiratory support modality used to manage newborns with respiratory distress, provides continuous pressure that helps prevent derecruitment of alveoli, increasing the lungs' functional residual capacity, and thus decreasing the work of breathing. bCPAP can be used to manage various respiratory conditions in the newborn. In this prospective study, we describe our experience using bCPAP therapy as the primary respiratory support in a level III neonatal unit in Amman, Jordan. In addition to reporting therapeutic indicati
