Role of Adenosine Deaminase in Common Chronic ENT Infections.

INTODUCTION: Adenosine Deaminase (ADA) has been suggested to be an important enzyme which is associated with the cell mediated immunity. But its clinical significance in ENT infections needs to be correlated. AIM: To evaluate the role of serum adenosine deaminase level estimation in common chronic ENT infections. MATERIALS AND METHODS: This was a prospective randomized study. The subjects were divided into 4 groups. Group A consisted of 25 normal healthy individuals who served as the controls. Group B consisted of 25 patients, who were clinically diagnosed as chronic tonsillitis. Group C con…

Concurrent malaria and arbovirus infections in Kedougou, southeastern Senegal.

BACKGROUND: Malaria is one of the leading causes of acute febrile illness (AFI) in Africa. With the advent of malaria rapid diagnostic tests, misdiagnosis and co-morbidity with other diseases has been highlighted by an increasing number of studies. Although arboviral infections and malaria are both vector-borne diseases and often have an overlapping geographic distribution in sub-Saharan Africa, information about their incidence rates and concurrent infections is scarce. METHODS: From July 2009 to March 2013 patients from seven healthcare facilities of the Kedougou region presenting with AFI …

Treatment of perinatal viral infections to improve neurologic outcomes.

Viral infections in the fetus or newborn often involve the central nervous system (CNS) and can lead to significant morbidity and mortality. Substantial progress has been made in identifying interventions decreasing adverse neurodevelopmental outcomes in this population. This review highlights progress in treatment of important viruses affecting the CNS in these susceptible hosts, focusing on herpes simplex virus (HSV), cytomegalovirus (CMV), human immunodeficiency virus (HIV), and enteroviruses. The observation that high-dose acyclovir improves mortality in neonatal HSV disease culminated dec…

Congenital and perinatally-acquired infections in resource-constrained settings.

INTRODUCTION: Congenital and perinatal infections are a leading cause of neonatal and infant morbidity and mortality. Maternal screening, vaccines or treatment where available, constitute effective prevention strategies to reduce the burden of these diseases. Data on the burden of congenital and perinatal infections are very limited for low and middle-income regions. AREAS COVERED: This review aims to summarize the burden of congenital and perinatal infections and the main challenges for their control in resource-limited settings. Articles were identified through the main electronic databases…

Bloodstream Infections in Hospitalized Children: Epidemiology and Antimicrobial Susceptibilities.

BACKGROUND: Bloodstream infection is a major cause of morbidity and mortality. Much of our understanding of the epidemiology and resistance patterns of bloodstream infections comes from studies of hospitalized adults. METHODS: We evaluated the epidemiology and antimicrobial resistance of bloodstream infections occurring during an 11-year period in a large, tertiary care children's hospital in the US. All positive blood cultures were identified retrospectively from clinical microbiology laboratory records. We excluded repeat positive cultures with the same organism from the same patient within…

Purpura fulminans secondary to rickettsial infections: A case series.

Purpura fulminans (PF) is a descriptive term used to describe a heterogeneous group of disorders characterized by rapidly progressive purpuric lesions that may develop into extensive areas of skin necrosis, and peripheral gangrene. This rare disorder is associated with laboratory evidence of consumptive coagulopathy and is often fatal. PF is usually associated with many infections, most notably with meningococcal, staphylococcal, and streptococcal infections. However, there are very few reports of this entity with spotted fever and scrub typhus from India. Rickettsial infections are an underdi…

Pathological Response of Northern Bobwhites to Oxyspirura petrowi Infections.

The effects of Oxyspirura petrowi infections in northern bobwhites ( Colinus virginianus ) are not well understood. While studies have reported O. petrowi infections, none has histopathologically examined the eye surface and intraorbital glands to assess cellular-level impacts associated with infection. This study is the first to document the histopathology associated with O. petrowi infections. Oxyspirura petrowi occurred on the eye surface as well as in the conjunctiva, lacrimal ducts, lacrimal glands, and Harderian glands. Histopathology showed infections of O. petrowi caused cellular damag…

[Filamentous fungal infections in immunosuppressed patients: prophylaxis and treatment].

Although the incidence of invasive aspergillosis has decreased in haematologic patients and solid organ transplant recipients due to the use of prophylaxis; aspergillosis has emerged in other populations undergoing immunosuppressive drugs where prophylaxis is not well defined presenting different clinical patterns. Voriconazole is the gold standard in the treatment of aspergillosis and probably combined therapy, with voriconazole plus anidulafungin, could have a role in the initial management of the infection.

Interferon at the crossroads of allergy and viral infections.

IFN-α/β was first described as a potent inhibitor of viral replication, but it is now appreciated that IFN signaling plays a pleiotropic role in regulating peripheral T cell functions. Recently, IFN-α/β was shown to block human Th2 development by suppressing the transcription factor GATA3. This effect is consistent with the role for IFN-α/β in suppressing allergic inflammatory processes by blocking granulocyte activation and IL-4-mediated B cell isotype switching to IgE. With the consideration of recent studies demonstrating a defect in IFN-α/β secretion in DCs and epithelial cells from indivi

Canine and human Dirofilaria infections in the Balkan Peninsula.

Dirofilaria immitis and Dirofilaria repens infections are mosquito-borne diseases, mainly of dogs. Both parasites are zoonotic, and they sometimes cause serious infections in humans. The aim of this short review was to examine the situation in the Balkan Peninsula, from where it is not always easy to obtain suitable data, often reported in journals and other publications difficult to be retrieved and with poor or no visibility. The review included data from international and regional literature, doctoral theses, and conference proceedings.

Risk factors for surgical site infections following spinal surgery.

The aim of this study was to determine the risk factors for postoperative infection after spinal surgery, in order to prevent its occurrence. We searched the Pubmed, Embase, and Cochrane library databases, and identified 25 case-control studies. The pooled results revealed that the major factors associated with infection were diabetes mellitus (odds ratio [OR] 2.04; 95% confidence interval [CI] 1.69-2.46), obesity (OR 2.13; 95% CI 1.55-2.93), smoking (OR 1.17; 95% CI 1.03-1.32), urinary tract infection (OR 3.19; 95% CI 1.68-6.06), hypertension (OR 1.67; 95% CI 1.26-2.22), transfusion (OR 3.64;

Burden of serious fungal infections in the Czech Republic.

We have estimated the number of serious fungal infections in the Czech Republic. All published epidemiology papers reporting Czech fungal infection rates were identified. Where no data existed, we used specific populations at risk and fungal infection frequencies in those populations. Population statistics were obtained from the 2011 Census data, prevalence and incidence data for at-risk conditions were obtained from publicly accessible healthcare statistics and relevant surveys. We estimate that 152,840 Czech women suffer with recurrent vaginal thrush. Allergic bronchopulmonary aspergillosis …

Burden of serious fungal infections in Trinidad and Tobago.

The information on the prevalence of fungal infections in the Caribbean region including Trinidad and Tobago (population 1,339,000 million) is scanty. Tinea capitis is common in children, being predominant in those of African descent, with no definitive estimate. Asthma is also common affecting 77,000-139,000 adults with an estimated 1927-3491 affected by allergic bronchopulmonary aspergillosis (ABPA) and 2544-4608 with severe asthma and fungal sensitisation (SAFS). An estimated 23,763 women have ≥4 attacks of vaginal candidiasis annually. Among the estimated 14,000 HIV-infected patients, 750 …

Update on prevention and treatment of intestinal helminth infections.

Intestinal helminth infections are some of world's most common tropical diseases and cause significant impairments in pediatric growth and cognitive impairment as well as maternal health, particularly in areas lacking adequate access to safe water or sanitation. Routine mass drug administration (MDA) of anthelminthic medications to children living in endemic areas and interventions to improve water hygiene and sanitation form the basis of current control efforts. We review recent evidence on the effectiveness of these approaches and outline the limitations of MDA, including poor cure rates aga…

Plasmodium knowlesi malaria: Overview Focussing on Travel-Associated Infections.

In 2004, Plasmodium knowlesi was first recognised as a relevant cause of human malaria in Southeast Asia. Since then, P. knowlesi has been described from all Southeast Asian countries except Laos and has become well-established as the fifth human malaria parasite and the first significant zoonotic Plasmodium species. As countries endemic for P. knowlesi malaria are among the most popular and most highly visited international destinations, travel medicine experts should be aware about disease and risks including prophylactic and therapeutic measures. Between 2005 and 2012, 15 cases of P. knowle

[New approaches to the treatment of the flavivirus infections].

Since spontaneous mutagenesis and quasi-species rearrangements of the RNA-containing viruses, as well as an absence of both viral and cellular RNA reparation systems, causes resistance to originally effective antiviral drugs, combination therapy with nucleoside and non-nucleoside inhibitors of the viral enzymes in combination with immunomodulators is recommended. The use of specific immunoglobulins does not result in complete elimination of the flaviviruses but rather in possible antibody-dependent enhancement of the flavivirus infection by means of increased penetration of complexes of virion

Herpes Simplex Virus Infections of the Central Nervous System.

PURPOSE OF THE REVIEW: This article summarizes knowledge of herpes simplex virus (HSV) infections of the central nervous system (CNS). Disease pathogenesis, detection of DNA polymerase chain reaction (PCR) for diagnosis and prognosis, and approaches to therapy warrant consideration. RECENT FINDINGS: HSV infection of the CNS is one of few treatable viral diseases. Clinical trials indicate that outcome following neonatal herpes simplex virus type 2 (HSV-2) infections of the CNS is significantly improved when 6 months of suppressive oral acyclovir therapy follows IV antiviral therapy. In contras…
