Periprosthetic osseointegration fractures are infrequent and management is familiar.

AIMS: Osseointegrated prosthetic limbs allow better mobility than socket-mounted prosthetics for lower limb amputees. Fractures, however, can occur in the residual limb, but they have rarely been reported. Approximately 2% to 3% of amputees with socket-mounted prostheses may fracture within five years. This is the first study which directly addresses the risks and management of periprosthetic osseointegration fractures in amputees. METHODS: A retrospective review identified 518 osseointegration procedures which were undertaken in 458 patients between 2010 and 2018 for whom complete medical re

Does exercise reduce the burden of fractures? A review.

The null hypothesis that exercise has no effect on fracture rates in old age cannot be rejected on the basis of any published, randomized, prospective data. The view that exercise reduces the number of fractures is based on prospective and retrospective, observational cohort studies and case-control studies, all hypothesis-generating, not hypothesis-testing. Consistently replicated sampling bias may confirm the finding when evaluating other than randomized prospective studies. Better health, better muscle function, more muscle mass, better coordination may lead to exercise. The causal relation

Associations Between Time to Reduction and Complications in Patients With First-Time Total Hip Arthroplasty Dislocation: A Retrospective Cohort Study.

BACKGROUND: Total hip arthroplasty (THA) is one of the most frequently performed surgeries worldwide. Approximately 1% to 10% of patients experience one or multiple dislocations of their THA, a painful condition requiring urgent treatment. In Denmark, the usual treatment is closed reduction, typically performed in the operating theater under general anesthesia. However, logistical and clinical challenges can delay treatment in an often frail, elderly population. This study investigated whether time to reduction and type of anesthetics were associated with post-reduction length of stay (LOS) an

Inflammation-linked adipokine profiles associate with acute organ dysfunction after severe trauma.

INTRODUCTION: Polytrauma initiates an abrupt shift in immunometabolic signalling, but the role of circulating adipokines in acute post-traumatic organ dysfunction is not fully understood. Resistin and adiponectin are circulating immune-metabolic markers at the interface of inflammation, metabolic stress, and renal dysfunction, but their dynamic response to trauma and their clinical relevance remain uncertain. We therefore characterised serial changes in circulating adipokines during the first 10 days after severe multiple trauma and evaluated their associations with systemic inflammation, orga

Glucagon-like peptide-1 receptor agonists are associated with improved 90-day outcomes after total hip and knee arthroplasty : a systematic review and meta-analysis.

AIMS: Obesity and diabetes are risk factors for developing osteoarthritis and associated with worse outcomes following total hip (THA) and total knee arthroplasty (TKA). Glucagon-like peptide-1 receptor agonists (GLP-1as) improve glycaemic control and promote weight loss. The aim of this study was to assess the impact of GLP-1a treatment on 90-day surgical complications following THA and TKA. METHODS: A systematic review was undertaken according to PRISMA guidelines. Studies were included if they reported at least one outcome of interest following primary THA or TKA. In all the studies which

Correlation Between Changes in Subchondral Bone Density Distribution and Clinical Outcomes at 2 Years After Opening-Wedge High Tibial Osteotomy.

BACKGROUND: High tibial osteotomy (HTO) for medial knee osteoarthritis corrects lower limb alignment, redistributing knee joint stress from the medial to the lateral compartment and improving clinical outcomes. Subchondral bone density reflects joint stress distribution. While redistribution of subchondral bone density after HTO has been reported, the relationship between computed tomography-osteoabsorptiometry (CT OAM)-derived changes in subchondral bone density distribution and postoperative clinical outcomes has not been well-established. PURPOSE: To investigate the relationship between su

Abaloparatide Increases Distal Femur Bone Mineral Density Post-Total Knee Arthroplasty.

BACKGROUND: Osteoporosis is common in joint replacement patients and increases adverse outcome risk. Data indicate that total knee arthroplasty (TKA) causes distal femur bone loss. We hypothesized that abaloparatide (ABL) would mitigate this loss. The study's purpose was to evaluate the effect of ABL initiated before TKA on distal femur bone mineral density (BMD) in osteoporotic patients. METHODS: TKA candidates age ≥55 years were enrolled in this open-label, prospective, observational 18-month study. Those with clinical osteoporosis, defined as lowest T-score ≤ -2.5 or < -1.0 if prior low-tr

Superficial femoral artery thrombosis after PFNA in an intertrochanteric fracture patient with sarcopenia and copd: a rare case report.

BACKGROUND: Intertrochanteric fractures in elderly patients are prevalent in trauma orthopedics and constitute a significant cause of mortality among the elderly population. Proximal Femoral Nail Antirotation (PFNA) is recognized as the standard treatment modality for these fractures. While the prevention of lower limb venous thrombosis is a major clinical focus, postoperative femoral artery thrombosis remains exceedingly rare; however, it can lead to severe disability or even death if not managed promptly. METHODS: This case report discusses an 83-year-old male patient with sarcopenia and ch

Increased Anticoagulant Requirement in Total Hip Replacement Patients Post-COVID: A Comparative Study in the Indian Population.

INTRODUCTION: India's healthcare landscape features a blend of modern medicine and alternative systems, which gained widespread use during the COVID-19 pandemic. The undocumented use of traditional remedies and unregulated substances limited formal diagnosis and treatment records, complicating post-operative risk assessment. The pandemic has had lasting effects on vascular and coagulation systems. This study aims to evaluate and compare low molecular weight heparin (LMWH) requirements in patients undergoing total hip replacement (THR) in the pre-and post-COVID eras. MATERIALS AND METHODS: A r

Outcomes following total shoulder arthroplasty in patients with obstructive sleep apnea.

BACKGROUND: Obstructive sleep apnea (OSA) is a known risk factor for various medical complications including cardiovascular disease, cognitive dysfunction, and respiratory failure. With global prevalence of OSA on the rise, understanding its impact on common orthopedic procedures has become increasingly important. Although the impact of OSA has been studied in patients undergoing total hip arthroplasty and cervicothoracic spinal surgery, its relationship with total shoulder arthroplasty (TSA) is still unclear. Therefore, the purpose of this study was to evaluate the relationship of complicatio
