Upper extremity injuries associated with strength training.

Most injuries sustained during strength training are mild strains that resolve with appropriate rest. More severe injuries include traumatic shoulder dislocations, tendon ruptures of the pectoralis major, biceps, and triceps; stress fractures of the distal clavicle, humerus, radius, and ulna; traumatic fractures of the distal radius and ulna in adolescent weightlifters; and compressive and stretch neuropathies. These more severe injuries are usually the result of improperly performing a strength training exercise. Educating athletes regarding proper strength-training techniques serves to rever

Hemarthrosis after trauma to the pediatric knee joint: what is the value of magnetic resonance imaging in the diagnostic algorithm?

In children, compulsory arthroscopy for hemarthrosis after knee trauma is not justified because ligamentous and meniscal damage is rare. In a prospective study, we analyzed the diagnostic value of radiography, magnetic resonance imaging (MRI), and arthroscopy in 51 patients up to 14 years of age with acute knee trauma. Plain radiography revealed 16 osseous lesions (5 metaphyseal, 3 patellar, 4 physeal fractures, 3 avulsions of the tibial spine, and 1 osseous ligamentous tear). In 29 patients, the cause of hemarthrosis remained unclear. All patients were evaluated by MRI. A diagnosis could be a

[Femoral neck fractures in adults: joint sparing operations. I. Results of an AO collective study with 328 patients].

In a multi-center study in 14 trauma hospitals 328 patients with fractures and osteosyntheses of the femoral neck were investigated with reference to time between accident and operation implants used, early and late complications, and results in the years from 1974 to 1987. A follow-up examination was possible in 266 patients a mean of 46.7 months after their accidents. General postoperative complications were recorded in 12.2% and local complications in 11.6%. In 9.8% pseudarthrosis was observed, in 26% an aseptic femoral head necrosis and in 2.4% deep infections. Functional results were exce

Suture-Button Stabilisation for Thumb Carpometacarpal Joint Instability and Dislocation: A Retrospective Case Series.

BACKGROUND: Suture-button suspension is commonly used post trapeziectomy, stabilising the first metacarpal base to its adjacent counterpart. It has more recently been used in the setting of carpometacarpal instability, avoiding the morbidity of tendon harvest and large tunnels required in traditional stabilisation techniques. Evidence for its use in this setting remains limited. METHODS: A retrospective review of 13 consecutive patients, including 4 adolescents, with CMC1 instability treated with suture-button stabilisation by 2 fellowship-trained hand surgeons between 2018 and 2024 was perfo

Primary total hip arthroplasty after prior osteosynthesis versus osteotomy : distinct risk profiles and the modifying effect of femoral fixation.

AIMS: To compare outcomes after primary total hip arthroplasty (THA) following prior ipsilateral osteosynthesis with prior ipsilateral osteotomy, and to assess whether femoral fixation modifies risk. METHODS: Using the German Arthroplasty Registry (EPRD; 2012 to 2024), two 1:1 matched analyses were performed: prior osteosynthesis versus no prior surgery (8,643 pairs) and prior osteotomy versus no prior surgery (5,569 pairs), matched on age, sex, BMI category, and Elixhauser score. Kaplan-Meier and Cox regression were used, censoring at death, with adjustment for femoral fixation (cemented, ce

Chondroblastoma of the femoral head: curettage without dislocation.

BACKGROUND: Chondroblastoma (CBL) of the femoral head is a rare disease, typically encountered in the epiphysis of long bones, with its occurrence in the femoral head being particularly uncommon. The unique location and aggressive nature of this tumor pose substantial challenges in its treatment, leading to ongoing controversies regarding the therapeutic approaches. In this study, we introduce a technique of curettage without surgical dislocation as a treatment option for CBL in this challenging location. METHODS: A total of 6 children diagnosed with chondroblastoma of the femoral head underw

Neglected subluxation of the hip after trauma: A case report.

INTRODUCTION: In children, the pelvis contains a large amount of cartilage components; therefore, when traumatic hip dislocation spontaneously reduces, it can be impossible to see on X-ray or computed tomography (CT) images in some cases, which can delay its detection. CASE PRESENTATION: We report the case of a 10-year-old boy who was injured by being hit by a car while walking. Upon diagnosis of pelvic ring fracture, the patient received conservative treatment. Seven months after injury, the patient was referred to our department with the chief complaint of limping. DIAGNOSIS: Marked limita

Proximal femoral replacement in non-oncologic patients undergoing revision total hip arthroplasty.

PURPOSE: Proximal femoral replacements (PFRs) have been recently utilized in complex revision arthroplasties where proximal femoral bone is compromised. The purpose of this study is to evaluate the clinical outcomes, complications, and survivorship of PFRs as a salvage treatment for severe bone loss after non-oncologic revision total hip arthroplasty. METHODS: This is a retrospective review of all patients who underwent femoral revision surgery using a single design PFR between 2004 and 2013 at our institution. Forty patients (41 hips) were included with a mean age of 64 years (29-90). Accord

Ankylosing Spondylitis Increases Perioperative and Postoperative Complications After Total Hip Arthroplasty.

BACKGROUND: Ankylosing spondylitis (AS) is a chronic autoimmune spondyloarthropathy that primarily affects the axial spine and hips. Progressive disease leads to pronounced spinal kyphosis, positive sagittal balance, and altered biomechanics. The purpose of this study is to determine the complication profile of patients with AS undergoing total hip arthroplasty (THA). METHODS: The Medicare sample was searched from 2005 to 2012 yielding 1006 patients with AS who subsequently underwent THA. Risk ratios (RRs) with 95% confidence intervals (CIs) were calculated for 90-day, 2-year, and the final p

Middle ear damages.

Middle ear damages. The eardrum and middle ear are often exposed to blunt and penetrating trauma, blasts, thermal or caustic injuries. These injuries may result in tympanic membrane perforation, middle ear haemorrhage, dislocation and fracture of the ossicular chain, perilymphatic fistula and damage to the chorda tympani and/or facial nerve. In case of life-threatening injuries and/or mass casualty incidents, middle ear trauma obviously does not take highest priority. However, middle ear lesions should be suspected and recognized as early as possible. After meticulous history taking, physical

The Modified Girdlestone Procedure With Muscle Flap for Management of Pressure Ulcers and Heterotopic Ossification of the Hip Region in Spinal Injury Patients: A 15-Year Review With Long-term Follow-up.

BACKGROUND: Pressure ulceration, pyogenic arthritis, and hip heterotopic ossification are complications encountered in spinal cord injury patients. The Girdlestone procedure has been established as an effective treatment modality for hip pathology in this population and is particularly efficacious when a muscle flap is used to fill the femoral resection defect. We previously reported its use as a 3-stage procedure. Through time, experience was gained, and consequently, our technique has evolved to a 1-stage procedure. METHODS: A retrospective analysis of all spinal cord injury patients who ha

[Post-traumatic palmar instability of the thumb metacarpophalangeal joint. The "other skier's thumb"].

"Passive" and "active" stabilizers give the thumb metacarpophalangeal (MP) joint the necessary palmar stability. It is possible to temporarily stabilize the joint by means of the flexor muscles in cases where there is an injury to the palmar ligaments due to hyperextension trauma. However, sudden severe or prolonged strain may lead to palmar instability and/or stress-related pain. In an experimental study, the MP joints of 40 specimen thumbs were hyperextended to varying degrees. The extent of tissue damage was evaluated during dissection. Between 1984 and 1992, in 72 cases with acute hyperext

Conversion of failed revision total knee arthroplasty in arthrodesis with modular nail maintaining the uncemented femoral stem in patient with extensor mechanism insufficiency: a case report.

BACKGROUND: The transition from revision total knee arthroplasty (RTKA) to arthrodesis involves the replacement of cemented femoral and tibial stems with a modular nail designed for arthrodesis. This conversion process is associated with challenges such as bone loss, blood loss, and prolonged surgical durations. Effectively addressing these complexities through a less invasive surgical approach could be pivotal in enhancing patient outcomes and minimizing associated complications. CASE PRESENTATION: A 75-year-old white Caucasian female patient with a revision total knee arthroplasty (RTKA) pe

Investigation of mechanical behavior on the cement hip spacer geometry under finite element method and compression load test.

BACKGROUND AND OBJECTIVE: Antibiotic cement spacers (CS) are standard treatment for periprosthetic hip infection. However, complications such as cement spacer fracture and spacer dislocation remain common after surgery. The aim of this study is to investigate the influence of CS geometry design on mechanical strength through finite element analysis and compression load testing. METHODS: The CS model was generated using the three-dimensional engineering program, and an aluminum molding block was created. The mechanical behavior was analyzed using finite element method (static structural versio
