Management Strategies for Tibial Periprosthetic Fractures After Total Knee Arthroplasty.

Tibial periprosthetic fractures following total knee arthroplasty are relatively uncommon, but represent a complex and increasingly encountered complication given the growing volume of primary and revision arthroplasty procedures. These injuries present unique challenges due to compromised bone stock, implant-related constraints, and patient-specific factors such as advanced age and osteoporosis. The location of the fracture, implant stability, bone quality, and the functional needs of the patient all influence management tactics. While operative intervention is frequently necessary for displa

Tumour-induced osteomalacia: the long road to diagnosis and recovery.

Tumour-induced osteomalacia is caused by tumorous production of fibroblast growth factor 23 (FGF23) leading to urinary phosphate wasting, hypophosphataemia and decreased vitamin D activation. The resulting osteomalacia presents with muscle weakness and bone pain but progresses to multiple pathological fractures. Patients often remain undiagnosed for years with severe physical, psychological and economic ramifications. A young woman presented with multiple spontaneous fractures including bilateral femoral fractures. Laboratory tests revealed severe hypophosphataemia, elevated bone turnover mark

Proton pump inhibitor therapy and osteoporosis.

Osteoporotic fractures, particularly hip fractures, can have a devastating impact on the well-being of the elderly population. Recently, two population-based observational studies reported a highly important association between the use of potent acid suppressive therapy and an increased risk of hip fractures. The mechanisms underlying such an association are not clear. However, a careful review of the existing evidence seems to suggest that the main physiologic consequences of proton pump inhibitor therapy may each have a theoretical influence on bone metabolism. Specifically, inhibition of th

Endoscopic endonasal reduction surgery for diplopia caused by orbital roof fractures.

BACKGROUND: Orbital roof fractures (ORF) frequently occur together with frontal bone fractures, and can cause diplopia as a sequela. However, ORF have never been treated using endoscopic endonasal reduction surgery. CASE: A 22-year-old woman was injured in a traffic accident and diagnosed with right ORF along with frontal bone fractures. She was referred to us for diplopia 2 weeks after the initial trauma. No facial deformation was seen. Endoscopic endonasal reduction surgery was performed under general anaesthesia. Ethmoidectomy was performed and, subsequently, the anterior superior portion

Glucocorticoid-induced osteoporosis: clinical and therapeutic aspects.

Glucocorticoid-induced osteoporosis (GIO) is the most common form of secondary osteoporosis. Fractures, which are often asymptomatic, may occur in as many as 30-50% of patients receiving chronic glucocorticoid therapy. Vertebral fractures occur early after exposure to glucocorticoids, at a time when bone mineral density (BMD) declines rapidly. Fractures tend to occur at higher BMD levels than in women with postmenopausal osteoporosis. Glucocorticoids have direct and indirect effects on the skeleton. They impair the replication, differentiation, and function of osteoblasts and induce the apopto

[Effect of vitamin D on muscle strength and relevance in regard to osteoporosis prevention].

A successful prevention strategy for fractures in the elderly should not be limited to an improvement in bone mineral density. Equally important is the prevention of falls. Thus, 90% of fractures in the elderly are associated with a fall and 30% of all ambulatory, and 50% of institutionalized elderly age 65 years and older fall at least once a year. Fall incidence increases 10% per decade thereafter. According to recent studies, vitamin D and calcium supplementation may be a promising treatment strategy targeting both bone mineral density, as well as muscle strength and the risk of falling. Th

Osteoporosis considerations in the frail elderly.

Osteoporosis is a growing public health problem throughout the world, in part because of the increasing numbers of people living beyond the age of 65 years. Skeletal fractures are the clinical manifestation of the disease, with older patients the most severely affected. Conditions associated with frailty such as falls and reduced muscle strength likely contribute to fractures, causing substantial mortality, morbidity, and economic cost. Screening guidelines for osteoporosis have been issued recently and take into account multiple risk factors for this condition. Falls are the chief mechanism b

Reversal of severe osteoporosis with vitamin B12 and etidronate therapy in a patient with pernicious anemia.

Pernicious anemia has recently been recognized as a risk factor for osteoporosis and fractures. Although vitamin B12 is important for osteoblast function, the effect of vitamin B12 replacement in states of vitamin B12 deficiency on bone density and fracture incidence is not known. We report 2-year follow-up data from a patient with severe osteoporosis, multiple vertebral compression fractures, and pernicious anemia who exhibited a dramatic response to treatment with vitamin B12 and cyclic etidronate. Serial bone density measurements demonstrated a 15% and 17% increase in the lumbar and greater

Stress fractures. Identifiable risk factors.

To answer the question why such large differences in stress fracture morbidity rates (2% to 64%) exist in different countries, we prospectively evaluated 312 recruits for possible risk factors for stress fractures. Prior to training, each recruit underwent an evaluation including the following: orthopaedic examination, foot and tibial radiographs, measurements of tibial bone width, bone mineral content, bone density, aerobic physical fitness and leg power, assessments of somatotype and smoking habits, and evaluation of sociological and psychological factors. Using a multivariate analysis, two

The use of bone scanning for the diagnosis and management of musculoskeletal trauma.

Bone scanning provides a unique way of monitoring early metabolic changes in bone and its adjacent soft tissues after trauma. An abnormal scan may often precede radiographic changes by days or weeks. This accounts for its usefulness in the early diagnosis of occult fractures and in fractures not clinically suspected or seen on initial x-ray films in the patient with multiple trauma. Dynamic imaging along with static imaging can be used to evaluate fracture healing and predict delayed union and nonunion. The combined technique is useful in assessing many of the complications arising after traum

Unusual Femur Stress Fractures in Children With Osteogenesis Imperfecta and Intramedullary Rods on Long-term Intravenous Pamidronate Therapy.

In this report, we describe 6 children with osteogenesis imperfecta with unusual stress femoral fractures. All children were on long-term cyclic pamidronate treatment. All fractures occurred without trauma or with minimal trauma and were located in the subtrochanteric or the diaphyseal regions of the femur over preexisting intramedullary rods. These fractures have very similar features to the reported minimal trauma atypical femoral fractures in adults on long-term bisphosphonate treatment. These fractures raise concerns about the role of prolonged remodeling suppression and microdamage accumu

Risk of vertebral fractures in hypoparathyroidism.

Parathyroid hormone (PTH) exerts both anabolic and catabolic actions on bone, depending on the duration and periodicity of exposure. Hypoparathyroidism is defined by inadequate production of PTH in the presence oflow serum calcium. In hypoparathyroidism it has been reported an increase in corticaland trabecular bone mass, but it is still unknown if these quantitative variations maybe accompanied by qualitative ones and increased bone strength. Despite the extensive data available on the effects of hypoparathyroidism on bone, itseffect on the hard end point in this area which is the risk of frac

Computational modeling of ultrasonic backscattering to evaluate fracture healing.

Fracture healing is a complex, regenerative procedure including several phases of recovery as the original mechanical and geometrical features of bone are gradually restored. Ultrasonic evaluation of bone pathologies such as osteoporosis and fracture healing has recently gained significant interest due to the non-invasive and non-radiating nature of the method. In this study, we present numerical simulations of ultrasonic backscattering in simple, two dimensional geometries of healing long bones to investigate the monitoring capacity of the acoustic pressure in the backward direction. The frac

The skeletal consequences of thyrotoxicosis.

Euthyroid status is essential for normal skeletal development and the maintenance of adult bone structure and strength. Established thyrotoxicosis has long been recognised as a cause of high bone turnover osteoporosis and fracture but more recent studies have suggested that subclinical hyperthyroidism and long-term suppressive doses of thyroxine (T4) may also result in decreased bone mineral density (BMD) and an increased risk of fragility fracture, particularly in postmenopausal women. Furthermore, large population studies of euthyroid individuals have demonstrated that a hypothalamic-pituita

Radiography and scintigraphy of suspected scaphoid fracture. A long-term study in 160 patients.

Radiographs of the scaphoid after injury are difficult to interpret, and bone scintigraphy is widely used to increase the accuracy of diagnosis, though many fractures suspected on scintigraphy cannot be confirmed radiologically. We have reviewed the clinical consequences, after one year, of managing suspected scaphoid fractures according to the bone-scan results. We studied 160 patients, 35 of whom had initially positive radiographs and were treated in a cast for 12 weeks. The other 125 had bone scintigraphy and were managed according to the result. After a minimum of one year 119 patients wer

[Diagnosis and treatment of osteoporosis in patients with chronic kidney disease : Joint guidelines of the Austrian Society for Bone and Mineral Research (ÖGKM), the Austrian Society of Physical and Rehabilitation Medicine (ÖGPMR) and the Austrian Society of Nephrology (ÖGN)].

DEFINITION AND EPIDEMIOLOGY: Chronic kidney disease (CKD): abnormalities of kidney structure or function, present for over 3 months. Staging of CKD is based on GFR and albuminuria (not graded). Osteoporosis: compromised bone strength (low bone mass, disturbance of microarchitecture) predisposing to fracture. By definition, osteoporosis is diagnosed if the bone mineral density T‑score is ≤ -2.5. Furthermore, osteoporosis is diagnosed if a low-trauma (inadequate trauma) fracture occurs, irrespective of the measured T‑score (not graded). The prevalence of osteoporosis, osteoporotic fractures and

Cushing's disease as a cause of severe osteoporosis: a clinical challenge.

Secondary osteoporosis is a frequently underestimated bone disorder. It is a secondary cause of bone loss that affects more than half of men and premenopausal and perimenopausal women, and about one-fitfth of postmenopausal women. We herein report an uncommon case of multiple fractures due to secondary osteoporosis caused by Cushing's disease. In this case the appearance of fractures in a 41 years old woman was the sign of alarm that ultimately lead us to the diagnosis.

Intramedullary Strut Bone Peg Technique for Revision of Metacarpal and Phalangeal Nonunion After Failed Plate Fixation.

Metacarpal and phalangeal nonunion is rare; however, surgery is required if these fractures result in instability or persistent pain. Revision of nonunion following plate fixation is challenging, as it demands bone grafting at the nonunion site and stable fixation. Here, we present a surgical technique for managing metacarpal and phalangeal nonunion after failed plate fixation using an intramedullary strut bone peg reinforced with intramedullary K-wires. This technique achieves a high union rate by increasing the intramedullary contact surface, promoting bone healing. Moreover, intramedullary

Scaphoid fractures in soccer goalkeepers.

The growing popularity of soccer in this country has led to an increasing number of soccer-related injuries. Epidemiologic studies report that soccer-related musculoskeletal injuries most frequently involve the lower extremities. These range from 56% to 95% of soccer injuries and player-to-player contact is the most common mechanism of injury. Upper extremity musculoskeletal injuries are less frequent, but have been reported, with goalkeepers being the most susceptible. There is a paucity in the literature regarding bony injuries about the wrist and hand among goalkeepers. The purpose of this
