Overview of bronchiolitis.

Respiratory syncytial virus RSV, a nonsegmented, single stranded ribonucleic acid virus, infects one-half of all infants within the first year of life. Respiratory syncytial virus possesses pathogenetic qualities that may be attributed to the interplay of viral and host-specific factors including virus strains of different virulence, size of the inoculum, family history of asthma or airway hyper-reactivity and immunologic anomalies of the host. Inflammatory cell recruitment and activation occur in response to RSV infection of epithelial cells. Epithelial cells initiate the inflammatory respons

Consanguinity and apnea of prematurity.

Consanguinity, marriage between relatives, has been associated with perinatal mortality and morbidity. Apnea of prematurity is defined as the cessation of breathing for longer than 20 seconds or that of any duration if accompanied by cyanosis and sinus bradycardia, for infants born before 37 weeks of gestation. The objective of the study was to examine the association between consanguinity and apnea of prematurity in Greater Beirut, an area having a relatively high prevalence rate of consanguineous marriages. The study was cross-sectional. Between September 1, 1998, and March 31, 2001, 21723 n

[No asymmetry of head rotation and shape found in three quarters of 2-3 year olds exhibiting postural preference in infancy].

OBJECTIVE: Assessment of the course of signs of asymmetry in toddlers who had a preferential posture as infants. DESIGN: Descriptive. METHOD: Children who at the age of 1-6 months showed a preferential posture (n = 623) and at the age of 7-14 months displayed asymmetry of the shape and/or the rotation of the head (n = 259 of 468 children examined; 55%), were invited in 1997/'98 at the age of 2-3 years by 71 physicians of Infant Health Care Centres in the Netherlands. These physicians examined 129 children for asymmetry of the rotation and shape of the head. The parents of children with asymm

Magnesium sulfate tocolysis and risk of neonatal death.

OBJECTIVE: Our purpose was to evaluate the association between in utero exposure to magnesium sulfate and neonatal death. STUDY DESIGN: Case-control comparison was performed of singleton infants who died at 0 to 28 days and control infants who survived to age 3 years and did not have disabling cerebral palsy. All subjects were born between 1983 and 1985 to mothers resident in one of four northern California counties. RESULTS: Magnesium sulfate tocolysis was associated with a decreased risk of neonatal mortality (odds ratio 0.25, 95% confidence interval 0.6 to 1.1). Adjustment for birth weigh

Effects of suboptimal intra-uterine growth on preterm infants between 30 and 32 weeks' gestation.

OBJECTIVE: To investigate the affects of suboptimal intra-uterine growth on the outcome of low-birth-weight (LBW) infants. DESIGN: Prospective observational study. SETTING: Neonatal unit of a tertiary care hospital. PATIENTS: A total of 104 LBW infants with a gestation of 30-32 weeks were selected from a larger cohort that had previously been studied to ascertain the prevalence of respiratory distress syndrome (RDS) and periventricular-intraventricular haemorrhage (PV-IVH). OUTCOME MEASURES: Multivariate analysis was used to examine the association between RDS, PV-IVH and death, and the ad

Development of a practical screening tool to identify preterm, low-birthweight neonates in Ethiopia.

Preterm, low-birthweight (LBW) newborn infants are at high risk of neonatal mortality and morbidity and need early referral for special paediatric care. In developing countries, birthweight and gestational age often cannot be measured and a practical screening tool based on surrogate neonatal body measurements to identify high-risk infants would be very useful. We studied a consecutive series of 843 singleton infants born at a referral hospital in Addis Ababa, Ethiopia. Gestational age, birthweight, and four body measurements (chest, head, and mid-arm circumferences and length) were accurately

Idiopathic intestinal perforations in the newborn: an increasingly common entity.

Between 1982 and 1987 seven neonates ranging in age from 24 hours to 1 week were treated for idiopathic intestinal perforations at the Montefiore Medical Center, Albert Einstein College of Medicine. Four of the infants were born prematurely; three were full term. Five were being treated in an intensive care nursery when the perforation was diagnosed. All infants whose perforations were due to necrotizing enterocolitis, appendicitis, Hirschsprung's disease, meconium ileus, intestinal atresias, or drug therapy are excluded from the series. None of the infants had associated anomalies. The sites

Current results in repair of esophageal atresia with tracheoesophageal fistula using physiologic status as a guide to therapy.

Since 1966, 118 infants with esophageal atresia associated with tracheoesophageal atresia (Vogt-Gross Type C) have been operated upon with an overall survival rate of 90% at our institution. Since Haight's first survivor in 1941, argument has continued about the relative merits of immediate repair, delayed primary repair, and staged repair, and the criteria for selection of each approach. The Waterston classification served as the foundation for selection of surgical management from 1966 to 1982. Since 1982, physiologic status has been used as the sole basis for surgical management without reg

[Detection and monitoring of high-risk children].

The authors first describe problems related to the lowering of infant mortality and deal with the terms "high-risk factor" and "high-risk baby". As a result of the dangers presented by prenatal and perinatal noxae the need arises for low-risk births and optimal care of new-born babies. The public health system must be suitably organized so that babies with case history details can be supervised and others with cerebral motor disturbances without pathological data can be registered in the case history. In the registration of the latter group child-welfare centers have an important role to play.

Is small for gestational age associated with reduced oxygen saturation to FiO2 ratio at 36 weeks postmenstrual age in preterm infants, independent of bronchopulmonary dysplasia? A retrospective Italian two-center cohort study.

UNLABELLED: The overrepresentation of small-for-gestational-age (SGA) preterm infants among those with bronchopulmonary dysplasia (BPD) poses a challenge in quantifying the extent to which SGA independently affects respiratory outcomes. This two-center study aims to evaluate whether SGA status at birth is associated with reduced oxygen saturation (SpO2) to fraction inspired oxygen (FiO2) ratio (SFR) at 36 weeks (W) in preterm infants, independent of BPD. We retrospectively reviewed clinical data of preterm infants born between 24+0/7 and 30+6/7W of gestational age (GA). SGA was defined as a bi

Routine probiotics and outcomes in infants < 29 weeks: a 13-year single-centre cohort with propensity-weighted sensitivity analyses.

UNLABELLED: Probiotic supplementation in preterm infants has been associated with reduced necrotising enterocolitis (NEC) and mortality, but reported effects vary across studies due to heterogeneity in populations, probiotic products, and co-interventions, with limited representation of extremely preterm infants. We performed a single-centre retrospective cohort study of infants born at < 29 weeks' gestation admitted for intensive care between 2012 and 2024 using electronic patient records. After excluding infants who died within the first 7 days and other predefined exclusions, 581 infants we

[The possible impact of the COVID-19 lockdown measures on the perinatal/neonatal outcome - a data analysis of the years 2019, 2020, 2021].

This study aims to contribute to investigating the potential impact of the SARS-CoV-2 pandemic and lockdown measures on perinatal care.A statistical analysis of diagnoses (ICD-10) of 21,393 newborns born between 2019 and 2021 from three Level 1 perinatal centers was conducted. During the study period, a statistical decrease in full-term births and very early preterm births (VLBW), coupled with an increase in infants born between 28 and 37.0 weeks of gestation, was observed in 2020 and 2021. Likewise, a significantly higher number of infants were born after 42.0 weeks of gestation. Neonatal mor

From vulnerability to resilience: unraveling the role of oxidative stress in preterm brain injury.

Neonatal medicine advancements have significantly improved preterm infants’ survival rates, yet optimizing their neurodevelopmental outcomes, is challenging. A central factor in these challenges is oxidative stress (OS), arising from an imbalance between free radicals (FRs) production and antioxidant defense mechanisms. In preterm infants, OS is particularly detrimental due to their underdeveloped antioxidant systems, high brain metabolic demands, immature blood-brain barrier (BBB), and increased vulnerability to inflammation and hypoxia. This review focuses on the multifaceted aspects of OS c

Severe Pertussis Infection With Hyperleukocytosis in a 10-Month-Old Unvaccinated Amish Female: A Case Report.

Bordetella pertussis (B. pertussis) commonly infects individuals of all ages. However, pertussis, the disease caused by B. pertussis infection, is most severe in young infants. Severe pertussis, defined by the presence of refractory hypoxemia, pneumonia, cardiogenic shock, and hyperleukocytosis, is associated with significant morbidity and mortality. Both hyperleukocytosis and pulmonary hypertension have been found to be predictive of mortality in young infants. Leukoreductive strategies such as leukapheresis and exchange transfusion have been employed to treat these complications. Pulmonary h

Screening of hydrocephalus in infants using either WHO or population-based head circumference reference charts.

AIM: The aim was to compare the performances of the World Health Organization (WHO) and population-based (PB) references in the screening for hydrocephalus in infants aged <2 years. METHODS: We collected 341 longitudinal head circumference (HC) measurements of hydrocephalic infants and 120 181 measurements of 15 145 healthy infants from primary care. The measurements were converted into z-scores, and a new screening parameter, change in HC standard deviation score (SDS) over time (ΔHC SDS), was calculated. Comparisons were made using receiver operating characteristics analysis and linear mixe

Preterm birth and sustained inflammation: consequences for the neonate.

Almost half of all preterm births are caused or triggered by an inflammatory process at the feto-maternal interface resulting in preterm labor or rupture of membranes with or without chorioamnionitis ("first inflammatory hit"). Preterm babies have highly vulnerable body surfaces and immature organ systems. They are postnatally confronted with a drastically altered antigen exposure including hospital-specific microbes, artificial devices, drugs, nutritional antigens, and hypoxia or hyperoxia ("second inflammatory hit"). This is of particular importance to extremely preterm infants born before 2
