Acute management and secondary prophylaxis of esophageal variceal bleeding: a western Canadian survey.

BACKGROUND: Acute esophageal variceal bleeding (EVB) is a major cause of morbidity and mortality in patients with liver cirrhosis. Guidelines have been published in 1997; however, variability in the acute management and prevention of EVB rebleeding may occur. METHODS: Gastroenterologists in the provinces of British Columbia, Alberta, Manitoba and Saskatchewan were sent a self-reporting questionnaire. RESULTS: The response rate was 70.4% (86 of 122). Intravenous octreotide was recommended by 93% for EVB patients but the duration was variable. The preferred timing for endoscopy in suspected ac

Legal liabilities in research: early lessons from North America.

The legal risks associated with health research involving human subjects have been highlighted recently by a number of lawsuits launched against those involved in conducting and evaluating the research. Some of these cases have been fully addressed by the legal system, resulting in judgments that provide some guidance. The vast majority of cases have either settled before going to trial, or have not yet been addressed by the courts, leaving us to wonder what might have been and what guidance future cases may bring. What is striking about the lawsuits that have been commenced is the broad range

Results with titanium ossicular reconstruction prostheses.

OBJECTIVES/HYPOTHESIS: Despite the enthusiasm of recent short-term reviews, no center in the United States has published results meeting American Academy of Otolaryngology-Head and Neck Surgery guidelines with titanium-based prostheses. The purpose of the study was threefold. The first purpose was to review results with a titanium prosthesis system in cases meeting American Academy of Otolaryngology-Head and Neck Surgery reporting guidelines. The second was to compare these results with previously published results using non-titanium-based prostheses. The third was to examine the authors' resu

The New Zealand graduated driver licensing system: teenagers' attitudes towards and experiences with this car driver licensing system.

OBJECTIVES: This study examined the attitudes of teenagers towards the New Zealand graduated driver licensing system (GDLS), and the extent to which it affected them. METHOD: Teenagers, who are members of a longitudinal study of a birth cohort, were interviewed at 15 years of age when the GDLS was first introduced and before they had begun licensure, and again at 18 years of age after they had experience with this licensing system. RESULTS: At both ages the majority (over 70%) agreed with the driving restrictions imposed by this system. After experience with the restrictions, however, signif

Workforce guidelines, burnout, work engagement and the intention to leave: A cross-sectional study among Dutch emergency physicians.

BACKGROUND AND IMPORTANCE: Emergency medicine physicians (EMPs) work in a dynamic and challenging environment, with high risk of burnout. The prevalences of work engagement, burnout and the intention to leave among Dutch EMP have not been studied before. This study aims to evaluate the adherence to the Dutch Society of Emergency Physicians (DSEP/NVSHA) workforce recommendations, and the association between these recommendations and signs of burnout, work engagement and the intention to leave the profession. METHODS: Cross-sectional online survey based study among all Dutch EMPs in 2019. Two m

Stereotypes About Clients With Intellectual Disabilities in Addiction Care: Evidence From Flemish Health Care Providers.

BACKGROUND: Substance use among individuals with intellectual disabilities (ID) remains an underrecognized concern in addiction care. This population faces dual exclusion due to both their disability and their substance use. While clinical guidelines emphasize person-centered approaches, little is known about how addiction care professionals evaluate clients with ID and co‑occurring substance use problems. METHODS: Eighty-two addiction care professionals in Flanders (Belgium) completed an online study in which they rated 2 fictional client vignettes, 1 describing a client with ID and 1 withou

Effect of preventive dental treatment on caries incidence in patients receiving head and neck radiation therapy: a systematic review.

BACKGROUND: Head and neck cancer patients receiving radiotherapy (RT) are at high risk of radiation-induced dental caries (RIC). Preventive dental interventions are employed to mitigate this complication, though their relative effectiveness varies across the literature. OBJECTIVE: To systematically review the impact of preventive dental treatments on dental caries incidence among patients undergoing head and neck RT. METHODS: Following PRISMA 2020 guidelines, PubMed, Embase, and Cochrane Library were searched (2000–June 2025) for studies reporting post-RT caries outcomes. Eligible designs incl

The importance of social vulnerability and exclusion in cancer prognosis and survival: a scoping review.

BACKGROUND: Cancer prognosis and survival are determined not only by biological and clinical factors, but also by patients' social environments. Social vulnerability, including socioeconomic deprivation, limited education, weak social networks, and social exclusion, can critically influence diagnosis, treatment adherence, and outcomes. This scoping review aims to synthesize the current evidence on the impact of social vulnerability and exclusion on cancer outcomes, comprising prognosis, survival, quality of life, and related psychosocial factors. METHODS: Following the Preferred Reporting Ite

Quality of life for post gastric sleeve surgery patients: a cross sectional study.

BACKGROUND: The prevalence of the global obesity epidemic continues to rise, and sleeve gastrectomy is widely recognized as one of the most successful treatments for morbid obesity. It results in considerable and long-lasting weight loss, remission of obesity-related health conditions, and long-term improvement in quality of life (QoL). This study aimed to primarily examine the correlation between patients’ reported postoperative practices and their quality of life following sleeve gastrectomy. METHODS: A cross-sectional descriptive correlational study was performed at the bariatric surgery ou

Intensive lipid lowering therapy strategies in acute coronary syndrome patients in contemporary clinical practice - The benefit of the "strike early, strike strong" approach.

INTRODUCTION AND OBJECTIVES: Current guidelines recommend a stepwise, low-density lipoprotein cholesterol (LDL-C) guided lipid-lowering therapy (LLT) approach for patients after acute coronary syndrome (ACS). This study aimed to assess whether an early intensive "strike early, strike strong" strategy with upfront combination LLT improves LDL-C goal attainment. METHODS: This single-center retrospective study included 211 consecutive ACS patients admitted between January and October 2020, with a median follow-up of six months. Metabolic profiles were assessed during hospitalization and at follo

Assessing the impact, barriers and facilitators of peer-driven interventions on the prevention of mother-to-child transmission of HIV in low- and middle-income countries: A systematic review and meta-analysis.

INTRODUCTION: Peer interventions may improve retention across the prevention of mother-to-child transmission (PMTCT) cascade in low- and middle-income countries (LMICs), where poor adherence to antiretroviral therapy (ART) remains a major barrier. This systematic review and meta-analysis evaluated the impact of peer support interventions on PMTCT outcomes to inform scalable strategies. METHODS: Following PRISMA guidelines, we searched PubMed, Cochrane Library, Scopus and Google Scholar (2008-2025) using terms including 'peer mentors,' 'PMTCT' and 'LMICs.' Independent reviewers conducted data

Partial Weight-Bearing after Lower Extremity Surgery: A Review of Current Standards, Innovations, and Patient Compliance.

Partial weight bearing (PWB) is a cornerstone of post-operative rehabilitation after lower extremity surgery (LES), balancing mechanical stimulation needed for bone, tendon, and cartilage healing with protection against excessive load that precipitates malunion, non-union, or joint dehiscence. This narrative review synthesizes literature to clarify current indications for PWB, evaluate traditional and emerging methods for prescribing and monitoring load, and identify factors influencing patient adherence. Hip, knee, ankle, and foot procedures commonly warrant PWB, yet optimal targets and progr

Perioperative Anemia Investigation and Management in Vascular Surgery: A Single-Center Retrospective Service Evaluation.

Background Perioperative anemia is common in vascular surgery, but it is unclear how well national guidelines are followed in its investigation. This service evaluation examined how closely routine vascular surgical practice adhered to the British Society for Haematology (BSH) recommendations for investigating iron deficiency. Methods A retrospective service evaluation of consecutive adults undergoing vascular surgery at a single NHS center was performed. Electronic records were reviewed to determine the prevalence of preoperative anemia, adherence to the BSH recommendations for iron deficienc

Female allogeneic hematopoietic cell transplant survivors' perspectives on cervical cancer screening: a retrospective study.

PURPOSE: Female allogeneic hematopoietic cell transplantation (AlloHCT) survivors are at increased risk for human papillomavirus (HPV)-related malignancies due to immunosuppression. Guidelines recommend annual cervical cancer screening for AlloHCT survivors with a cervix aged 21-65. This study explored cervical cancer screening knowledge, behaviors, attitudes, experiences, and preferences-specifically provider-collected versus self-collected methods-and identified adherence barriers and facilitators. METHODS: We conducted a mixed-methods observational study of female AlloHCT survivors aged 21

Diagnostic accuracy of nanopore sequencing for the rapid diagnosis of extrapulmonary tuberculosis: a protocol for a systematic review and meta-analysis.

BACKGROUND: Extrapulmonary tuberculosis (EPTB) is a serious type of tuberculosis (TB) which can cause systemic clinical manifestations. Rapid diagnosis of EPTB for intervention is of great importance. Nanopore sequencing as a third-generation gene sequencing method is a new kind of rapid TB detection. Previous studies have shown that nanopore sequencing has higher diagnostic sensitivity and specificity for TB diagnosis compared with other diagnostic methods. The aim of this research is to develop a systematic review and meta-analysis protocol for assessing the accuracy of nanopore sequencing i

Successful implementation of managed alcohol programs in the San Francisco Bay Area during the COVID-19 crisis.

Background: The COVID-19 crisis presents new challenges and opportunities in managing alcohol use disorders, particularly for people unable to shelter in place due to homelessness or other reasons. Requiring abstinence for shelter engagement is impractical for many with severe alcohol use disorders and poses a modifiable barrier to self-isolation orders. Managed alcohol programs (MAPs) have successfully increased housing adherence for those with physical alcohol dependence in Canada, but to our knowledge, they have not been implemented in the United States. To avoid life-threatening alcohol wi
