Guideline for the management of neurogenic bowel dysfunction in spinal cord injury/disease.

INTRODUCTION: Almost all people with spinal cord injury/disease (SCI/D) suffer from neurogenic bowel dysfunction (NBD), with a considerable impact on quality of life. The Association of the Scientific Medical Societies in Germany (AWMF e.V.) guideline for NBD in SCI/D aims to provide practice-oriented support for the care of patients with NBD resulting from congenital or aquired SCI/D. The guideline describes the diagnosis and bowel management of NBD in people with SCI/D. Thus, treatment processes in acute medical care and rehabilitation as well as for lifelong aftercare are presented. METHOD

Helping Families Cope with the Severe Stress of Dravet Syndrome.

A child with Dravet syndrome shakes family life to the core. Dravet syndrome usually has three phases: (1) up to 1-1½ years: with episodes of febrile status epilepticus but normal development; (2) age 1½ to ~6-10 years: with frequent seizures of varying types, developmental stagnation, behavioural and sleep problems; (3) after ~10 years: improvement in seizures, deteriorating gait, intellectual disability but some developmental gains. Complete seizure control is rare-simply prescribing medication is inadequate to help families. Based on structured interviews with 24 families and confirmed by m

Accuracy of the all patient refined diagnosis related groups classification system in congenital heart surgery.

BACKGROUND: Administrative data are increasingly used to evaluate clinical outcomes and quality of care in pediatric congenital heart surgery (CHS) programs. Several published analyses of large pediatric administrative data sets have relied on the All Patient Refined Diagnosis Related Groups (APR-DRG, version 24) diagnostic classification system. The accuracy of this classification system for patients undergoing CHS is unclear. METHODS: We performed a retrospective cohort study of all 14,098 patients 0 to 5 years of age undergoing any of six selected congenital heart operations, ranging in co

District nurses' attitudes toward patient consent: the case of mechanical ventilation on amyotrophic lateral sclerosis patients: results from a French national survey.

PURPOSE: In France, a recent law emphasizes patient rights and prohibits unwanted therapies in end-of-life care. As end-of-life home care is increasing, we aimed to assess French district nurses' attitudes toward terminally ill patients' autonomy. MATERIALS AND METHODS: We used data from a nationwide telephone survey conducted in 2005 among a random sample of French district nurses (n = 602). Nurses' attitudes were assessed with a clinical case describing a patient (randomly defined as a male or a female aged 50 years) having amyotrophic lateral sclerosis (ALS) who urgently needs mechanical v

Electronic patient-reported outcome assessment at inpatient admission: a descriptive analysis of real-world data for 1423 radiotherapy patients.

BACKGROUND: Patient-reported outcomes (PRO) capture patients' self-reported and treatment-related experiences, including functional impairment, symptoms, and health-related quality of life (HRQOL). Although PRO have shown benefits in many studies and are recommended by guidelines, systematic PRO assessment is still rarely integrated into routine cancer care. This paper describes results of the long-term implementation of electronic PRO assessment (ePRO) in the routine workflow of a radiation oncology inpatient clinic by analyzing patients' HRQOL at admission. METHODS: Admission ePRO included

The journey of people accessing first trimester abortion in Aotearoa New Zealand: health consumers' perspectives.

INTRODUCTION: Aotearoa New Zealand (NZ) achieved abortion law reform in 2020, allowing people to choose abortion without legal restriction up to 20 weeks gestation. Abortion can now be provided by a range of health practitioners in a variety of settings. While some progress has been made, we are yet to see abortion care being provided within the full extent of the law. AIM: To describe the journey of people accessing first-trimester abortion care in NZ to identify how abortion services can be developed to deliver optimal first-trimester abortion care. METHODS: This was a qualitative study in

Cost-effectiveness and budget impact of breast cancer genetic profiling in Egypt.

Breast cancer is the most common cancer among women in Egypt and poses a growing public health challenge with significant socio-economic implications. While traditional diagnostic and treatment approaches remain central to care, advances in genetic testing offer new pathways for early detection, prevention and individualised treatment. Genetic testing, particularly for BRCA1, BRCA2 and other high-penetrance gene mutations, can identify women at elevated risk, enabling proactive surveillance, risk-reducing strategies and tailored therapeutic decisions. This article evaluates the cost-effectiven

Physiotherapy approaches for pain control in patients who are critically ill.

Pain is a significant challenge in critical care settings, affecting patient outcomes, recovery time, and quality of life. While pharmacological interventions remain the cornerstone of pain management in intensive care units (ICUs), they are associated with numerous adverse effects including respiratory depression, delirium, and prolonged ICU stays. To examine evidence-based physiotherapy approaches that can effectively complement traditional pain management strategies in patients who are critically ill. We conducted a comprehensive literature review of physiotherapy modalities used for pain c

Integrative Medicine for Cancer-Related Pain: A Narrative Review.

Cancer-related pain (C-RP) is a prevalent and debilitating concern among patients with cancer, with conventional treatments limited in their ability to provide adequate relief, and by the adverse effects associated with their use. Complementary and integrative medicine (CIM) modalities have been shown to be potentially effective and safe for the treatment of pain and related symptoms, when used in conjunction with conventional medications and under medical supervision. An increasing number of oncology centers provide CIM within their conventional supportive and palliative care service, in an "

Endoscopic transnasal management of inverted papilloma involving frontal sinuses.

This presentation focuses on the challenges and practical issues faced each day by orphans of the HIV epidemic and the holistic care that can be provided, as they continue to grow from toddlerhood to adolescence and beyond. An HIV Research Trust Scholarship enabled me to spend quality time in a sub-Saharan African province worst hit by the HIV epidemic and to interact with local experts and learn from mutual clinical experience. It was an immensely useful exercise as the clinical spectra of the diseases are very similar to ours and they have ongoing active research programs very relevant to ou
