Amelioration of naloxone-precipitated opioid withdrawal symptoms by peripheral administration of the enkephalinase inhibitor acetorphan.

The effects of 60 min pretreatment with the enkephalinase inhibitor acetorphan were assessed on naloxone-precipitated (2.5 mg/kg IP) abstinence in chronically morphinized rats. In addition, the antinociceptive activity of the compound was investigated in mice. Intraperitoneal injection (50 mg/kg) in rats attenuated some aspects of the opioid withdrawal syndrome such as burrowing, wet dog shakes, squeal on touch hostility, tachypnoea, ptosis and rough hair, whereas jumping and escape behaviour were significantly increased in acetorphan-treated animals. No effect was observed on withdrawal hypot

Detection of heroin intake in patients in substitution treatment using oral fluid as specimen for drug testing.

BACKGROUND: Detection of heroin use is among the major tasks for drug testing and can be best performed by using 6-acetylmorphine as the target analyte. This study was performed to document analytical findings in oral fluid after OF heroin intake. METHODS: The samples were from routine drug testing of patients in substitution treatment. The analytical investigation was made with a forensic accredited liquid chromatography-tandem mass spectrometry method. RESULTS: Out of 2814 samples, from 1875 patients, sent for routine drug testing, 406 contained one or more opiate in the drug screening whe…

Thoracic Epidural Analgesia: Do the Alternatives for Abdominal Surgical Pain Control Measure up?

Thoracic epidural analgesia (TEA) is well-known as the gold standard for perioperative somatic and visceral analgesia in abdominal surgery. Recently, there has been a shift away from TEA with the emergence of alternative pain control techniques such as fascial plane blocks (FPBs). Proponents of these alternatives often note the high failure rate of TEA, which can reach up to 25%. The alternatives to TEA are not a panacea. Important techniques have emerged to improve upon the reliability of TEA. In cases where TEA is contraindicated or impractical, alternatives including intrathecal morphine, p…

Patient-Controlled Therapy with Intravenous Oxycodone in Breathlessness due to Advanced Cancer: A Case Report.

Dyspnoea is a debilitating symptom in medicine, especially in palliative care. Opioids are the pharmacological agents of choice in the treatment of dyspnoea in palliative medicine. Morphine is the best-studied opioid, and recent literature on oxycodone is encouraging. In refractory cases, opioid infusion and palliative sedation may have to be used. We present a case that used oxycodone in a patient-controlled device specifically for dyspnoea and its effects in relieving dyspnoea in a fast and timely manner. This helped in meeting the demands of the patient and relieving suffering rapidly with …

Quantitation of Bupropion in Urine by Liquid Chromatography-Tandem Mass Spectrometry.

Bupropion is a medication commonly used to treat depression and help people quit smoking. However, it has the potential to be misused or diverted, so it is important to have accurate and sensitive methods to detect it in bodily fluids. We present a fast and dependable quantitative LC-MS/MS method for measuring bupropion levels in urine. This method involves enzymatic hydrolysis and separation using a reversed-phase C18 column, where the effectiveness of the hydrolysis is monitored using morphine-3-b-D-glucuronide. The analysis is conducted using an AB Sciex 3500 mass spectrometer with dual-cha…

Mastocytosis in Pregnancy.

Mastocytosis is a rare neoplastic disorder of the mast cell lineage resulting in unregulated proliferation and activation of mast cells. Symptoms worsen in about one-third of pregnant patients. Treatment focuses on management of symptoms with antimediator therapy (H1 & H2 antihistamines, glucocorticoids, and epinephrine, if required). Medication selection requires care during labor and delivery. Although it is generally considered safe to use a medication patient tolerated before, some common medications may need to be avoided or used with caution (eg, codeine, morphine, nonsteroidal antiinfla…

Reduction of Drug-Drug Interaction Risk; CDC Guidelines Influence on Opiate Benzodiazepine Prescribing.

We examined the results of 1.3 million drug tests performed on patients being monitored for compliance with pain medications and substance abuse rehabilitation to determine if the 2016 CDC prescribing guidelines had any impact on opiate benzodiazepine use. We observed that the combination of the opiate drugs morphine, oxycodone, and hydrocodone with the benzodiazepine metabolites oxazepam, alphahydroxyalprazolam, and 7-aminoclonazepam showed many patients were on a combination of these drugs. This ranged from approximately 9 to 16%. There was considerable variability between opiate drug pairs,…

Bilateral continuous posterior quadratus lumborum block for analgesia after open abdominal surgery: A prospective case series.

The quadratus lumborum (QL) block provides analgesia to the abdominal wall while sparing the side effects of neuraxial blocks. We describe a case series of eight patients treated with a continuous infusion of local anesthetic via bilateral posterior QL catheters infusion block for analgesia after abdominal surgeries. We found that the median duration of the procedure was 26 min and the median opioid consumption over the first postoperative 72 h was 110 mg of morphine equivalents. The bilateral continuous posterior QL block is a feasible analgesic intervention and can be considered as a compone…

Modeling prenatal opioid exposure in animals: Current findings and future directions.

The past decade has seen a drastic rise in the number of infants exposed to opioids in utero. It is unclear what lasting effect this exposure may have on these children. Animal models of prenatal opioid exposure may provide insight into potential areas of vulnerability. The present review summarizes the findings across animal models of prenatal opioid exposure, including exposure to morphine, methadone, buprenorphine, and oxycodone. Details regarding the drug, doses, and duration of treatment, as well as key findings, are summarized in tables with associated references. Finally, significant ga…

Is low BMI associated with specific drug use among injecting drug users?

Body mass index (BMI) of a sample of people who regularly inject drugs (N = 781) was examined to gauge the impact of specific types of drug use. Cross-sectional interviews were undertaken in 2010 as part of a national monitoring program funded by the Australian Government. Latent class analysis identified three groups of drug users, with heroin users at 3.4 times the risk of being underweight compared with amphetamine users, and amphetamine users were at almost twice the odds of being obese compared with lower level morphine users. Nutrition should play a part in harm minimization.

[Treatment and management of intractable chronic pain patients--preface and comments].

Chronic intractable pain is well known to be very difficult in management. The mechanisms of intractable chronic pain are complicated. Therefore, such a patient is often involved with multiple pain mechanisms and needs multiple drugs which have different types of analgesic actions. This new insight into pain mechanisms is important in the diagnosis and treatments of chronic intractable pain patients and the concept of balanced analgesia is widely accepted. Therefore, the evaluation of many factors which modulate the pain behavior is important. Opioids such as morphine and other analgesics are …

The discovery of alkaloids.

This paper presents the history of the discovery of the first alkaloids. Isolation of alkaloids is connected with the study of the active principles of medicines of plant origin, for example opium and cinchona bark. Sertürner described morphine as a plant alkali and claimed that it was capable of neutralizing free acids yielding salts. The recognition of alkaloids as a new class of compounds was an important step at that time because of the dogmatic denial of the possible existence of plant bases. Isolation of alkaloids is a significant event from the point of view of chemistry, physiology and…

Nicotine cue in rats: effects of central administration of ganglion-blocking drugs.

In rats trained to discriminate nicotine from saline, a single intraventricular injection of a small dose of the quaternary ganglion-blocking drug chlorisondamine blocked the response to nicotine for four weeks. pentolinium was only weakly active and hexamethonium was inactive as a nicotine antagonist under the conditions used, even in doses that were just below those producing myoclonic jerks. Chlorisondamine had no blocking effect in rats trained to discriminate the non-nicotinic drugs midazolam or morphine from saline. Intraventricular injections of chlorisondamine have a specific and unusu…

Opiate agonists inhibit neurogenic plasma extravasation in the rat.

Neurogenic plasma extravasation (NPE) evoked by antidromic stimulation of the saphenous nerve was inhibited by more than 50% by i.p. administration of the synthetic enkephalin analogue (D-Met2, Pro5)-enkephalinamide, 0.3-10 mg/kg and, to a lesser extent, by morphine. The effect of enkephalinamide (10 mg/kg) was reversed by naloxone (1 mg/kg, s.c.). Evidence is presented that the effect of enkephalinamide was not secondary to haemodynamic changes or alteration of mast cell function. It is suggested that opiate agonists inhibit NPE through specific opiate receptors, located peripherally on senso…

The diagnosis and treatment of functional disorders of the biliary tract.

An approach to the diagnosis and treatment of patients with presumed functional disorders of the biliary tract (biliary dyskinesia) is described. The current diagnostic criteria are pain compatible with biliary pain in the absence of gallstones and other organic gastrointestinal disease, or other disorders which might produce abdominal pain, together with reproduction of the patient's symptoms by cholecystokinin, or morphine, or both. Other diagnostic methods are described together with their limitations. The results of operation in 38 of 45 patients seen in this Unit during the past six years…

Apomorphine-induced pecking in pigeons.

Apomorphine produced persistent pecking in pigeons, the latent period, intensity and duration of which were related to the dose. The ED50 was estimated as 78.1+/-11.1 mug./kg. On chronic administration of apomorphine there was a significant decrease in latent period and weight which quickly returned to normal on stopping the drug. No conditioning and no tolerance were observed. The uncertain emetic effect of apomorphine in pigeons has been confirmed. Ten other centrally acting agents tested (caffeine, cocaine, 5-hydroxytryptamine, lysergic acid diethylamide, methamphetamine, morphine, nalorphi…

Fetal and Infant Effects of Maternal Opioid Use during Pregnancy: A Literature Review including Clinical, Toxicological, Pharmacogenomic, and Epigenetic Aspects for Forensic Evaluation.

The two primary classes of opioid substances are morphine and its synthetic derivative, heroin. Opioids can cross the placental barrier, reaching fetal circulation. Therefore, at any gestational age, the fetus is highly exposed to pharmacologically active opioid metabolites and their associated adverse effects. This review aimed to investigate all the studies reported in a timeframe of forty years about prenatal and postnatal outcomes of opioid exposition during pregnancy. Clinical and toxicological aspects, as well as pharmacogenetic and epigenetic research focusing on fetal and infant effect…
