Pimicotinib versus placebo for tenosynovial giant cell tumour (MANEUVER): an international, randomised, placebo-controlled, phase 3 trial.

BACKGROUND: Tenosynovial giant cell tumour (TGCT) is a rare, locally aggressive neoplasm that affects otherwise healthy adults. There are few systemic treatment options, highlighting an unmet need. We report the results of part 1 of the MANEUVER trial, which aimed to evaluate the efficacy and safety of pimicotinib, a highly selective, potent, colony-stimulating factor-1 receptor inhibitor, in patients with TGCT. METHODS: MANEUVER is a randomised, placebo-controlled, phase 3 study done in 40 specialised hospitals in Asia, Europe, and North America. Patients aged 18 years and older with unresec

Purkinje Cell Loss and Changes in Basket Cell Morphology in Essential Tremor and Other Cerebellar Degenerative Diseases: A Postmortem Study of 332 Brains.

Essential tremor (ET) is among the most prevalent neurological diseases. Postmortem studies have demonstrated Purkinje cell (PC) loss in ET. In addition, PCs and their micro-circuitry in the cerebellar cortex-including basket cells and climbing fibers-display several characteristic pathological features in these studies. While several studies in ET have focused on the interactions between PCs and climbing fibers, less has been written about basket cells and their interactions with PCs (i.e., formation of empty baskets and hypertrophy of basket cell axonal processes around the PC soma). These c

Genetic, clinical, and biological risk factors associated with optic nerve thinning in sickle cell disease.

OBJECTIVES: To determine whether adults with sickle cell disease (SCD) without glaucoma exhibit subclinical structural optic neuropathy (SON) on optical coherence tomography (OCT) and to identify genetic, clinical, and biologic correlates of optic nerve thinning. METHODS: Monocentric, retrospective cross-sectional study of 185 eyes from 96 adults with SCD (116 eyes/59 HbSS; 69 eyes/37 HbSC) and 40 eyes from 20 controls. Spectral-domain OCT measured Bruch's membrane opening-minimum rim width (BMO-MRW), peripapillary retinal nerve fibre layer (RNFL), and macular ganglion cell complex (GCC). SON

Functional Characterization of a Novel Splice-Altering Intronic Variant in AMPD2 Associated with Pontocerebellar Hypoplasia Type 9.

BACKGROUND: A two-year-old female patient was referred to our clinic due to speech and gait disturbances, strabismus, vacant staring, truncal hypotonia, and spasticity in the extremities. The patient had a history of a complicated delivery resulting in perinatal asphyxia. Electrocardiogram (ECG), echocardiogram (ECHO) and abdominal ultrasound findings reported no abnormalities. Previously performed spinal muscular atrophy (SMA) test, chromosomal microarray and karyotype analyses yielded normal results. On clinical examination, facial dysmorphic features included: prominently low-set ears, stra

Efficacy and safety of mitapivat in adults with transfusion-dependent α-thalassaemia or β-thalassaemia (ENERGIZE-T): a double-blind, randomised, multicentre, placebo-controlled, phase 3 trial.

BACKGROUND: The absence of disease-modifying therapies for patients with α-thalassaemia and oral disease-modifying therapies for patients with β-thalassaemia has been a substantial unmet need in these patients. We assessed the efficacy and safety of mitapivat, an oral allosteric activator of pyruvate kinase, in adults with transfusion-dependent thalassaemia. METHODS: ENERGIZE-T is a global, double-blind, randomised, placebo-controlled, phase 3 trial, conducted across 19 countries in North America, Europe, Asia-Pacific, South America, and the Middle East. Patients aged 18 years or older with t

CD4+ cell count trends after common cancers in people with HIV: a multicohort collaboration.

OBJECTIVE: Whilst cancer is increasingly common in people with HIV, insight into immunological outcomes after cancer diagnosis (CaD) is limited. European guidelines recommend opportunistic infection prophylaxis (OIP) at CD4+ cell count of less than 200 cells/μl, and during chemotherapy/radiotherapy regardless of CD4+ cell count. We assessed CD4+ cell count trends and predictors of post-CaD immunosuppression across common cancers. DESIGN: Participants from D:A:D and RESPOND with Kaposi's sarcoma, non-Hodgkin lymphoma (NHL), lung, anal or prostate cancer and at least two CD4+ cell count measure

Adult obesity and risk of severe infections: a multicohort study with global burden estimates.

BACKGROUND: Adult obesity has been linked to specific infections, but evidence across the full spectrum of infectious diseases remains scarce. In this multicohort study with impact modelling, we examined the association between this preventable risk factor and the incidence, hospitalisations, and mortality of 925 bacterial, viral, parasitic, and fungal infectious diseases, and estimated their global and regional attributable impact. METHODS: We used pooled data from two Finnish cohort studies and repeated analyses in an independent population from the UK Biobank. BMI was assessed at baseline

Impact of introducing RTS,S/AS01E malaria vaccine on mortality in young children in Ghana, Kenya, and Malawi: an observational evaluation of a cluster-randomised implementation programme.

BACKGROUND: Malaria vaccines have been added to immunisation schedules in 25 sub-Saharan African countries, with the expectation that deaths in young children would be prevented. The introduction of the RTS,S/AS01E malaria vaccine (RTS,S) in Ghana, Kenya, and Malawi in 2019 was evaluated over 4 years to show the impact on mortality in young children and to monitor severe malaria admissions, vaccine uptake, and safety. Favourable evidence on safety and impact on severe malaria admissions during the first 2 years contributed to WHO's recommendations on malaria vaccines. Here, we report the prima

Next step after PrEP: HIV prevention pathways among former PrEP users disengaged from care.

OBJECTIVES: To gain insight into HIV prevention pathways of individuals following pre-exposure prophylaxis (PrEP) care disengagement at the Center of Sexual Health in Amsterdam (CSHA), the Netherlands, to improve prevention services. DESIGN: Cross-sectional study. METHODS: CSHA clients disengaged from PrEP care were asked to complete an online survey (January-April 2025). The survey included open-ended, closed-ended, and multiple-choice questions on PrEP care discontinuation reasons at CSHA, continuing PrEP care elsewhere and sexual behavior. This was complemented by routine CSHA data. Data

Evolving drivers of preexposure prophylaxis discontinuation among US MSM, 2018-2024.

OBJECTIVE: To examine temporal trends in reasons for stopping preexposure prophylaxis (PrEP) among MSM from 2018 to 2024, including how reasons co-occur and vary by demographics. DESIGN: Pooled repeated cross-sectional analysis of seven annual waves of the American Men's Internet Survey (AMIS), a national behavioral survey of US cisgender MSM (N = 54 612). METHODS: Among 3074 noncurrent PrEP users who reported reasons for stopping, we collapsed checklist items into six thematic categories and estimated adjusted prevalence by survey year using multivariable logistic regression with g-computat

Efficacy and safety of upadacitinib in adults and adolescents for treatment of non-segmental vitiligo (Viti-Up): results of two phase 3 randomised controlled studies.

BACKGROUND: Non-segmental vitiligo is an autoimmune inflammatory disease characterised by loss of melanocytes leading to skin depigmentation. No systemic therapies have been approved, and additional therapeutic options are needed. Here, we report the efficacy and safety of upadacitinib, a selective once-daily Janus kinase inhibitor, in adults and adolescents with non-segmental vitiligo. METHODS: Viti-Up includes two phase 3, global, multicentre, randomised, double-blind, placebo-controlled studies conducted at 138 institutions in 18 countries in Asia, Europe, North America, and South America.

Post-Infectious Cerebellitis: A Systematic Review of Aetiology, Clinical Presentation, Treatment, and Outcomes.

Post-infectious cerebellitis is an immune-mediated cerebellar inflammatory syndrome that follows a preceding infection (or, rarely, vaccination) after a latent interval. It spans a spectrum from benign post-infectious/acute cerebellar ataxia (ACA) through more severe acute cerebellitis (AC) with leptomeningeal enhancement and mass effect, to rare, life-threatening acute fulminant cerebellitis (AFC) with hydrocephalus risk. Evidence is largely limited to individual case reports and small case series, and no prior review has systematically pooled patient-level data across etiologies. A PRISMA-gu

Low-dose yellow fever vaccination in infants: a randomised, double-blind, non-inferiority trial.

BACKGROUND: WHO recommends fractional dose vaccination to address yellow fever vaccine shortages during outbreaks. In adults, a 500 IU dose has recently been shown to be non-inferior to the full standard dose, but the minimum effective dose for children is unknown. METHODS: We conducted a randomised, double-blind, non-inferiority trial at two centres in Kenya and Uganda, including infants aged 9-12 months with no previous yellow fever vaccination or infection. Participants were randomly assigned 1:1 in blocks of variable sizes of four, six, or eight to receive either the standard dose (>13 00

Immunogenicity of fractional one-fifth and one-half doses of 17DD yellow fever vaccine compared with full dose in children aged 9-23 months in Uganda: a phase 4, single-blind, randomised clinical trial.

BACKGROUND: Fractional doses of yellow fever vaccines have been shown to be safe and immunogenic in randomised controlled trials in adults and children. However, little evidence still remains in children younger than 2 years. We assessed immunogenicity of one-fifth (0·1 mL) and one-half (0·25 mL) fractional doses compared with full doses of 17DD yellow fever vaccine in children aged 9-23 months in Kampala, Uganda. METHODS: This phase 4, single-blind, randomised clinical trial evaluating fractional dose 17DD yellow fever vaccine among children was conducted at three public-sector health facili

A Rare Case of Anti-Yo Positive Paraneoplastic Limbic Encephalitis Associated with Thyroid-Parathyroid Adenoma.

Paraneoplastic neurological syndromes (PNS) are typically associated with malignant neoplasms. The anti-Yo antibody is classically linked to paraneoplastic cerebellar degeneration; its coexistence with limbic encephalitis is extremely rare. We report the first case of anti-Yo–positive limbic encephalitis associated with a benign thyroid–parathyroid adenoma. A 59-year-old male presented with epileptic seizures, left upper limb weakness, dysarthria, and confusion. Serum anti-Yo antibody was positive. Cranial MRI revealed diffusion restriction in the cerebellar hemispheres and middle cerebellar p
