Despite significant advances in immunosuppression strategies, antibody-mediated rejection (AMR) remains a leading cause of graft dysfunction and loss after heart transplantation. Donor-specific anti-HLA antibodies (DSA), either preformed or de novo, are key contributors. Blood transfusion is a frequent sensitizing event that can amplify alloimmune responses. We report the case of a 64-year-old woman transplanted for end-stage ischemic cardiomyopathy, requiring ECMO support, who developed severe AMR shortly after receiving red blood cell and pooled platelet transfusions. Although pre-transplant

BACKGROUND: The relative efficacy of calcineurin inhibitors tacrolimus and cyclosporine in lung transplantation remains unclear. To clarify, we conducted a systematic review and meta-analysis.
METHODS: We searched through EMBASE, MEDLINE and Cochrane CENTRAL until 23 October 2023 for randomised trials comparing tacrolimus with cyclosporine in lung transplant recipients. Data extraction and bias risk assessment were done independently. Analyses included random effects pairwise meta-analysis and trial sequential analysis, with GRADE system for evidence certainty.
RESULTS: We found four eligibl

Chronic histiocytic intervillositis (CHI) is a rare placental inflammatory lesion increasingly viewed through maternal-foetal immune rejection. In this invited commentary, we discuss how a recent study bolsters the paradigm that CHI represents maternal immune rejection of the semi-allogeneic foetus, analogous to antibody-mediated rejection in organ transplantation. We highlight the shared histological and molecular features between CHI and kidney allograft rejection, including macrophage-dominated inflammation, complement activation, and interferon-gamma-driven gene expression signatures and e…

BACKGROUND: Cardiac transplantation is a crucial intervention for end-stage heart failure, but complications such as acute cellular rejection and cardiac allograft vasculopathy continue to challenge patient outcomes. While endomyocardial biopsy is the gold standard for diagnosis, it is invasive, prompting the use of noninvasive methods like echocardiography. Point-of-care ultrasound (POCUS) has become increasingly accessible in the Emergency Department (ED), offering potential for early detection of transplant-related issues.

BACKGROUND: Solid organ transplant (SOT) recipients receive immunosuppressive drugs that predispose them to opportunistic infections, including cytomegalovirus (CMV) infection. In these patients, CMV infection can lead to life-threatening consequences, such as transplant rejection. This systematic review and meta-analysis investigated the role of CMV infection in the development of acute rejection in SOT recipients.
METHODS: PubMed, Embase, Scopus, and Web of Science databases were systematically searched for eligible publications until Jan 31, 2025. English-language studies reporting the ass

Liver transplantation has undergone progressive advancements since the 1960s, with optimized surgical techniques and improved outcomes resulting from standardized procedures and therapeutic regimens. However, postoperative complications-such as ischemia-reperfusion injury, acute rejection, hepatocellular carcinoma recurrence, and biliary injury-continue to significantly affect both survival rates and quality of life in liver transplant recipients. Efferocytosis, the phagocytic clearance of apoptotic cells, plays a critical role in preventing necrosis-induced inflammation and promoting tissue r

BACKGROUND: There is an increasing number of biopsies with histological features of antibody mediated rejection but lacking the detection of anti-human leucocyte antigen (HLA) antibodies or C4d deposition. Anti-Human neutrophil antigens-3 (HNA-3) have been reported in severe and fatal cases of Transfusion-Related Acute Lung Injury and Neutropenia Alloimmune Neonatal, a phenomenon mediated by antibody endothelial activation. This study investigated the association between the presence of pre-formed anti-HNA-3 antibodies and the development of a recurrent episode of acute rejection (AR) after ki

The term "chronic lung allograft dysfunction" has emerged to describe the clinical syndrome of progressive, largely irreversible dysfunction of pulmonary allografts. This umbrella term comprises 2 major clinical phenotypes: bronchiolitis obliterans syndrome and restrictive allograft syndrome. Here, we discuss the clinical manifestations, diagnostic challenges, and potential therapeutic avenues to address this major barrier to improved long-term outcomes. In addition, we review the immunologic mechanisms thought to propagate each phenotype of chronic lung allograft dysfunction, discuss the vari

BACKGROUND: Evidence on the optimal maintenance of immunosuppressive regimen in kidney transplantation recipients is limited.
METHODS: The Amsterdam, LEiden, GROningen trial is a randomized, multicenter, investigator-driven, noninferiority, open-label trial in de novo kidney transplant recipients, in which 2 immunosuppression minimization strategies were compared with standard immunosuppression with basiliximab, corticosteroids, tacrolimus, and mycophenolic acid. In the minimization groups, either steroids were withdrawn from day 3, or tacrolimus exposure was reduced from 6 mo after transplan

Therapeutic plasma exchange (TPE) is a cornerstone treatment for antibody-mediated rejection (AMR) post-organ transplantation, aiming to eliminate pathogenic donor-specific HLA antibodies (DSA). However, limitations in HLA antibody interpretation due to the prozone-like effect (PLE) can lead to inaccurate assessment of treatment efficacy. We present a case of a heart transplant recipient with suspected AMR, where an unexpected increase in DSA levels post-TPE prompted investigation into PLE. Solid-phase Luminex assays were employed to detect HLA antibodies. Serum was run neat as well as after t

In solid organ transplantation (SOT), biologicals such as recombinant therapeutic proteins, monoclonal antibodies, fusion proteins and conjugates are increasingly used for immunosuppression, desensitization, ABO (blood group) incompatibility, antibody-mediated rejections and atypical haemolytic uremic syndrome. In this paper, we review the medical evidence available for biologicals used in SOT and the potential for improvement by the application of therapeutic drug monitoring (TDM) and model-informed precision dosing. Biologicals are used for off-label indications within the field of SOT, buil

The management of patients with kidney transplant failure (KTF) remains a complex process involving multiple stakeholders. A working group of the Transplantation Commission of the French-speaking Society of Nephrology, Dialysis and Transplantation (SFNDT) conducted a survey on the management of immunosuppressants (IS) after KTF among nephrologists at transplant centres and general nephrologists in France, Switzerland and Belgium between March and June 2023. We analysed 232 replies from 58 nephrologists at transplant centres and 174 general nephrologists, aged 43.6 (+10.6) years. In the first t

Autoantibodies to Angiotensin II type 1 receptor (AT1R) are associated with detrimental outcomes in organ transplants. However, reports showed that adsorption with latex beads reduced positive anti-AT1R antibodies, suggesting possible false reactivity. To investigate this conundrum, we studied 11 samples positive for AT1R antibodies with an ELISA kit before and after adsorption. Adsorption significantly reduced the measurable level of AT1R antibodies (28.3 ± 9.8 vs. 6.3 ± 3.0 U/ml, p < 0.001). AT1R antibodies were lower when post-adsorption serum was added back at 1:1 ratio to the neat serum c

BACKGROUND: Among heart transplant (HT) recipients who develop advanced graft dysfunction, cardiac re-transplantation may be considered. A smaller subset of patients will experience failure of their second allograft and undergo repeat re-transplantation. Outcomes among these individuals are not well-described.
METHODS: Adult and pediatric patients in the United Network for Organ Sharing (UNOS) registry who received HT between January 1, 1990 and December 31, 2020 were included.
RESULTS: Between 1990 and 2020, 90 individuals received a third HT and three underwent a fourth HT. Recipients were

BACKGROUND: Chronic lung allograft dysfunction (CLAD) is the leading cause of death among lung transplant recipients. Eosinophils, effector cells of type 2 immunity, are implicated in the pathobiology of many lung diseases, and prior studies suggest their presence associates with acute rejection or CLAD after lung transplantation.
RESEARCH QUESTION: Does histologic allograft injury or respiratory microbiology correlate with the presence of eosinophils in BAL fluid (BALF)? Does early posttransplant BALF eosinophilia associate with future CLAD development, including after adjustment for other k

Everolimus has recently been used to prevent graft rejection in liver transplantation and reduces the incidence of kidney dysfunction caused by calcineurin inhibitors. In this study, a population pharmacokinetic analysis was conducted to improve the individualization of everolimus therapy. Japanese post-liver transplant patients whose blood everolimus concentrations were measured between March 2018 and December 2020 were included in this study. A nonlinear mixed-effect modeling program was used to explore covariates that affect everolimus pharmacokinetics. Individual everolimus pharmacokinetic…

BACKGROUND: Caregivers play an important role in maintaining a functioning graft after pediatric liver transplantation. Therefore, the psychosocial factors of both patients and caregivers can have a critical impact on transplant outcomes. Appropriate assessment and recognition of these factors pre-transplantation may allow transplant teams to better define the needs of pediatric organ recipients and develop specific countermeasures, which may then contribute toward improving transplant outcomes.
METHODS: We studied 136 pediatric LT recipients followed at Texas Children's Hospital. Licensed so

Personalization of maintenance immunosuppression in kidney transplant recipients has long remained a goal in the transplant community. The recent addition of donor-derived cell-free DNA assays to detect allograft rejection and monitor allograft health may permit for reductions in maintenance immunosuppression in recipients with stable levels. Herein, we described 5 patients with stable donor-derived cell-free DNA levels who underwent reduction in maintenance immunosuppression without precipitation of clinical rejection, proteinuria, or de novo donor specific antibody formation.
