Treatment for Acute Stage Complex Regional Pain Syndrome Type II with Polydeoxyribonucleotide Injection.

Complex regional pain syndrome (CRPS) type II is a syndrome that develops after nerve injury. Symptoms may be severe, and vary depending on the degree of sympathetic nerve involvement. As yet, there is no satisfactory treatment. We report the case of a female patient who had an L5 left transverse process fracture and an S2 body fracture, who developed symptoms of CRPS type II in her left lower leg that were aggravated during ambulation in spite of absolute bed rest for one month after the trauma. Several treatments, including bed rest, medication, and numerous nerve blocks were attempted, but

Rare cause of paraparesis: bilateral obturator neuropathy after hysterosalpingectomy.

Bilateral obturator nerve injury during pelvic surgery is an infrequent cause of lower limb paraparesis. We report the case of a 45-year-old woman with a large uterine leiomyoma who underwent simple total hysterectomy and bilateral salpingectomy. At 24 h after the surgery, the patient noticed loss of muscle strength when adducting both legs. She had no problem with other movements and no sensory or sphincter abnormalities. Neurological examination confirmed that there was loss of strength only in the adductor muscles, with preserved sensory function and reflexes, suggesting bilateral obturator

[Myxoid liposarcoma of the lumbosacral spine. Clinical case report].

BACKGROUND: liposarcoma is a malignant tumor of soft tissue and the most common sarcoma in the adult life. The myxoid liposarcoma accounts for 30 to 35 % of all liposarcomas. It is commonly seen on the thighs. Only two cases in which the liposarcoma was found in the vertebral column have been reported. CLINICAL CASE: this is a 28 year old woman with symptoms of spinal cord injury by compression (tottering gait, weakness 4/5 and decreased sensibility in right leg). The MRI showed a right paravertebral multinodular mass from L4 to S2, with solid and cystic aspects, following the S1 and S2 inter

Gas gangrene in orthopaedic patients.

Clostridial myonecrosis is most often seen in settings of trauma, surgery, malignancy, and other underlying immunocompromised conditions. Since 1953 cases of gas gangrene have been reported in orthopaedic patients including open fractures, closed fractures, and orthopaedic surgeries. We present a case of 55-year-old obese woman who developed rapidly progressive gas gangrene in her right leg accompanied by tibial plateau fracture without skin lacerations. She was diagnosed with clostridial myonecrosis and above-the-knee amputation was carried out. This patient made full recovery within three we

A Method for the Autonomous Control of Lower Limb Exo-skeletons for Persons with Paraplegia.

Efforts have recently been reported by several research groups on the development of computer-controlled lower limb orthoses to enable legged locomotion in persons with paraplegia. Such systems must employ a control framework that provides essential movements to the paraplegic user (i.e., sitting, standing, and walking), and ideally enable the user to autonomously command these various movements in a safe, reliable, and intuitive manner. This paper describes a control method that enables a paraplegic user to perform sitting, standing, and walking movements, which are commanded based on postura

Limb immobilization and corticobasal syndrome.

BACKGROUND: Recently, we evaluated two patients with corticobasal syndrome (CBS) who reported symptom onset after limb immobilization. Our objective was to investigate the association between trauma, immobilization and CBS. METHODS: The charts of forty-four consecutive CBS patients seen in the Mayo Clinic Alzheimer Disease Research Center were reviewed with attention to trauma and limb immobilization. RESULTS: 10 CBS patients (23%) had immobilization or trauma on the most affected limb preceding the onset or acceleration of symptoms. The median age at onset was 61. Six patients manifested th

Timing in the absence of supraspinal input III: regularly spaced cutaneous stimulation prevents and reverses the spinal learning deficit produced by peripheral inflammation.

In the absence of brain input, spinal systems can adapt to new environmental relations. For example, spinally transected rats given a legshock each time the leg is extended exhibit a progressive increase in flexion duration that minimizes net shock exposure, a simple form of instrumental learning. This capacity for learning is modulated by prior stimulation; both variable shock and inflammation produce a lasting inhibition of learning. An extended exposure to fixed spaced shock has no adverse effect on learning and opposes the consequences of variable shock. The present studies expand on these

An infant surgical table for laser photocoagulation: ergonomic improvement analysis.

Current methods of treatment for retinopathy of prematurity, using laser photocoagulation, require surgeons to assume awkward standing positions, which can result in occupational injury. A new infant surgical table was designed for improving this surgical procedure. To quantify its benefits, an ergonomic comparison of the standard and modified procedures was carried out, using specialized checklists, Nordic Musculoskeletal Questionnaires, and analysis of videotaped procedures using an Ovako Working Posture Analysing System method. Analysis of the typical laser photocoagulation procedure reveal

Snare technique for the placement of a peripherally inserted central catheter in the neonatal and pediatric patient.

PURPOSE: The placement of a peripherally inserted central catheter (PICC) for neonatal or pediatric patients may at times be technically challenging. We describe an alternate method of placing a PICC in neonatal and pediatric patients with difficult venous access. METHODS: An Amplatz gooseneck snare is advanced from the femoral vein to the basilic vein, as a target for puncture and guidewire positioning, allowing PICC placement. Between 1999 and 2006, 44 PICCs were placed in the manner described. The medical records were reviewed allowing for up to 7 yrs of patient follow-up. RESULTS: Ninety

A preliminary study into rider asymmetry within equitation.

Rider asymmetry is anecdotally associated with underperformance and injury. In this study, rider axial rotation (AR) and shoulder angle displacement (SAD) were measured using video analysis, and leg length inequality (LLI) was measured to assess any correlation between LLI, AR and SAD. Two video cameras, recording transverse and sagittal views, simultaneously filmed each of 17 female riders in walk, trot, and left and right canter. The rider's shoulder angles relative to the cranial caudal line were measured at each limb impact for a complete stride cycle. All riders exhibited left AR and a gr

Ethnicity and other risk factors for acute lower limb cellulitis: a U.K.-based prospective case-control study.

BACKGROUND: Acute lower limb cellulitis is a common yet potentially serious condition. Previous studies have identified risk factors in the non-U.K. population. Ethnicity has been postulated as a possible risk factor but has not previously been investigated. OBJECTIVES: To identify risk factors for acute lower limb cellulitis in the U.K. population. METHODS: One hundred and fifty consecutive patients with cellulitis requiring hospital admission and 300 controls were recruited to this prospective case-control study. Controls were matched for age and sex. RESULTS: Strongly predictive risk fac

Endovenous laser ablation of varicose veins: review of current technologies and clinical outcome.

Symptomatic lower extremity varicose veins represent one of the most common vascular conditions in the adult population. Associated symptoms ranged from mild conditions such as fatigue, heaviness, and itching to more serious conditions such as skin discoloration and leg ulceration. The predominant causative factor of this condition is reflux of the great saphenous vein (GSV), which is traditionally treated with surgical saphenofemoral ligation and stripping of the incompetent saphenous vein. In recent years, there have been significant advances in saphenous vein ablation using percutaneous tec

[Two cases of rhabdomyolysis with a different clinical course].

Rhabdomyolysis is a syndrome provoked by the injury to skeletal muscles and the release of muscle cell contents into the plasma. The aetiology and clinical course are extremely variable. This is sometimes the reason of the diagnostic difficulties or even errors. Acute renal failure is the often and serious complication of rhabdomyolysis. The correct and early diagnosis of rhabdomyolysis is a key to successful treatment and prevention of the possible complications. Two cases of rhabdomyolysis with different aetiology and clinical course are presented in this paper. A 39-year-old man was admitte

[The importance of a complete diagnostic workup in patients with nontraumatic (partial) paraplegia].

In two women, aged 86 and 56 years, respectively, who suffered from back pain and loss of strength, and in a 55-year-old man who lost sensation and strength in his left leg, spinal-cord compression in connection with vertebral destruction was seen on radiological examination. When spinal-cord compression is the result of a local malignant tumour, the therapy often entails emergency radiotherapy. In the first two patients, histological examination revealed a solitary plasmocytoma and curative high-dose radiotherapy was applied. The third patient also had a lung tumour and received low-dose pall

Electrophysiologic correlates of weakness in L5/S1 radiculopathy.

INTRODUCTION: Management of patients with radiculopathy involves estimating the degree of physiologic and anatomic injury, and weighing that to predict the likely clinical course. OBJECTIVE: To determine whether low distal peroneal and tibial CMAP amplitudes correlate with weakness and fibrillations of functionally relevant muscles in L5/S1 radiculopathy (LSR). METHODS: We reviewed clinical and electrophysiologic data in 66 consecutive patients with LSR. RESULTS: A significantly greater number of patients with low peroneal CMAP amplitudes had weakness of L5 (p = 0.025) and S1 innervated leg

Autologous vein-covered stent repair of a cervical internal carotid artery pseudoaneurysm: technical case report.

OBJECTIVE AND IMPORTANCE: Stenting of a cervical internal carotid pseudoaneurysm is presented using a stent covered with saphenous vein. This procedure resulted in immediate exclusion of the aneurysm and maintained patency of the carotid artery. CLINICAL PRESENTATION: A gunshot to the neck resulted in airway obstruction and respiratory arrest with neurological injury in a male patient. Angiographic investigation revealed an enlarging cervical internal carotid pseudoaneurysm, possibly the source of a cerebral embolism. Anticoagulation had to be stopped because of bleeding complications. A deci

Posttraumatic osseous tunnel formation causing sciatic nerve entrapment.

Sciatic nerve entrapment in an osseous tunnel has only been reported twice previously. We describe a 19-year-old man evaluated for left lower limb pain and weakness that began one and one half years after sustaining stab wounds to the left buttock and midline back near the T11 vertebrae. The patient had sciatica and demonstrated motor and sensory deficits on physical exam. Electrodiagnostic studies demonstrated a localized injury to the sciatic nerve in the proximal thigh. Radiographic studies of the left pelvis and femur showed an 8 to 10cm linear opacification overshadowing the left femoral

Anterior displacement of the tibial tuberosity in the treatment of chondromalacia patellae.

Anterior displacement of the tibial tuberosity combined with shaving of the diseased cartilage was used as treatment for chondromalacia patellae in 32 knees in 28 consecutive patients and in one patient with chondromalacia femoris. The patients were aged 17 to 57 years. One patient had earlier sustained a serious injury to the ligaments of the knee, resulting in instability. All patients except one were unfit for work before the operation. Postoperative extra-articular wound infection was noted in two knees and venous thrombosis occurred in three legs. The postoperative observation period vari
