Methodological Development and Assessing Prescribing Determinants Through Cumulative Drug Exposure in Hospitalized Patients: Proof-of-Concept Retrospective Study.

BackgroundPreventing adverse drug reactions requires accurate monitoring of drug exposure throughout patient care. Conventional metrics, measured at admission or discharge, fail to capture the dynamic and cumulative nature of drug burden during hospitalization. Improving exposure assessment is essential to support clinical decision-making and medication safety. Clinical data warehouses (CDWs), which integrate detailed drug administration records, enable the retrospective reuse of hospital data to develop more granular and dynamic measures of in-hospital drug exposure.ObjectiveThis exploratory

Patient-Specific Mobilization: A Structured, Systemwide Model of Mobility Care.

Hospital-associated functional decline resulting from immobility and prolonged bed rest is a widespread yet underrecognized complication in acute care. Despite strong evidence linking immobility directly to severe in-hospital complications and significant postdischarge impairments - including reduced independence, diminished quality of life, and impaired ability to perform activities of daily living (ADLs) - many U.S. hospitals lack standardized mobility care programs. Mobility practices vary widely and are typically driven by individual clinician knowledge, institutional culture, and informal

The Relationship Between Kangaroo Mother Care Duration and Clinical Outcomes in Preterm and Low Birth-Weight Infants: A Dose-Response Meta-Analysis.

BACKGROUND: Determining the optimal daily Kangaroo Mother Care (KMC) duration is important for improving outcomes in preterm and low birth-weight (LBW) infants, but existing evidence is mostly as broad categories rather than a continuous dose-response relationship. PURPOSE: To examine the dose-response association between daily KMC duration and 4 outcomes in preterm/LBW infants: weight gain and hospital stay (primary), breastfeeding (including direct feeding and receipt of expressed human milk) and hypothermia (secondary). DATA SOURCES: PubMed, Web of Science, Cochrane Library, China Biology

A Retrospective Chart Review of Uninsured Encounters in a Primary Care Hospital in Japan.

Introduction Lack of health insurance is known to delay care-seeking and contribute to preventable morbidity. Although Japan has established a universal health insurance system, more than 10% of households have unpaid insurance premiums, and approximately 2.5% possess insurance certificates with restricted or no validity. Under these circumstances, preventable deaths attributable to delayed medical care for financial reasons have been reported. Individuals facing unstable employment or job loss are particularly vulnerable to premium non-payment, even when willingness to pay exists. The actual

Outcomes and Risk Factors for Bacteriobilia in Cancer Patients Undergoing Endoscopic Retrograde Cholangiopancreatography of Malignant Biliary Obstruction at a Teaching Hospital in Northern India.

BACKGROUND: Malignant biliary obstruction (MBO) is a common complication in cases of locally or systemically advanced gastrointestinal malignancies. Placing a biliary stent is a method to relieve blockages in the biliary tract with fewer chances of bacteriobilia after endoscopic retrograde cholangiopancreatography (ERCP). MATERIALS AND METHODS: This retrospective study was carried out in the Department of Microbiology at a teaching hospital. The hospital information system and the case files for the ward data entry were used to collect the clinical data of every patient. Records of management

Hospital-treated infectious diseases and polygenic susceptibility in relation to heart failure and cardiac remodelling: evidence from the UK Biobank and ARIC cohorts.

AIMS: Infectious diseases (IDs) are increasingly recognized as precipitants of heart failure (HF). However, the impact of diverse pathogens and genetic susceptibility on HF and cardiac remodelling remains unclear. METHODS AND RESULTS: We analysed 480 154 UK Biobank (UKB) participants and 9816 Atherosclerosis Risk in Communities (ARIC) participants free of HF at baseline. Hospital-treated IDs were ascertained with 931 ICD-10 codes and modelled as time-varying exposures. Incident HF and HF with preserved (HFpEF) and reduced ejection fraction (HFrEF) were identified through hospital records and …

Factors associated with prolonged viral shedding in COVID-19 infection: A retrospective cohort from Karachi, Pakistan.

BACKGROUND: The implications of prolonged viral shedding in COVID-19 are of major public health concern. There are several studies elucidating the impact on transmission; there is a lack of data on outcomes. The objective of this study was to identify factors associated with prolonged viral shedding and its impact on disease outcomes in COVID-19. METHODS: This retrospective cohort was conducted on hospitalized throat swab-PCR confirmed COVID-19 patients admitted between March 01, 2020, and June 07, 2020, at the Aga Khan University Hospital in Karachi, Pakistan. Validation of a Fall Predictive Model for Inpatients in Japanese Long Term Care Hospitals.

Background: The Saga Falls Risk Model 2 (SFRM2) is a simplified fall prediction model that we recently developed. It uses eight items that are easy to assess at the time of admission to an acute care hospital. However, patients in long-term care hospitals have poor activities of daily living and a high risk of falls compared to those in acute care hospitals. Although effective fall predictive models exist for long-term care hospitals, their accuracy remains suboptimal. This study aimed to validate the SFRM2 for predicting falls in long-term care hospital patients. Methods: This multicenter ret

Baseline predictors of in-hospital mortality among patients with chronic kidney disease admitted to the emergency department.

BACKGROUND: Chronic kidney disease (CKD) contributes significantly to emergency department (ED) presentations in low- and middle-income countries. These patients frequently have multiple comorbidities and face high in-hospital mortality. However, limited data exist on early predictors of mortality at ED admission. Identifying key clinical and laboratory features associated with adverse outcomes may support timely risk stratification and targeted interventions for acutely ill CKD patients. AIM: To identify baseline predictors of in-hospital mortality in adult Indian patients with CKD admitted

Impact of Diagnosis-Intervention Packet (DIP) reforms on inpatient services for low-income populations in central China: A multi-stage interrupted time-series analysis.

As global healthcare costs continue to rise, concerns about health equity have become increasingly prominent. In response, China introduced the Diagnosis-Intervention Packet (DIP) reform in 2021 to optimize healthcare resource allocation and control costs. While the reform has been widely discussed in terms of its overall cost-control effects, its heterogeneous impact on low-income populations, especially across different hospital tiers, remains unclear. This study aims to fill this gap by examining the differentiated impact of the DIP reform on low-income patients' inpatient service utilizati

Effects of Motor-Evoked Potential Monitoring During Spine Surgery on the Length of Hospital Stay and Postoperative Acute Pain.

BACKGROUND: In spine surgery, intraoperative motor-evoked potential (MEP) monitoring is a crucial safety measure, helping to prevent and detect nerve injuries early during surgical operations. When performing intraoperative MEP monitoring, anesthesia management with total intravenous anesthesia (TIVA) is recommended. This study aimed to determine whether the introduction of MEP monitoring and TIVA contributes to a reduction in the length of hospital stay and postoperative acute pain in patients undergoing spinal surgery under general anesthesia. METHODS: One hundred and fifty-six patients, wh
